Purpose Prognostic factor analyses have proven useful in predicting outcome in patients with newly diagnosed malignant glioma. Similar analyses in patients with recurrent glioma could affect the design and conduct of clinical trials substantially. Patients and Methods Between 1995 and 2002, 333 adults with recurrent gliomas were enrolled onto 10 phase I or II trials of systemic or local therapy. The studies had similar inclusion criteria and were conducted within the New Approaches to Brain Tumor Therapy CNS Consortium. Ninety-three percent of the patients have died. Cox proportional hazards (PH) regression and recursive partitioning analysis (RPA) were performed to identify prognostic factors. Results Factors associated with an increased risk of death were increased age, lower Karnofsky performance score (KPS), initial and on-study histologies of glioblastoma multiforme (GBM), corticosteroid use, shorter time from original diagnosis to recurrence, and tumor outside frontal lobe. The final PH model included initial histology of GBM (relative risk [RR] = 2.01), 10-year increase in age (RR = 1.23), KPS less than 80 (RR = 1.54), and corticosteroid use (RR = 1.49). RPA resulted in seven classes. Median survival time was poorest in non-GBM patients with KPS less than 80 or GBM patients, age ≥ 50 years, corticosteroid use (4.4 months; 95% CI, 3.6 to 5.4 months); median survival was best in patients with initial histology other than GBM with KPS ≥ 80 and tumor confined to the frontal lobe (25.7 months; 95% CI, 18.7 to 52.5), and was 7.0 months (95% CI, 6.2 to 8.0 months) for all patients. Conclusion Initial histology, age, KPS, and corticosteroid use are prognostic for survival in recurrent glioma patients. To allow comparisons across phase II trials, enrollment criteria may need to be restricted.

Crop protection on major crops is now required to follow the principles of integrated pest management so the timing and accuracy of any application of a pesticide or biopesticide has to be more precise to minimize adverse effects on non-target species. The development of UAVs (unmanned aerial vehicles) provides a means of providing a more targeted application of the correct dose, especially by using formulations that are more persistent, thus minimizing loss of spray in areas subject to rain. Avoiding use of too high a dosage allows greater survival of natural enemies and reduces the selection pressure for pests becoming resistant to specific modes of action. The downward flow of air from a UAV should also provide better distribution and impaction of droplets within a crop canopy, reduce soil impaction caused by taking heavy loads of spray applied with 200 l ha–1 of water, and allow treatments when fields are too wet to access with ground equipment. In Asia, many smallholder farmers are using a drone in preference to using a knapsack sprayer. According to Matthews, it has been shown that ULV spraying can be effective, but it needs a narrow droplet spectrum with the droplets remaining stable and not shrinking to become too small. Formulation research can reduce the volatility of the spray, hence the success of oil-based sprays. However, instead of petroleum-based oils, there is a chance to develop vegetable oil carriers with micro-sized particle suspensions to deliver low toxicity pesticides in droplets that can be deposited within the crop and not drift beyond the crop boundary. Oil deposits will be less prone to loss after rain so less should be lost in neighbouring ditches and water courses, especially as rainfall patterns are forecast to change. More studies are needed to evaluate the swath for deposition, buffer zones, formulation, nozzle selection, to guide future specific legislation for UAV applications.

<p>The design and fabrication of Si-based multi-functional nanomaterials for biological and biomedical applications is an active area of research. The potential benefits of using Si-based nanomaterials are not only due to their size/surface-dependent optical responses but also the high biocompatibility and low-toxicity of silicon itself. Combining these characteristics with the magnetic properties of Fe<sub>3</sub>O<sub>4</sub> nanoparticles (NPs) multiplies the options available for real-world applications. In the current study, biocompatible magnetofluorescent nano-hybrids have been prepared by covalent linking of Si quantum dots to water-dispersible Fe<sub>3</sub>O<sub>4</sub> NPs <i>via</i> dicyclohexylcarbodiimide (DCC) coupling. We explore some of the properties of these magnetofluorescent nano-hybrids as well as evaluate uptake, the potential for cellular toxicity, and the induction of acute cellular oxidative stress in a mast cells-like cell line (RBL-2H3) by heat induction through short-term radio frequency modulation (10 min @ 156 kHz, 500 A). We found that the NPs were internalized readily by the cells and also penetrated the nuclear membrane. Radio frequency activated nano-hybrids also had significantly increased cell death where > 50% of the RBL-2H3 cells were found to be in an apoptotic or necrotic state, and that this was attributable to increased triggering of oxidative cell stress mechanisms. </p>

Object. The purpose of this study was to clarify the effectiveness of gamma knife radiosurgery (GKS) in achieving a partial or complete remission of so-called radioresistant metastases from renal cell carcinoma (RCC) and to propose guidelines for optimal treatment Methods. During a 5-year period, 29 patients (19 male and 10 female) with 92 brain metastases from RCC underwent GKS. The median tumor volume was 4.7 cm3 (range 0.5–14.5 cm3). Fourteen patients (48%) also underwent whole-brain radiotherapy (WBRT) before GKS, and two patients (6.8%) after GKS. The mean GKS dose delivered to the 50% isodose at the tumor margin was 16.8 Gy (range 13–30 Gy). All cases were categorized according to the Recursive Partitioning Analysis (RPA) classification for brain metastases. Univariate analysis was performed to determine significant prognostic factors and survival. The overall median survival was 7 months after GKS treatment. Age, sex, Karnofsky Performance Scale score, and controlled primary disease were not predictors of survival. Combined WBRT/GKS resulted in median survival of 18, 8.5, and 5.3 months for RPA Classes I, II, and III, respectively, compared with the median survival 7.1, 4.2, and 2.3 months for patients treated with WBRT alone. Conclusions. These results suggest that WBRT combined with GKS may improve survival in patients with brain metastases from RCC. Furthermore, this improvement in survival was seen in all RPA classes.

Triterpenoids are natural compounds with a large variety of biological activities such as anticancer, antiviral, antibacterial, antifungal, antiparazitic, antiinflammatory and others. Despite their low toxicity and simple availability from the natural resources, their clinical use is still severely limited by their higher IC50 and worse pharmacological properties than in the currently used therapeutics. This fact encouraged a number of researchers to develop new terpenic derivatives more suitable for the potential clinical use. This review summarizes a new approach to improve both, the activity and ADME-Tox properties by connecting active terpenes to another modifying molecules using click reactions. Within the past few years, this synthetic approach was well explored yielding a lot of great improvements of the parent compounds along with some less successful attempts. A large quantity of the new compounds presented here are superior in both activity and ADME-Tox properties to their parents. This review should serve the researchers who need to promote their hit triterpenic structures towards their clinical use and it is intended as a guide for the chemical synthesis of better drug candidates.
