Surgical Science is one of the branches of medical science that deals with surgical and medicinal interventions, which can cure surgical wounds, cuts, burns and so on. The accelerated recovery process has always been a concern for surgeons and physicians. Standard local antibiotic drugs such as silver nitrate, mafenide acetate, sulfadiazine silver and AgNO3 are also used to treat wounds which are associated with some complications. Considering the fact that no effective herbal remedy has been introduced for wound healing, the purpose of this review is to investigate and identify indigenous Iranian herbs used for wound healing after surgery, especially burn wounds. In this systematic review paper, published articles in the period 1952 to 2017 with keywords of surgery, wound healing, burns, herbs, etc. from databases such as SID, MEDLIB, ProQuest, Magiran, Europe PubMed Central and Google Scholar. Abstract and title of all articles were studied and non-relevant articles were deleted and remaining articles were used for review. Based on the results of the study, Malva sylvestris, Camellia sinensis, Aloe vera, Cydonia oblonga, Scrophularia striata, Centella asiatica, Arnebia euchroma, Calendula officinalis and Hypericum perforatum are the most important herbal remedies for burn restoration. The following is an explanation of the most important herbal remedies for native burn wounds in Iran.

Background: Centella asiatica L. Urban is a tropical plant whose spread is quite broad as Indonesia. One of the ingredients of Centella asiatica L. Urban is asiaticoside which has excellent wound healing abilities. However, research on diabetic wound healing with Centella asiatica L. Urban extract formulation in the form of a gel has not been found. Therefore, it is necessary to look at the healing activity of diabetic wounds using Centella asiatica L. Urban extract in the form of a gel. Objective: This experimental study aims to explore the effect of gel extract derived from the Centella asiatica L. Urban on the length of time for wound healing. Methods: The subjects in this study were eight weeks old Balb-C mice conditioned to hyperglycemia and were divided into five groups. The Centella asiatica L. Urban extract is provided in three concentration levels, with 3%, 5%, and 7%. As a form of negative control, used gel without Centella asiatica L. Urban extract and positive control without gel, only hydrocolloid dressing. Results: Centella asiatica L. Urban at concentrations of 3% (with the value of Sig. > 0.05), 5%, and 7% showed the ability to heal wounds. Conclusions: Centella asiatica L. Urban gel extract with a concentration of 3% had a significant effect on wound healing compared to other preparations.

Transdermal patch is a drug delivery system using a polymer for controlling the drug release. In this study, polysaccharides from aloe vera gel were used as polymer in transdermal patches because they are compatible, biodegradable, and safe to apply on skin without any side effect. Pentacyclic triterpenoids rich (asiatic acid, madecassic acid, asiaticoside and madecassoside) Centella asiatica extracts were added as active ingredients. Both polymer and active ingredients show pharmacological effects as wound healing. It was found that the optimized formulation contained polysaccharides from aloe vera gel blended with 20% glycerine and 15% sodium alginate calculated based on aloe vera mass could produce the good films. These films could be controlled with respect to physical properties thickness (0.509±0.015 mm), tensile strength (1.391±0.131 MPa), elongation at break (116.2±13.15%), peel strength (0.116±0.044 N), the highest swelling times (20 minutes), and the percentage of degradation at 30 minutes (51.57±5.96%). About 0.34 % centella extracts (57% pentacyclic triterpenoids) could be found in these films. After stability studies at 4, 25 and 45 °C for 10 weeks, films stored at 45 °C were loss of flexibility and brittleness that could not be further evaluated. While films stored at 4 and 25 °C were good physiochemical properties and stable as same as films base.

Centella asiaticaherbs have been prescribed as a traditional medicine for wound healing in China and Southeast Asia for a long time. They contain many kinds of triterpenoid compounds, mainly including glycosides (asiaticoside and madecassoside) and corresponding aglycones (asiatic acid and madecassic acid). To identify which is the major active constituent, a comprehensive and comparative study of these compounds was performed. In vitro, primary human skin fibroblasts, originating from healthy human foreskin samples, were treated with various concentrations of asiaticoside, madecassoside, asiatic acid, and madecassic acid, respectively. Cell proliferation, collagen synthesis, MMP-1/TIMP-1 balance, and TGF-β/Smad signaling pathway were investigated. In vivo, mice were orally administered with the four compounds mentioned above for two weeks after burn injury. The speed and quality of wound healing, as well as TGF-β1levels in skin tissues, were examined. Interestingly, in contrast to prevalent postulations, asiaticoside and madecassoside themselves, rather than their corresponding metabolites asiatic acid and madecassic acid, are recognized as the main active constituents ofC. asiaticaherbs responsible for burn wound healing. Furthermore, madecassoside is more effective than asiaticoside (P=0.0446for procollagen type III synthesisin vitro,P=0.0057for wound healing speed, andP=0.0491for wound healing patternin vivo, correspondingly).
