Aim. Optimization of an endodontic retreatment by performing the following tasks, including the selection of an optimal and safe solvent.Materials and methods. In order to choose the most suitable solvent for the problem, an experiment was conducted with clove, mint, eucalyptus, orange, and grapefruit oils. The surface tension coefficient was measured by the Rebinder apparatus. In order to justify the choice of an effective essential oil, as well as the dissolving ability and the absence of solvent and debris after irrigation with sodium hypochlorite in the root canal, a tube-shaped electron scanning microscopy was performed.Results. Grapefruit oil has the most dissolving activity (p < 0.05). In addition to the solubility of the filler and siller, grapefruit essential oil is able to dissolve the smear layer.Conclusion. Optimization of the endodontic retreatment with root canals previously filled with gutta-percha, is possible using essential oils, however, a grapefruit oil is the most effective.

Endodontic retreatment is an attempt to reachieve a healthy periapical after it was previously been carried out an inadequate or reinfected endodontic retreatment that has been filled due to the leakage of apical and coronal. One stage in this retreatment is an uptake of obturasi material using hedstrom file (H-file). The solvent material used in this endodontic retreatment is xylol. Case report. A 22-year-old male patient came to the Clinic of Conservative Dentistry Dental Hospital Prof. Soedomo, Faculty of Dentistry, Gadjah Mada University (UGM) wanted to treat his mandibular second right premolar (45) that has been painful since two months ago. The tooth had root canal treatment one year ago. The pain suddenly appears and disappears as soon as the patient consumes painkillers. Dental preoperative (periapical) radiograph tooth 45 showed a picture of gutta percha root canals and radiolucent images at the tip of the root of the tooth. Diagnosis of tooth 45 is non-vital tooth after root canal treatment accompanied by apical, symptomatic periodontitis. The operator performed a root canal treatment by dismantling the restoration on the occlusal part of tooth 45 and taking gutta percha using a hedstrom file followed by treatment of the root canal, crown lengthening, installation of individual formable fiber post and porcelain fused to metal crown. Conclusion. Retreatment of root canal treatment with non-surgical methods still can be conducted effectively and obtain good results with the final treatment results in smaller lesions in the apical portion of tooth 45.

The present communication describes the endodontic retreatment of a rare four-rooted maxillary second molar. A 25-year-old patient was referred to our dental practice requesting an apicoectomy because of continuous and permanent pain reaction six months after the first endodontic treatment. The sent radiograph demonstrated three filled root canals (one mesial, two distal) and four radiographically superimposing roots (two mesial, two distal). Due to the diagnosed chronic apical periodontitis and based on the visible untreated root canal, we decided to endodontically re-treat the quadrangular tooth against the referred apicoectomy. In the first session, only the previously untreated mesiopalatal root canal was mechanically prepared and filled with a corticosteroid- and tetracycline-containing paste. After two symptom-free weeks, the gutta-percha was removed from the other canals and calcium hydroxide was applied. Another two weeks later, the four root canals, whose orifices were “irregular quadrilateral” shaped on the pulp chamber floor representing Versiani Type A, were obturated. After the verification of this rare anatomy by cone beam computed tomography, the tooth was classified as Christie’s radicular type II. One characteristic of this type is parallel running buccal and palatal roots, which caused a radiographic superimposition and probably led to the endodontic mistreatment in the first case.

AbstractIn case of failed root canal treatment endodontic retreatment of the root canal system is necessary. The aim of the present study was to compare the gutta-percha removal with Protaper retreatment files (Dentsply Maillefer, Ballaigues, Switzerland), K3 (SybronEndo) and Hero Shapers (MicroMega, Besancon, France). Method: Thirty freshly extracted human single rooted teeth were prepared with ProTaper rotary instruments up to file F3 andfilled with 30 # GP cones and AH plus sealer. The teeth were then stored for 1 week. The teeth were divide into 3 retreatment groups (n=10). The gutta-percha was removed using Protaper retreatment files, K3 and Hero Shapers. Teeth were then evaluated using radiographs. Results: Comparing the ratio between clean canals Protaper and K3 showed better debris removal as compared to Hero shaper. But no significance difference was found.

