Quality assurance of production and the associated maintenance systems in recent decades changed a lot and are subject to change requirements for reliability - Reliability and Availability of machinery and equipment subject to the optimal maintenance costs. The paper deals with design options and assessment of the maintenance system intended method of reliability-oriented maintenance and maintenance costs. They use the methods of statistical and probabilistic analyzes of data on failure mode of operation, as well as methods priori reliability. The emphasis is on cost-effectiveness evaluation.

The loading and Unloading operational activities in port often run into obstacle, because happened malfunction occurrence of the equipment used. Failure to operate the equipment has resulted in downtime decreased of company productivity. Therefore, a system is needed maintenance of planning in order to produce the equipment that can be operated optimally. This research was choosen the company PT. XXX which is a company that enganged in the field maintenance of equipments port operational (quayside container crane, rubber tyre gantry crane, gantry luffing crane, and others) the place where the equipment maintenance system, especially the rubber tyre gantry crane (RTGC) which has been done are still a breakdown maintenance and corrective maintenance. The aim of this research is to analyze by method of reliability centered maintenance (RCM) to able to determined the optimal maintenance and it can be predict steps to overcome the damage that might will happen in the next period based on existing data. From data processing and analysis was obtained that the components most frequently damaged on the equipment of RTGC, cause of failure, selection decision and recommendations for maintenance systems are based on Reliability Centered Maintenance (RCM).

Background: A pharmacoeconomic analysis of the RISCAVID database aimed at assessing the cost effectiveness of phosphate binders in preventing CV mortality and morbidity over 7 years was performed. Methods: Morbid or fatal events occurring in 750 chronic HD patients were recorded. Statistical analysis evaluated the distribution of variables and the effect of sevelamer on survival. A cost-effectiveness evaluation was performed using a probabilistic model based on a Markov chain. Results: Multivariate analysis showed that treatment with sevelamer was associated with a reduced stroke incidence by 52% (p = 0.04) and reduced levels of C-reactive protein (p < 0.01). Cost-effectiveness evaluation evidenced a 33% decrease in hospital-days for patients treated with sevelamer, with and without comorbidities compared to patients undergoing calcium binders treatment. Conclusion: Treatment with sevelamer was associated with a reduced risk of stroke in HD patients, with a clear saving on disease-related costs for the Italian National Healthcare System.
