Recognition Patterns Construction of Coronary Heart Disease Patients with Qi Stagnation Syndrome Based on Decision Tree

Coronary heart disease (CHD), called Thoracic Obstruction in TCM, is one of the most important types of heart disease for its high incidence and high mortality. The methods of syndrome studies in TCM can not be completely in accordance with that of modern medicine because of the complexity itself. In this paper, we decide to investigate the ability of Decision Tree to predict CHD patients with or without qi stagnation syndrome. Predictions with CHAID Decision Tree (one type of the Decision Trees), we obtained recognition patterns made up of seven biological parameters. The accuracy of this diagnosis pattern was 80.7%, the sensitivity and specificity could reach 72.1% and 83.0%. The ADTree recognition pattern include six biological indicators. The accuracy of this diagnosis pattern was 94.6%, the sensitivity and specificity could reach 100% and 94.3%.

In this paper, we investigated the ability of Artificial Neural Networks (ANNs) to predict CHD patients with or without qi deficiency syndrome. Predictions with Multilayer Perceptron Neural Network (MPLNN, one type of the ANNS), we obtained recognition patterns made up of eight biological parameters. The accuracy of this recognition pattern was 82.2%, and the accuracy of validation pattern was 80.0%.

Coronary heart disease (CHD) is one of the most important types of heart disease because of its high incidence and high mortality. TCM has played an important role in the treatment of CHD. Syndrome differentiation based on information from traditional four diagnostic methods has met challenges and questions with the rapid development and wide application of system biology. In this paper, methods of complex network and CHAID decision tree were applied to identify the TCM core syndromes of patients with CHD, and to establish TCM syndrome identification modes of CHD based on biological parameters. At the same time, external validation modes were also constructed to confirm the identification modes.

Coronary heart disease (CHD) is the leading cause of death. As the main treatment of CHD, modern medicine has improved dramatically in recent years. Although researches of TCM and integrative medicine on CHD are witnessed encouraging progress in many respects, the role TCM playing in the prevention and treatment of CHD has been unprecedentedly challenged under such circumstance of the very fast development of modern medicine. In order to share mutual complementary advantages of TCM and western medicine, this review summarizes the relatively prominent researches of TCM and integrative medicine on CHD in recent years, and illuminates the issue of the orientation of the further research of integrative medicine on CHD, including (1) original innovation of TCM etiology and pathogenesis, (2) combination of disease and TCM syndrome, (3) biological basis of TCM syndrome of CHD, (4) clinical design and quality control of integrative medicine research, (5) herb-drug interaction, (6) difficulties and hot issues of modern medicine.

According to research, hostility is a reliable predictor of coronary heart disease. Cook-Medley-defined hostility in particular has been seen as a significant precursor of coronary disease. It is important to specify the kind of hostility measured by the Cook-Medley scale. The present study examined the construct validity of the Cook-Medley scale for a sample of 219 university students in Finland, a country with a high incidence of coronary heart disease. The results supported the interpretation of the Cook-Medley scale primarily as a measure of cynicism and distrust. On the basis of our previous research, a subscale of the Cook-Medley scale was formed. This 9-item scale, a measure of Cynical Distrust, correlated positively with cynicism and negatively with trust. Moreover, the over-all pattern of correlations between the new 9-item factor, cynical distrust, and other variables assessing hostility, anger, and trust, was similar to the one obtained for our previous sample of students. It is suggested that this new 9-item scale is a shorter and more specific alternative to the longer Cook-Medley scale as a measure of cynical distrust.

Angina pectoris (AP) with coronary heart disease (CHD) is one of the common cardiovascular diseases in clinical practice, which can be classified as “chest paralysis” in Chinese medicine according to its symptoms, and it is described in many ancient documents. Ancient Chinese medicine believes that the main pathogenesis of the disease is poor blood flow leading to paralysis of the heart and veins, so it is often treated by activating blood and removing blood stasis. In this study, 120 patients with AP of CHD of Qi stagnation and blood stasis type were randomly divided into the observation (n = 60) and the control group (n = 60). In the control group, basic care, conventional treatment, and unselected copper acupuncture scraping were used, while in the observation group, copper acupuncture scraping was performed at the right time of the heart meridian (11 : 00–13 : 00) on the basis of the control group, and all patients received the treatment for a total duration of 4 weeks. We collected data on the traditional Chinese medical (TCM) syndrome score, frequency and duration of angina attacks, nitroglycerin dosage, inflammatory factor levels, and hematological indices pretreatment and posttreatment in both groups. Patients’ adverse effects during treatment were recorded, and the clinical efficacy and ECG efficacy in both groups were evaluated after 4 weeks. We used SPSS.20 statistical software to statistically analyze the above data, and the results showed that the clinical efficacy and ECG efficacy of the observation group were significantly higher than the control group posttreatment. After treatment, the TCM symptom score, angina attack frequency, attack duration and nitroglycerin dosage, serum interleukin-8 (IL-8), hypersensitive C-reactive protein (hs-CRP), and tumor necrosis factor-α (TNF-α) levels, whole blood viscosity (WBV), plasma viscosity (PV), fibrinogen (FIB), and hematocrit (Hct) were significantly lower in both groups compared with those posttreatment. And the observation group showed a greater decrease when compared with the control group. The results also showed that the overall incidence of adverse reactions was lower in both groups during the treatment period. The above results indicate that while ensuring high safety, the copper stone based on theory of midnight-noon ebb-flow can more effectively improve the symptoms and inflammatory response of the body and reduce the viscosity of the blood in AP with CHD of Qi stagnation and blood stasis, and it has better therapeutic effects.