ABSTRACT Objective To quantify the persistence of residues after endodontic retreatment related to the root canal obturation technique (lateral compaction or single cone) previously performed and the solvent used (xylol or Endosolv-R) in the cervical and root apical segment using digital radiograph. Materials and methods Forty root canals were prepared using the F2 instrument and filled with gutta-percha and AH Plus, 20 canals using #25 point by lateral compaction and the other 20 using F2 point by single cone. The root canals were transversely sectioned, and two segments (cervical and apical) were obtained. Radiographs were taken from the segment in buccolingual and mesiodistal direction, as well as the area that was originally filled with the root canal obturation was measured using ImageJ. After the specimens were set on special devices, the obturations were removed using the rotary system and divided into four groups (n = 10): G1 — lateral compaction and xylol; G2 — lateral compaction and Endosolv-R; G3 — single cone and xylol; and G4 — single cone and Endosolv-R. A new radiograph was taken and the area occupied by residues was measured. The persistence of residues was obtained from the obturation remaining in the root canal in percentage. The data were analyzed using Kruskal—Wallis test (p = 0.05). Results There was no difference among the examined groups, regardless of the radicular segment or the radiograph direction (p > 0.05). Conclusion The endodontic retreatment using xylol or Endosolv-R in the obturations previously performed by lateral compaction or single cone technique presented similar persistence of residues in the root canal, regardless of the radicular segment. Background: Complete obturation of the root canal with an inert filling material and creation of a fluid-tight seal are among the major goals of successful endodontic treatment. Methods: Extracted mandibular premolar teeth were decoronated, prepared in a crown-down fashion and irrigated with 2.5% sodium hypochlorite and 17% EDTA. The specimens were randomly assigned into 4groups (n=10/each) and obturated with gutta-percha and Resilon/Epiphanywith different sealers. Horizontal sections were obtained of the coronal, middle and apical third at levels of 14-mm, 8-mm and 2-mm from the apex respectively and were viewed under magnification. The area of obturating material, sealer and voids were calculated using Image Jsoftware. Statistical comparisons between the Resilon and gutta-percha groups were made with Kruskal-Wallis Test and intergroup comparisions were made by Mann-Whitney U test with Bonferroni correction. Results: The results of this study showed that root canals obturated with gutta-percha or Resilon with Epiphany as sealer had significantly less area of sealer plus voids and voids alone as compared to root canals obturated with gutta-percha and AH Plus as sealer or gutta-percha and GuttaFlow. Conclusion: From the results of this study it can be concluded that methacrylate resin-based sealer Epiphany will lead to a better obturation of the root canals and adhesive sealers hold a better prospective. The aim of this study was to evaluate the apical and periapical repair after root canal treatment of dogs' teeth with pulp necrosis and chronic periapical lesion using different root canal sealers. After periapical lesion induction, forty-four root canals of 3 dogs were submitted to biomechanical preparation using 5.25% sodium hypochlorite as an irrigating solution. A calcium hydroxide dressing (Calen PMCC) was applied for 15 days and the root canals were filled using the lateral condensation technique with gutta-percha points and Sealapex, AH Plus or Sealer Plus for sealing. After 180 days, the animals were sacrificed by anesthetic overdose and the obtained histological sections were stained with hematoxylin-eosin for optical microscopic analysis of the apical and periapical repair. The groups filled with Sealapex and AH Plus had better histological repair (p < 0.05) than the group filled with Sealer Plus, that had unsatisfactory results.

