Bronchial asthma incidence and mortality dynamics in adult population of the North-Western federal district

SummaryAim. Study the BA incidence and mortality dynamics in adult population of the North-West Federal District (SZFО) throughout 2010–2019, as well as compare these param-eters of SZFO subjects with other regions. Маterials and methods. Official data from statistical books of the Min-istry of Healthcare of the Russian Federation have been used, in particular adults’ presentation to the medical in-stitutions of the Russian Federation, Moscow and SZFO subjects with BA throughout 2010–2019, as well as BA deaths. Microsoft Excel and SPSS programmes were used to process statistical data. Results. Among the federal districts of the Russian Federation, the North-West was #3 in 2018 with primary BA incidence. Сomparative analysis of 2010–2019 data showed that both general BA inci-dence and newly diagnosed cases of BA among the adult population of SZFO may be assessed as not very high on a global scale. The BA incidence dynamics throughout the period under study had an ascending trend. Mortality due to bronchial asthma on the whole tend to de-crease, but there is a significant variability among various regions of SZFO. BA primary incidence is much higher in Nenets Autonomous District and Arkhangelsk Region (М=191.1±12.6 and 106.4±5.7, respectively), while gener-al incidence was much higher in Arkhangelsk and Pskov Regions, higher than the average levels over Russia and the incidence levels in the majority of SZFO subjects. BA general incidence had minimal levels in the adult popu-lation of SZFO in 2010 (934.6), and maximum ones — in 2019 (1233.1). The BA general incidence in all the regions showed pronounced ascending trend, except Saint Pe-tersburg where it was stable (Тпр.ср.=0.73%). The highest BA mortality level among SZFO population was noted in 2010 — 1.6 сases per 100,000 population, the lowest — in 2016 (0.5 сases). Сomparative analysis demonstrated that BA mortality level in 2017 in the overall population of Russia was 22.2% higher than among SZFO population. Conclusion. Higher bronchial asthma incidence among adult population of SZFO regions is due to commonly recognized factors that promote the development of this condition, including ecology, pollution (environment, air) and climate and geographic features, but also are con-nected with improvement in BA diagnostics, last but not least thanks to better quality of continuous education of pulmonology specialists

Objective: to analyze the epidemiological situation on Ixodidae tick-borne borreliosis in the Russian Federation in 2019 in comparison with the period of 2002–2018.Materials and methods. The paper uses the data contained in Form No. 2 of the state statistical reporting for 2002–2019 and information obtained by the Reference Center for Monitoring Borreliosis of the Omsk Research Institute of Natural Focal Infections from 74 constituent entities of Russia in 2019. The main research method is epidemiological one with the use of modern information technologies.Results and discussion. In Russia, 8048 cases of tick-borne borreliosis (Lyme disease – LD) were recorded (5.48 0/0000) in 2019. The actual indicators of the LD incidence for the whole country and federal districts (FD) in 2019 were within the confidence limits predicted with linear regression based on the study of the dynamics of the epidemic process in 2002–2018 in the vast majority of cases. A steady upward trend in the LD incidence was observed during 2002–2019 in the Central Federal District due to 10 out of 18 entities (Moscow, Belgorod, Lipetsk, Moscow, Bryansk, Voronezh, Kursk, Ryazan, Tambov, Tula regions); in the Southern Federal District because of the Krasnodar Territory and the Volgograd Region; in the North Caucasus Federal District – because of the Stavropol Territory. Despite the fact that a downward trend in the incidence of LD has been established over the past 18 years in the North-West, Volga and Ural Federal Districts, in some subjects of these regions a trend towards an aggravation of the epidemiological situation is observed (the Komi Republic and Chuvashia, Penza Region). In the absence of a pronounced tendency to change in the incidence rate of LD in the Siberian and Far Eastern Federal Districts, Kemerovo Region-Kuzbass, the Republic of Tuva and the Trans-Baikal Territory where a growing trend has been identified require special attention. In the Ural and Siberian Federal Districts, the share of non-erythema forms among laboratory-confirmed cases of LD was higher than in other regions, which merits further study of the genome-specific features of borrelia populations and their carriers. Effective control of the LD epidemiological situation in Russia is possible provided that the control is improved and maintained, and the capacity of preventive measures and zoological-entomological monitoring of the activity and structure of the natural foci of LD is enhanced in the entities with the long-term tendency towards increase in the incidence of the disease.

