Background: Breast pain among women, with or without lump is common complaint and a cause of signicant anxiety and fear of breast cancer. Breast feeding is additionally one of the reasons for non-cyclic pain, brocystic breast disease is otherwise called broadenosis. Material and Methods:This is prospective study in the Department of General Surgery, Indira Gandhi Institute of Medical Sciences, Patna, Bihar. Conclusion: Centchroman is a selective anti estrogen effective in the treatment of brocystic disease. Centchroman is effective in reducing the sizes of breast lumps, providing relief from mastalgia and in curing cyclical and noncyclical breast pain in the premenopausal women.

Cerebrovascular disease include most common devastating disorders :ischaemic stroke and haemorrhagic stroke.The incidence of cerebrovascular disease increases with age and the number of strokes is projected to increase as the elderly population grows.A stroke is the second leading cause of death worldwide. Some of the Indian studies have shown a stroke prevalence of 471.58/100000 population. OBJECTIVE:To study the clinical presentation, risk factors,neurological presentation,pattern of brain strokes,areas of brain affected as per CT/MRI Brain scan findings in a newly admitted patients in GNRC Institute of Medical Sciences, North Guwahati,Assam. METHODS:This is a prospective study included all new patients with stroke admitted in ICU and Ward under Neurology Department of GNRC institute of Medical Sciences ,North Guwahati,Assam,from 1st August,2018 to 31st July ,2019. RESULTS: The cerebrovascular strokes are more common in males (64.4%) than females (35.6%). Most common age group was 58-67 years (28.29%).Most common clinical feature was hemiplegia (71.21%).Most common risk factor was Hypertension (89.51%) followed by diabetes mellitus(51.70%) . Most common type of stroke was haemorrhagic (60.24%) followed by ischaemic (39.75%). In stroke infarct most common involved areas were parietal (13.65%). In hemorrhagic stroke most common site was basal ganglia (21.95%) followed by thalamus (10.97%). CONCLUSION: Males were more commonly affected with cerebrovascular accident with hypertension was the most common risk factor among the stroke patients and most common type of stroke was haemorrhagic.

Background: Though benign breast diseases are very common, not many studies have focussed on this entity. In Kashmir valley, no major study has been undertaken before, to look into the profile of the benign breast diseases in women.Methods: This prospective study was conducted in the Postgraduate Department of General Surgery of SMHS Hospital from October 2012 to September 2014. The patients with features of benign breast diseases were subjected to detailed history, clinical examination and investigations. Analysis was done using this data.Results: 80% of the benign breast lesions presented in the second and third decades of life. Mean age was 24.46 years. Most common benign breast lesion was fibroadenoma. Lump in the breast was the predominant symptom. Duration of symptoms mostly ranged from 1 to 6 months. There was a slight preponderance of lesions in the right breast. Most of lesions presented in upper outer quadrant.  81.39% of the patients had only a solitary lump.76.74% of lumps had a size of 2 to 5cm. Clinicopathological correlation in case of fibroadenoma, showed 60% sensitivity, 75% specificity, PPV=85.71% and NPV=42.85%. Cytohistological correlation in case of fibroadenoma, showed 85% sensitivity, 87.5% specificity, PPV=94.4% and NPV=70%. 38 cases (38%) were managed conservatively and 62 cases (62%) were managed surgically.Conclusions: Results grossly similar to other parts of the word were obtained. Fibroadenoma was the most common benign lesion encountered followed by fibroadenosis and breast abscess. Majority of breast lumps were painless. Conservative approach for fibroadenoma is acceptable option in the adolescents.

Background: Cholecystitis can be divided into simple and severe cholecystitis. Untreated simple cholecystitis resolves within 7–10 days if it does not progress to more severe cholecystitis. Aims and objectives were to evaluate whether neutrophil-to-lymphocyte ratio can differentiate between simple cholecystitis and severe cholecystitis. To evaluate role of NLR as a prognostic indicator.Methods: The source of data for our study will be patients admitted in the department of general surgery diagnosed with acute cholecystitis in Konaseema Institute of medical sciences and research foundation, Amalapuram. All patients between 15 to 70 years of age with a clinical diagnosis of acute cholecystitis confirmed with histopathology study. Results: With an NLR value of 4.35, the sensitivity and specificity were 67% and 87%, respectively. Therefore, we considered 4.35 as the cutoff value, and divided the patient population into two groups: those with preoperative NLR values below 4.35 (n=50) and those with values equal to or greater than 4.35 (n=15). 53.33% of higher NLR group patients had severe cholecystitis compared to only 8 % of patients in lower NLR group (p<0.05).  Conclusions: It was seen that the patients with cholecystitis can be divided into low risk (NLR<4.35) and high risk (NLR≥4.35) groups for severe cholecystitis as per the NLR value at admission.
