Dysphagia Management in Culturally-Linguistically Diverse Populations

Culturally competent management of adults with swallowing disorders involves more than simply including ethnically appropriate foods in dysphagia therapy. It requires an understanding of the client's health beliefs, challenges, and unique cultural perspective regarding all aspects of food to ensure unbiased and culturally appropriate services are provided. This chapter begins with an overview of dysphagia management followed by a closer look at cultural beliefs regarding food and ethical conflicts that may arise. The strategies for shared decision-making presented help create a culturally sensitive dynamic between the clinician and the patient/family that positively influence therapy outcomes. The chapter concludes with a case study that highlights the importance of ethnographic interviewing needed to establish understanding and trust between the clinician and an elderly Mexican woman and her family. The strategies and techniques presented here can be applied across all cultures to achieve successful management of dysphagia.

Purpose: This study investigated the health beliefs of Taiwanese women in Anping, an urban district in Taiwan that was introduced to Western medicine in the 1860s. Design: A qualitative design with content analysis was used. Fourteen Anping women aged 44 to 84 years were interviewed. Results: The women integrated both traditional and Western biomedicine without any dissonance. Three themes were found: cultural beliefs about medicine–diet homology or the lack of a distinction between medicine and food, reliance on both doctor and deity, and a pattern of health practice based on situational decision making about which health practice to employ. Implications: Understanding health-related viewpoints in Chinese culture and its rationale will help health workers provide culturally competent care.

Health beliefs influence a variety of health-related behaviors and should be regarded as an important dimension of cultural variability. Such variability may explain disparities in health-care access, compliance, and treatment outcomes for persons from culturally and linguistically diverse (CLD) backgrounds. Cultural competence and client-centered service delivery require recognition of the effects of divergent health beliefs during clinical encounters. Assumptions derived from the Health Beliefs Model (HBM) can inform research to uncover communication-specific health beliefs held by persons from diverse populations. We conclude this article with a research protocol used to survey African American adults' knowledge and assumptions about communication processes and disorders.

This issue describes the Assessment 360° process, which takes a panoramic approach to the language assessment process with school-age English Language Learners (ELLs). The Assessment 360° process guides clinicians to obtain information from many sources when gathering information about the child and his or her family. To illustrate the process, a bilingual fourth grade student whose native language (L1) is Spanish and who has been referred for a comprehensive language evaluation is presented. This case study features the assessment issues typically encountered by speech-language pathologists and introduces assessment through a panoramic lens. Recommendations specific to the case study are presented along with clinical implications for assessment practices with culturally and linguistically diverse student populations.

Abstract Background Alcohol consumption in China has increased rapidly, and there have been calls for policies and programs to address the issue. Alcohol plays a complex and important role in Chinese culture, where it is considered a symbol of happiness and is associated with wellbeing. Alcohol reduction policies may fail unless they take these cultural and social meanings of alcohol, and its relationship to wellbeing into consideration. Baijiu is a clear fermented spirit that is widely consumed in China and has strong cultural associations with health, wellbeing and prosperity. There is a lack of research on how consumption patterns relate to cultural beliefs and subjective wellbeing. Methods An online survey of n = 1992 Chinese adults was conducted to determine frequency and volume of baijiu consumed; beliefs about health benefits and traditional importance; and associations with subjective wellbeing. Results Higher frequency and volume consumed were associated with higher subjective wellbeing, controlling for age and income. We also found small to medium significant associations between consumption frequency and volume and: belief that baijiu is healthy; and that tradition dictated they should drink baijiu. Conclusions The traditional and health values placed on baijiu, and its association with wellbeing, may help inform policy developments and alcohol reduction campaigns.

Abstract Objective The following case will demonstrate a systematic approach to neuropsychological evaluation with Spanish-speaking individuals, which includes creating a suitable test battery, interpreting results with appropriate normative samples, and incorporating personal history. Case Description 61-year-old, right-handed, Mexican female with 2 years of formal education, and with a recent history of subarachnoid hemorrhage with hydrocephalus. She was referred by her social worker and primary care provider to discern whether the reported cognitive complaints were due to a neurocognitive condition or depression. Diagnostic Impressions and Outcomes The evaluation was administered entirely in Spanish and some exams were modified to accommodate her limited literacy skills. She exhibited deficits in executive functioning, verbal fluency, and memory. Emotional testing revealed moderate depression with anxious distress, which she attributed to significant changes in everyday life. Her family informed us that she was the “matriarch of the family” and worked as a farm field truck driver, with significant difficulties/lack of engagement in both of these roles. Therefore, diagnoses of probable major vascular neurocognitive disorder and major depressive disorder with anxious distress were assigned. With this information, her providers were able to connect the family with community resources. Discussion The Hispanic population continues to be the fastest growing demographic in the United States. As more clinicians will work with members of this ethnicity in outpatient settings, it is important that they incorporate culturally-relevant factors in their approach to testing and interpretation of results. Nonetheless, this case demonstrates the current challenges and limitations, including modification of exams, differences in educational system that underlie test construction, and patient’s history. Recommendations for future areas of study and practice will also be discussed.