The need to formulate a strategy for the development of farms in the North-West of the Russian Federation is confirmed by the fact of their significant contribution to maintaining rural areas and ensuring food security in the region. The article proposes recommendations for creating strategic foundations for the development of peasant farm enterprises in the North-West of the Russian Federation, which can be used by the authorities when developing such a strategy. The study is based on the works of both domestic and foreign scientists-economists dealing with the issues of strategic planning considering farms and identifies various nuances of this issue. According to the foreign researchers, it is important for a farmer to find his competitive advantage, and this must be done taking into account the assessment of internal and external opportunities and threats. The analysis used the data of the Federal State Statistics Service for the municipal districts of the Northwestern Federal District, as well as methods of socio-economic analysis, economic and statistical (groupings), SWOT analysis, the problem tree method, the method of typing the municipal districts by A.I. Kostyaev. The study made it possible to propose recommendations for the formation of a strategic framework for developing farms in the North-West of the Russian Federation. Main scenarios for the development of this category of farms in accordance with the determined types of municipal districts and the classification of alternative farming strategies existing in modern science were identified, and SWOT was carried out, i.e., the analysis of the peasant farm enterprise development from the point of view of the farmer and the state. Also a problem tree for developing these farms was built, taking into account the specifics of the Northwestern Federal District, which made it possible to identify the difficulties that hinder the successful functioning of farms. In order to improve the functioning of peasant farm enterprises in the region, measures are recommended that should become the basis for the development strategy of these farms in the North-West of the Russian Federation, including: increasing state support, improving the legal framework, expanding educational programs and consulting activities, supporting the sales market by the government authorities.

Background.Analysis of statistical data in the federal districts of the Russian Federation provides opportunities to assess the quality of medical care delivery in children with cancer and plan further development with consideration for the point elimination of existing defects. Objective. Our aim was to analyze the main indicators characterizing medical care for children with cancer in the North-West Federal District.Methods.The study analyzed operative reports for 2017 provided by the executive public health authorities of 11 subjects of the Russian Federation affiliated to the North-West Federal District.Results.The pediatric population was 2 537 133 children (0–17 years), the pediatric oncological bed capacity — 174 (0.7 per 10 000 aged 0–17 years), the annual berth average occupancy — 290.7 days. 6 (54.5%) subjects did not provide pediatric population with departments of pediatric oncology, 4 (36.4%) subjects did not have bed capacity. The number of practitioners providing medical care to children with cancer was 38, 27 among them (71%; 0.1 per 10 thousand 0–17 years) had a certificate of pediatric oncologist. 2 (18.2%) subjects did not have pediatric oncologists in the medical stuff. The incidence of malignant tumors in children aged 0–17 was 15.6 (per 100,000), the prevalence was 92.6 (per 100,000), the mortality rate was 2.6 (per 100,000), one-year mortality rate — 4.5% (18/397). The number of patients identified actively was 11 (2.8%). 160 (40.3%) primary patients were sent to Federal medical facilities, 6 (1.5%) — left the territory of the Russian Federation for the further treatment.Conclusion.The incidence and mortality rates are rather low which indicates the defects in patient detection and lack of reliable follow-up data. The percentage of patients referred for treatment to Federal medical facilities is not very high; however, to interpret this indicator according to patient routing, we have to analyze the medical history of all the patients. We can assert that health care delivery service for children with oncological diseases is at an acceptable standard level but needs to be improved.

An important role in the normal development of the fetus and the successful outcome of pregnancy is played by the state of health of pregnant women. One of the significant factors of perinatal risk is anemia of pregnant women. In order to assess the level and dynamics of the incidence of anemia in pregnant women, official statistical reports and publications of the Federal State Statistics Service for 2005-2017 and Central Research Institute for Organization and Informatization of Health of the Ministry of Health of the Russian Federation for 2012-2018 were analyzed. Using a trend analysis using a trend method, a prognosis was made for the incidence of anemia in pregnant women up to 2021 in the Russian Federation as a whole and separately in the North-West Federal District. In order to assess the impact of the predictor “anemia of pregnant women” on the infant mortality rate, data from primary medical documentation was copied to 250 children who died before the age of 1 year in the North-West Federal District. It has been established that the incidence of anemia in pregnant women in the North-West Federal District significantly exceeds the average level in the Russian Federation. Provided that factors affecting the course of pregnancy remain unchanged, the incidence of anemia in pregnant women by 2021 on average in the Russian Federation will decrease to 32.2% of the number of women who have completed pregnancy, and in the North-West Federal District will increase to 37.5%. During pregnancy, the incidence of anemia in women whose child died before the age of 1 year in the North-West Federal District amounted to 44.16 ± 0.39% of the number of women who completed the pregnancy, which significantly exceeded the average figure in the district, which in 2017 was equal to 35.90 ± 0.35% (p < 0.05). A direct correlation has been established between the predictor “anemia in pregnant women” and the infant mortality rate.

