Stochastic and Deterministic Petri Networks Model for Joint Management Model for Spare Parts and Maintenance: Simulation Study

Maintenance management is an orderly procedure to address the planning, organization, monitoring and evaluation of maintenance activities and associated costs. The maintenance management allows to have an efficient tool either to the management of the preventive or curative activity, an optimization of the production tool, and finally a follow-up of the costs and the performances. A good maintenance management system can help prevent problems and damages to the operating and storage environment, extend the life of assets, and reduce operating costs.In this paper, we will first present our model on the joint management of spare parts and maintenance. We will do a simulation study of our model, presented in the first section of this paper. The results of this study are presented in the second section through the presentation of the influence of certain parameters of the model on the operation of the system under consideration. This study carried out on the graphical interface of Matlab, which is one of the performance evaluation techniques. It allows to visualize the variations and anomalies which can be reached in the system considered as an overcoming of the repair of the machines by the unforeseen breakdowns.

A properly implemented maintenance management system has an impact at different levels. Maintenance is defined as the set of actions to maintain a property in a specified state. The unavailability of the spare parts required, to carry out the maintenance intervention, causes an extension of the inactivity time of the installation. On the contrary, an excessive stock of spare parts confines enormous capital and entails an enormous cost of ownership. According to the literature already made, we have directed in our work to propose a model of joint management of maintenance and spare parts based on stochastic-deterministic batch Petri networks. We studied this model by simulation using a graphical interface dedicated to the graphical tool used. So, we present, in this paper, the analytical study of the model by defining the performance indicators and viewing the influence of system parameters on these indicators. The main stages of the analytical study are developing the μ-marking graph, the associated Markov process which gives the associated transition matrix, and the definition of performance indicators using the probability distribution of the states. We deal with an application of the analytical evaluation of the proposed model. We end this article with an analysis and synthesis.

Abstract Background Since last year, COVID-19, the disease caused by the novel Sars-Cov-2 virus, has been globally spread to all the world. COVID-19 infection among pregnant women has been described. However, transplacental transmission of Sars-Cov-2 virus from infected mother to the newborn is not yet established. The appropriate management of infants born to mothers with confirmed or suspected COVID-19 and the start of early breastfeeding are being debated. Case presentation We report a case of the joint management of a healthy neonate with his mother tested positive for Covid-19 before the delivery and throughout neonatal follow-up. The infection transmission from the mother to her baby is not described, even after a long period of contact between them and breastfeeding. Conclusion It may consider an appropriate practice to keep mother and her newborn infant together in order to facilitate their contact and to encourage breastfeeding, although integration with infection prevention measures is needed.

PURPOSE Until human papillomavirus (HPV)–based cervical screening is more affordable and widely available, visual inspection with acetic acid (VIA) is recommended by the WHO for screening in lower-resource settings. Visual inspection will still be required to assess the cervix for women whose screening is positive for high-risk HPV. However, the quality of VIA can vary widely, and it is difficult to maintain a well-trained cadre of providers. We developed a smartphone-enhanced VIA platform (SEVIA) for real-time secure sharing of cervical images for remote supportive supervision, data monitoring, and evaluation. METHODS We assessed programmatic outcomes so that findings could be translated into routine care in the Tanzania National Cervical Cancer Prevention Program. We compared VIA positivity rates (for HIV-positive and HIV-negative women) before and after implementation. We collected demographic, diagnostic, treatment, and loss-to-follow-up data. RESULTS From July 2016 to June 2017, 10,545 women were screened using SEVIA at 24 health facilities across 5 regions of Tanzania. In the first 6 months of implementation, screening quality increased significantly from the baseline rate in the prior year, with a well-trained cadre of more than 50 health providers who “graduated” from the supportive-supervision training model. However, losses to follow-up for women referred for further evaluation or to a higher level of care were considerable. CONCLUSION The SEVIA platform is a feasible, quality improvement, mobile health intervention that can be integrated into a national cervical screening program. Our model demonstrates potential for scalability. As HPV screening becomes more affordable, the platform can be used for visual assessment of the cervix to determine amenability for same-day ablative therapy and/or as a secondary triage step, if needed.

The building maintenance is a crucial part of the life cycle of the building. The maintenance strategies are planned to maintain the condition of the building for specified functions. The maintenance planning requires a comprehensive assessment in determining the effectiveness of building performance, especially in the maintenance budgets planning process. The effective budget preparation will reduce the risk of cost overruns and help the organization to execute the repair works efficiently with sufficient resources. This study attempts to identify the issues related to maintenance cost and proposed the key strategies for improving the sustainable building maintenance budgeting in dealing with the cost overruns. This paper utilizes a qualitative approach through a literature review of secondary data from previous studies. The proposed cost model of maintenance strategies will be used as a basis for further investigation and validation towards promoting the sustainable building maintenance management. The studies identified the push factors that influence the maintenance cost; human factors, tools and equipment, spare parts and materials, funds allocation and available information. Each of the push factors needs to be well-considered to ensure that the maintenance activities can be done efficiently and to avoid the issues of cost overruns.

Objective The new coronavirus infection from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been recognized as a global public health emergency, and neonates may be more vulnerable due to their immature immune system. The first aim of this study was to report our experience on the management of neonates from mothers with SARS-CoV-2 infection focusing on a 28-day follow-up since birth. The second aim is to assess how many data on neonatal outcomes of the first month of life are reported in literature, performing a systematic review and meta-analysis. Study design We report our experience based on routine management of neonates born to mothers with SARS-CoV-2 infection and follow-up until 28 days of life. Results In our experience at discharge, 1/48 (2.08%) of entrusted (mother refusing personal protective equipment) and none of separated presented positive nasopharyngeal swab (p = NS). All babies show good outcome at 28 days of life. The literature data show that the percentage of positive separated infants is significantly higher than the percentage of infants entrusted to positive mothers with appropriate control measures (13.63 vs. 2.4%; p = 0.0017). Meta-analysis of studies focused on follow-up showed a 2.94% higher risk of incidence of SARS-CoV-2 infection in entrusted newborns than in separated newborns (95% confidence interval: 0.39–22.25), but this was not significant (p = 0.30). Conclusion A vertical transmission in utero cannot be totally excluded. Since in newborns, the disease is often ambiguous with mild or absent symptoms, it is important to define the most efficient joint management for infants born to COVID-19 positive mothers, being aware that the risk of horizontal transmission from a positive mother, when protective measures are applied, does not seem to increase the risk of infection or to affect the development of newborns from birth to first four weeks of life, and encourages the benefits of breastfeeding and skin-to-skin practice.