This chapter discusses family group conferences (FGCs) and marginalised communities. FGCs offer an alternative and more sustainable approach to working with families who are marginalised or socially excluded. Used in the right way, and with due regard paid to the culture and context in which families are living, FGCs are a powerful and effective intervention. The danger is that practitioners and services believe that it is automatically culturally competent, and they need to be aware that it operates as a framework that embraces culture if true partnerships are formed with families and they are empowered to make choices about the way that the model works for them. In order to do this, practitioners must be able to have conversations with families about their cultural beliefs, values, and practices and work with them to ensure that the FGC embodies this. Furthermore, FGCs have the potential to strengthen and build communities and social capital.

The presence of refugee students in culturally and linguistically diverse classrooms relatively influences thedevelopment of local students as well. Students with different languages and cultural background in a learningenvironment create a different atmosphere and may influence the attitude of local students to other languages andcultures. Therefore, this study investigated how and to what extent native students were influenced from refugeestudents in terms of linguistic and cultural perspective. The mixed method was used in this study. Quantitative datawere collected from local students in culturally and linguistically diverse classrooms through a questionnaire andqualitative data were collected from teachers teaching in these classrooms by means of interviews. The results of thestudy revealed that there were remarkable positive effects of the integration process on both sides though variousproblems emerged in diverse classrooms. The findings of this research may also (delete) give implications about theinfluence of refugee movement for other cultures.

Health and social service agencies need to be responsive to the healthcare requirements of culturally and linguistically diverse (CALD) groups in the community. This is a challenging proposition, particularly due to shifting demographics in developed Western countries such as Australia. Organisations that strive for cultural competence can potentially reduce the barriers associated with inequitable access to services by CALD groups. Community health services play a vital role in the provision of culturally competent health services to people from CALD groups. Additional research related to cultural competence in the community health context is needed. Thus, the aim of this paper is to explore the positioning of cultural competence within community health from multiple perspectives using a qualitative case study of a community health service located in Victoria, Australia. The findings suggest that if the essential needs of clients are met, regardless of cultural background (e.g. able to communicate with staff, trust and a respectful and caring environment), then issues related to cultural background may be of less significance for some clients.

Miner et al. (2018) claim that focusing on individual factors to understand gender inequity in the fields of science, technology, engineering, and mathematics (STEM) provides an incomplete explanation of the phenomenon. They challenge the appropriateness of individual-level explanations that hold women responsible for the injustices they experience, suggesting that this perspective fails to consider larger social-contextual influences. Instead, to explain gender disparity in the STEM fields, Miner et al. offer a social-structural lens through which to view the situation that relies on commonly held beliefs about women in society. The inequality that characterizes these fields, however, is a worldwide phenomenon that spans societal boundaries. Therefore, understanding the social-contextual factors that contribute to gender inequality in the STEM fields requires a cross-cultural examination of norms and values. In this commentary, I first outline a program of research aimed at developing an empirically supported theoretical framework that explains gender inequity in the STEM fields from a cross-cultural perspective. Then, I review the ways in which cultural beliefs influence education and careers in the STEM fields. Finally, I provide some practical suggestions of ways to promote gender equality in STEM fields. As such, this commentary serves as a call to integrate concepts from vocational, educational, and cross-cultural psychology to address an issue of upmost importance: equal representation.

<p>Culture has a heavy overlay on the perceptions and the subsequent handling of persons with language and speech disorders. This article reviews the cultural perspectives of language and speech disorders as portrayed by persons with language and speech disorders, parents, teachers, and language and speech pathologists. The information on cultural perspectives was collected between November 2006 and August 2007 during Operation Smile, Kenya Chapter Missions. 20 persons, aged between 8 and 53 years, were interviewed. The research objective was to establish the cultural perspective that determines the interpretations of language and speech disorders and their subsequent management.</p> <p>The finding of the study is that there is a notable cultural association or correlation between cultural beliefs and language and speech disorders. There is also ignorance of the causes and management procedures of language and speech disorders. We conclude that there is a need to provide assessment and treatment protocols that are culturally fair, effective, and acceptable. Such protocols include taking into account gender preferences, adhering to culturally relevant communication patterns, and using collaborative therapy.</p>
