In Poland, over 66,000 physiotherapists are practicing. For many years, the profession of physiotherapist was not regulated by law. The Act on the profession of physiotherapist has been in force since May 31, 2016. In 2019, physiotherapists obtained new rights in the field of therapeutic rehabilitation financed by the National Health Fund. Currently, they can already choose / order rehabilitation and order medical devices under the conditions of the law. The aim of the article is to present and analyze changes in law affecting the new status of a physiotherapist in the healthcare system in Poland. In addition, drawing attention to changes in the competence of doctors in the field of therapeutic rehabilitation and the principles of their cooperation with physiotherapists. Conclusions: The analysis shows that changes in law in recent years have significantly influenced the increase in the role of physiotherapists in the health care system, and they are also important for practicing the medical profession.

Health care systems face challenges related to the technological advances in medicine, demographic changes and limited opportunities for growth funding for health, necessitating greater involvement in the search for more efficient systems. The authors present the functioning of the Polish health care system based on social, historical outline of the healthcare system in Poland and the functioning of the National Health Fund (NFZ). Poland has undergone many reforms of the health care system, the Bismarck model, the model Siemaszko, and finally to a model of universal health insurance. So everyone has the same right to health care services financed by the NFZ or directly from the state budget (eg. The system of state emergency medical services). The National Health Fund allows anyone insured to free healthcare and reimbursement of medicines. Introduced information about information programs.

Nurses are the largest group of Polish medical staff. There are currently approximately 230,000 nurses employed in Poland. There is a statutory profession for many years. Nurses provide health services on the basis of a medical order or on their own. As a result of changes in the law, the scope of their professional competences has been increasing for several years, including to independently administer medicines and issue prescriptions. The purpose of the article is to present and analyze legal norms determining the status of a nurse in the Polish health care system. In addition, the definition of the statutory principles of cooperation between doctors and nurses. The analysis shows that changes in law in recent years have significantly influenced the increase in the role of nurses in the health care system and they are also relevant to the practice of the medical profession.

Many changes have taken place in the professional status and political role of the Spanish medical profession during the process of construction of the health care system (1940–1990). There is a strong correlation between the different characteristics of the several phases of construction of the national health care system and some of the changes in the status, organization, and political activity of the medical profession. The democratic transition coincided with changes in the orientation of the system brought about by financial imperatives, which forced readjustments in the survival and political projects of the profession. The creation of a democratic regime has allowed the medical profession to create its own independent mechanisms of interest representation and has given rise to a process of negotiation of the new political relations (and their mechanisms) between the State and the profession. The last 15 years have witnessed important transformations in some parameters of the profession, in its political organization, its political role, and its relation with the State.

ABSTRACTSince the birth of the National Health Service in 1948 there have been periodic discussions of the potential role of pricing and insurance in the United Kingdom health care system. This article is concerned with discussing the problems inherent in these mechanisms and it advocates more careful articulation of the cost and benefits of such policies. The first section gives a description of some quite recent proposals to extend the role of the pricing and insurance mechanisms which have been made by the British Medical Association and the McKinsey consultancy company. The second section uses economic analysis to show that both the pricing and the insurance mechanisms have inherent problems which may vitiate their efficiency in many western health care markets. The third section is concerned with the mechanisms by which the efficiency of the health care system can be improved, and radical experimentation is advocated. Without radical experimentation and the implementation of suitable incentive systems, inefficiency and inequality will continue in the National Health Service.

The Academy has been involved with the concept of the pediatric nurse associate (PNA) since its evolution in the early 1960's. At that time it was recognized that there were children in this country who were receiving inadequate or no health care. In addition, predictions were being made of an impending physician shortage. It was also becoming increasingly apparent that there was a reservoir of health professionals who were not being utilized to their fullest potential within the health care system. This reservoir was nursing; thus, the concept of the PNA was conceived, developed, and has continued to grow over the past ten years. The education of the PNA has many unique aspects, for in this endeavor medicine and nursing assume joint responsibility in the development and administration of the educational programs. The PNA is a professional nurse with additional education in the field of child health care. In addition to his/her nursing role, she/he assumes certain functions and responsibilities which traditionally have been the domain of medicine. In the provision of these medical services the PNA functions under the supervision, direction, and review of the physician. Thus, it is imperative that medicine be actively involved, not only in the educational process, but also in the development of the role of the PNA within the health care system. There have been many changes not only within the specialty of pediatrics, but also within the health care system in general in recent years. To mention a few: a decline in the birth rate; the development of national health programs; rapid expansion of medical schools and classes; an increased emphasis on primary care and avoidance of fragmentation of care; the emergence of the family physician; and a crisis in the medical profession regarding professional liability coverage.

The research is focused on the analysis of the role of professional dynasty in the reproduction of the medical profession. The materials of autobiograph-ical interviews show how the medical dynasty influ-ences the choice of a profession and education, employment, and career advancement. The article considers the competitive advantages of representa-tives of medical dynasties in the educational (access to education, ease of learning from the perspective of starting opportunities for gaining knowledge and experience, access to internships) and social and labor spheres (employment under patronage, recog-nition of the dynasty and recognition by patients). It has been established that the family dynasty has all the necessary resources (educational, cultural, so-cial, etc.) and has a competitive advantage over oth-er strategies for the reproduction of the medical profession in general. Therefore, the popularization of family professional dynasties can play an im-portant role in solving the personnel issue of the health care system.

The article examines the problems of private medicine in the health care system of the country. The aim of the article is to determine the structural components of the mechanism of activation of the private medical sector as a reserve for the provision of quality and affordable medical services and a driver for the development of the medical industry. The descriptive-analytical method, methods of analysis and synthesis, comparison, statistical, analysis and scientific generalization were used in the paper. The study substantiates the strengthening of the role of the private medical sector in the health care management system. The key problems of the domestic private medical sector and the restraining factors of its development are identified. It is concluded that it is necessary to develop a mechanism to promote the development of private medicine through a policy of active change in the health care system. The directions of activation of the private medical sector as a target reference point in the process of determining the structural elements of the organizational and economic mechanism are outlined. The structural detail of the mechanism of activation of the private medical sector for optimization of the health care system is offered. The practical significance of the obtained results is that the results of the research presented in the article are a practical basis for the development and improvement of mechanisms of public administration in the field of health care.
