Smoking Cessation in COPD Patients by a Selective Partial Nicotinic Agonist

The fight against smoking through various smoking cessation methods could be very effective by early education especially in young people for preventing the occurrence or the severity of obstructive lung diseases. The aim of the study was to present the impact of varenicline, a selective partial nicotine agonist, in adults of 40-49 year-old, active smokers, recently diagnosed with COPD, from 2009 to 2011, in a Smoking Cessation Center of the Pulmonary Rehabilitation Clinic Iasi, Romania. There were included all male employees, without occupational exposure, active smokers, with a personal history of smoking �10 packs-year cigarettes, recently diagnosed with COPD, who inform consented to be enrolled for counseling and smoking cessation treatment provided by varenicline. All patients received COPD therapy, according to the current GOLD recommendations. The method of evalution the impact of smoking cessation methods consisted in the COPD Assessment Test (CAT) completed by cases before and after 12 weeks program of smoking cessation counseling and therapy and respiratory rehabilitation. The CAT is a short questionnaire, simple and validated tool of COPD symptoms assessment, measuring the severity of COPD on a patient�s quality of life. Results reveals an important decreasing of CAT score with important amelioration of symptoms especially in severe COPD patients. Counseling, smoking cessation and respiratory rehabilitation interventions have shown positive effects of smoking cessation with varenicline among young COPD patients 40-49 year-old.

Abstract Introduction: Policies to promote smoking cessation among Medicaid-insured pregnant women have the potential to assist a significant proportion of pregnant smokers. In 2010, Kansas Medicaid began covering smoking cessation counseling for pregnant smokers. Our aim was to evaluate the use of smoking cessation benefits provided to pregnant women as a result of the Kansas Medicaid policy change that provided reimbursement for physician-provided smoking cessation counseling. Methods: We examined Kansas Medicaid claims data to estimate rates of delivery of smoking cessation treatment to Medicaid-insured pregnant women in Kansas from fiscal year 2010 through 2013. We analyzed the number of pregnant women who received physician-provided smoking cessation counseling indicated by procedure billing codes (ie, G0436 and G0437) and medication (ie, nicotine replacement therapy, bupropion, or varenicline) located in outpatient managed care encounter and fee-for-service claims data. We estimated the number of Medicaid-insured pregnant smokers using the national smoking prevalence (14%) in this population and the number of live births reported in Kansas. Results: Annually from 2010 to 2013, approximately 27.2%–31.6% of pregnant smokers had claims for nicotine replacement therapy, bupropion, or varenicline. Excluding claims for bupropion, a medication commonly prescribed to treat depression, claims ranged from 9.3% to 11.1%. Following implementation of Medicaid coverage for smoking cessation counseling, less than 1% of estimated smokers had claims for counseling. Conclusions: This low claims rate suggests that simply changing policy is not sufficient to ensure use of newly implemented benefits, and that there probably remain critical gaps in smoking cessation treatment. Implications: This study evaluates the use of Medicaid reimbursement for smoking cessation counseling among low-income pregnant women in Kansas. We describe the Medicaid claims rates of physician-provided smoking cessation counseling for pregnant women, an evidence-based and universally recommended treatment approach for smoking cessation in this population. Our findings show that claims rates for smoking cessation benefits in this population are very low, even after policy changes to support provision of cessation assistance were implemented. Additional studies are needed to determine whether reimbursement is functioning as intended and identify potential gaps between policy and implementation of evidence-based smoking cessation treatment.

Background: The history of microdermabrasion dates back to ancient Egypt. The first treatment in Europe occurred in 1985 and has since become a popular method of exfoliation of the superficial layers of skin. Microdermabrasion is an effective method of taking care of the skin of the face and body, bringing about immediate effects in the treatment of deep scars and stretch marks. Aim of the study: To determine the effectiveness of diamond microdermabrasion on a person with hyperuricemia of the sebaceous glands. Materials and methods: Based on interviews, a case analysis, and the impact of diamond microdermabrasion on the structure of the skin’s surface, the level of hydration, oiliness and epidermal exfoliation were analyzed. Case report: The subject was a 22-year-old woman struggling with oily skin. A few acne lesions and a large number of blackheads were observed in her nasal area. An interview was conducted prior to the procedure to eliminate any contraindications. The study was conducted within a period of 4 months at 3-week intervals. The Nati Skin Analyzer was used to determine the skin parameters of the patient. Results: After applying a series of treatments using the microdermabrasion apparatus, satisfactory results were obtained in the form of reduced peeled sebum and improved skin hydration in the T and U zones. Conclusions: The study shows that diamond microdermabrasion treatment has a beneficial effect on the patient’s skin condition. Phot ographs before and after the surgery document the positive effects this series of treatments had. There was a decrease in the level of sebum secretion and the elimination of skin changes in the form of acne or open comedones.

ObjectiveExplore potential changes in the characteristics of patients requesting smoking cessation treatment at an outpatient setting in Spain before and after Law 42/2010 was enacted.DesignThis is a cross-sectional study with convenience sampling. The information was obtained from the medical records of patients receiving smoking cessation treatment from January 2008 to December 2014.SettingHospital Clinico Universitario de Valencia.Participants423 patients who sought smoking cessation treatment 36 months before or 48 months after the enactment of the law.ResultsAfter the enactment of a comprehensive smoke-free law in Spain, the patients seeking smoking cessation treatment were older (p=0.003), had lower values of exhaled CO (p<0.0001), lower number of previous attempts to quit (p=0.027) and more history of medical problems related to smoking (p=0.002).ConclusionOur findings support the idea that society–nation level interventions could have an impact at the individual level, reflected by the change of patients’ characteristics. It seems that the Law 42/2010 mobilised certain group of patients to seek treatment.