The territories of the timber industry complex of the constituent entities of the Russian Federation that are part of the North-West Federal District are considered. A number of regions in the North-West Federal District possess a significant forest resource base and a system of logging and woodworking centers, which is a prerequisite for the intensive development of the timber industry complex and its territories. However, at present, the wood resource potential of these constituent entities of the Russian Federation and the capabilities of the timber industry complex of the North-West Federal District are not being used rationally and are being subject to extracting the current profits by renters of forest areas, which саuses stagnation of wood processing companies and places of their locations. In order to search for potential field of development of territories belonging to the timber industry complex of the North-West Federal District, urban planning aspects of the modern use of these territories have been studied, key urban-planning and resource problems requiring a scientifically based solution have been identified and structured.

To improve the rheumatology service in the Russian Federation, it is necessary to determine the true extent of primary and general incidence of osteoarthritis (OA).The aim of the study is a detailed assessment of trends in the dynamics of OA incidence in the Russian Federation, the North-Western Federal district (NWFD) and the Arkhangelsk region (AR).Materials and methods. We evaluated trends in the dynamics of both incidence and prevalence of OA in the Russian Federation (RF), NWFD and AR in the period 1994–2018 based on data from the annual statistical reports of the Ministry of health of the Russian Federation (form 12). Data on the population of AR were obtained in the regional Bureau of statistics – Archangelskstat, for the northwestern Federal district and the Russian Federation from freely available statistical collections of Rosstat. We analyzed the indicators of the adult population (over 18 years old). To evaluate time trends, we used segmented analysis using the Joinpoint Regression Program (National Cancer Institute, USA) to analyze linear trends, evaluate their statistical significance, and identify time points of their change (joinpoints).Results. In the period from 2004 to 2012 in Russia was a decline in the number of annually detected cases of OA, the prevalence of OA increased steadily in the 2016 year recorded a decline in the number of cases of OA in NWFD and AR. The analysis showed a discrepancy in the trends of incidence of OA in the Russian Federation, the northwestern Federal district and AR, which was nonlinear and chaotic. In all territories, the prevalence was increasing. In the Russian Federation, the period from 2004 to 2008, when the primary incidence of OA fell sharply, was marked by a stable increase in the prevalence from 2041.6 to 3383.3 per 100 thousand population. Conclusion. Analysis of official statistical information on the both incidence and prevalence of OA shows a significant variation in indicators, their fluctuations are not related to changes in the practice of diagnosis and treatment of this disease and, most likely, are associated with gaps in its accounting. Improving the epidemiological assessment of OA is possible with the introduction of a system of personalized patient registration-the osteoarthritis registry.

This article analyzes the trends in international migration processes in federal districts. The analysis of state statistics (2016-2018) revealed conflicting trends in its various aspects. The results show that international educational migration in Russia has an extremely low proportion, which is only 3% of the total migration inflow. The number of educated migrants has grown by almost a third. The main contributing countries of the educational sphere are Kazakhstan, China, Turkmenistan, Tajikistan, Uzbekistan, India, and Iraq among others. At the same time, the number of educational migrants from Kazakhstan, Ukraine, and Moldova has slightly decreased over three years. The results show significant imbalances in the territorial distribution of educational migration flows in the Russian Federation. They mostly take place in the regions and the North-West Federal District (about 50% of migrants by educational institutions, Volga and Siberia&nbsp;— more than 30%). This means that the education system and the use of infrastructure are unlimited. This article identifies the types of dynamics of the formed clusters in federal districts, which resulted in the corresponding typology. The author has calculated the growth rate of educational migration, which is the ratio of the growth in the number of educational migrants to the increase in the total number of people who arrived in the federal district and registered as migrants. It seems appropriate that within the framework of research in the field of the modern sociocultural approach, the mechanism of redistribution of human capital should be involved. This will allow a deeper study of sociocultural factors and the consequences of educational migration.