This article is a literature review aimed at presenting the general state of knowledge in manufacturing engineering and materials engineering to develop engineering materials applied for endodontic treatment as filling materials. Particular attention was paid to theoretical analyses concerning the selection of methods for developing and obturating root canals and discussing the results of experimental studies available in the literature. These activities aimed to compare the importance of the most commonly used endodontic filling materials based on gutta-percha or polymeric polyester materials, commonly known as resilon, respectively. The motivation to take up this complex, multi-faceted topic in this paper is the extent of caries, periodontal disease, and other oral diseases in 3 to 5 billion people, often affecting toothlessness and contributing to an increase in the index of disability-adjusted life years (number). Endodontics is an important element of the authors’ concept of Dentistry Sustainable Development (DSD) > 2020. The principles of qualifying patients for endodontic treatment are discussed. The introduction of rotary tools, especially manufactured from Nitinol alloy, to develop root canals and the latest thermohydraulic and condensation techniques for obturation guarantee progress in endodontics. The “Digital Twins” methodology was used, rooted in the idea of Industry 4.0 and the resulting idea of Dentistry 4.0, as well as knowledge management methods, to perform experimental research in virtual space, concerning methods of developing and obturating the root canal and assessing the tightness of fillings. Microscopic visualization methods were also used. Significant factors determining the effectiveness of endodontic obturation are the selection of the filling material and the appropriate obturation method. The generalized dendrological matrix of endodontic filling materials considers the criteria of mechanical strength influencing the potential root fracture and the quality of root canal filling. The results of the SWOT point analysis (strengths and weaknesses, opportunities, threats) were also compared. For both filling materials, the weaknesses are much less than the strengths, while the threats are slightly less than the opportunities for the gutta-percha-based material, while for resilon the opportunities are much smaller than the threats. It requires the application of an appropriate development strategy, i.e., MAXI-MAXI in the case of a filling material based on gutta-percha and MAXI-MINI in the case of resilon. Therefore, the analysis of these experimental data does not indicate the real competitiveness of resilon for the gutta-percha-based material. This material deservedly maintains its strong position as the “Gold Standard of Endodontics”.

It is a well-recognised fact that incomplete cleaning, shaping and obturation of root canals will lead to endodontic failure. Mandibular second premolars usually have a single root and a single root canal. The presence of four separate roots and four root canals is quite rare. Hence, a comprehensive knowledge about the normal canal configuration along with its variations becomes an indispensable pre-requisite to achieve the success of endodontic treatment. Authors hereby present a case of a 26-year-old male patient and describe its succesful treatment of the mandibular second premolar with four roots and four root canals. The clinical implications of this paper aim at establishing an accurate diagnosis of the root canal system using diagnostic aids such as angulated radiographs and making use of advanced endodontic instruments for successful retreatment of endodontic therapy (C+ files and NiTi rotary endodontic instruments).

The obturation quality of GuttaFlow Bioseal in curved root canals is not commonly investigated although there has been a current approach toward utilizing this material in extracted molars in recent years. This study assessed the obturated surface area, extrusion of root filling material beyond the apical foramen and duration of obturation procedure in curved root canals using monocone obturation technique. Access cavity was prepared in 20 human mandibular molars. Root canals with curvature of more than 10 as determined according to Schneider’s method were included. Samples were prepared using Hyflex CM rotary files and divided into two groups (n=10): Group 1 [gutta-percha cone and GuttaFlow Bioseal] and Group 2 [gutta-percha cone and RoekoSeal Automix root canal sealer]. The duration of obturation procedure was recorded and obturation radiographs were taken. Samples were bisected and the mesial roots were sectioned horizontally to obtain 3 root segments; apical, middle and coronal. All resected roots were mounted on brass stubs, sputter-coated with thin platinum coating and observed under scanning electron microscope (SEM) at 70x magnification. The SEM images were transferred to the SketchAndCalc Area Calculator software. No statistically significant differences in the obturated surface area and extrusion of root filling material were observed between Group 1 and 2, irrespective of the status of root canal curvature. Duration for obturation in severe root canal curvatures between Group 1 and 2 were statistically significant. Obturated surface area and extrusion of root filling material were not affected by the root canal curvature, however duration for obturation using GuttaFlow Bioseal in severe root canal curvatures was slightly longer.

The aim of this ex vivo was to investigate if two radiopaque root canal sealers with different formulations would influence the radiographic perception of root canal fillings. The root canals of 48 extracted maxillary canines were prepared and randomly assigned to 3 groups of 16 specimens each. In each group, the root canals were filled by lateral condensation of gutta-percha and one of the tested sealers: Endométhasone, Sealer 26, or a non-radiopaque sealer. A through-and-through void was simulated in half of the specimens from each group (n=8). The buccolingual radiographic images obtained were randomly interpreted for voids existence by a radiologist and an endodontist. The differences in sensitivity and specificity between groups and examiners were compared using, respectively, Fisher's Exact and McNemar tests at 5% significance level. Both radiopaque sealers caused a significant decrease in sensitivity at the coronal part of fillings. The use of Endométhasone increased specificity values for both coronal and apical portions of the root canal fillings. In conclusion, the tested sealers influenced the radiographic perceptions of laterally condensed root canal fillings in a different way.
