Combination Treatment of Skin Needling, Platelet-Rich Plasma and Glycolic Acid 70% Chemical Peeling for Atrophic Acne Scars in Fitzpatricks Skin Type IVVI

Background: Acne Vulgaris is a self-limiting, chronic skin lesion, which may heal with squally of different types of scars (ice pitted, boxcar, rolling, and keloid). Different options used to deal with scars such as laser dermabrasion, surgical excision, and chemical peeling. This study aims to assess the safety and effectiveness of using a combination of radiofrequency (RF) microneedling and platelet-rich plasma (PRP) in managing the atrophic acne scars among a sample of Iraqi patients. Method: An interventional study conducted from January 2017 to December 2018 at a private dermatological clinic in Kalar City, Al-Sulaymmania Province, Iraq. Patients with the atrophic scar and willing to participate are adequately assessed and treated by RF and PRP. A different number of sessions of therapy used, and patients followed for three months after the last meeting. Results: Thirty-one patients were involved in the study. The majority of them (27, 87.0%) were females with a mean age of 26.41±8 years. Twenty patients (64.5%) had opened mouth (boxcar and rolling) scars, and eleven (35.5%) had ice pitted scars. Ten patients (32.25%) showed an excellent response to RF and PRP therapy with two grades improvement, fifteen patients (48.38%) showed good response with one-grade improvement and six patients (19.6%) showed poor response without improvement in grades, (all of them of ice pitted scars). There was a direct relationship between the number of sessions and the response to therapy. Three patients developed folliculitis at sits of puncturing with the isolation of staphylococcus auras bacteria, which cleared by topical and systemic antibiotics. Conclusions: Although ice pitted scars showed an inadequate response to therapy, however, the reaction of the boxcar and rolling scars to RF and PRP was excellent, indicating that combination procedure is a safe, efficient and satisfactory option for the treatment of atrophic acne scars.

Abstract Background Acne scars, is a challenge for dermatologists, despite having multiple treatment modalities like microneedling, dermabrasion, Fractional CO2 Laser, dermal fillers, etc. However, monotherapy has been hardly satisfactory because of the polymorphism seen with the scars. Objective Comparison between microneedling with platelet rich plasma versus Fractional carbon dioxide laser with platelet rich plasma in treatment of atrophic post acne scars. Patients and methods This study was carried out in department of dermatology, venereology and andrology, in Kobry El-Kobba Military complex during the period (from September 2018 to July 2020 ) on 20 patients of both sexes aged from 20 to 60 years old presenting with Goodman and Baron Grade II, III, IV acne scars . Results The study revealed a statistically significant difference (p = 0.017) between Goodman and Baron scar grades on the right side of the face before and after treatment indicating that microneedling with platelet rich plasma was effective in improving acne scars. Also, there is a statistically significant difference (p = 0.010) between Goodman and Baron scar grades on the left side of the face before and after treatment, indicating that fractional CO2 laser with platelet rich plasma was effective in improving acne scars. Conclusion and recommendation Further controlled and randomized studies are needed to validate our findings in a larger cohort of patients and longer follow up. Also, number of sessions might be more than 3 sessions.

<p class="abstract"><strong>Background:</strong> Although various treatment options are available for the management of melasma, still it is challenging disorder to treat. Apart from topical therapies, newer modalities like laser toning and chemical peeling are being explored to manage this notorious dermatosis. To evaluate and compare the efficacy and side effects of laser toning versus 50% glycolic acid peel in the management of melasma.</p><p class="abstract"><strong>Methods:</strong> A split face comparative study was done in 30 female patients with melasma. Over the right half of patient’s face laser toning was done whereas other half was treated with 50% glycolic acid peel. 4 such sessions were given at an interval of 15 days. Response to treatment was assessed using melasma area and severity index (MASI) score.<strong></strong></p><p class="abstract"><strong>Results:</strong> Baseline mean MASI score on right and left half of patient’s face was 7.85±3.34 and 6.25±1.56 respectively. After completion of 4 treatment sessions, it was reduced to 4.23±1.45 and 3.71±1.10 respectively with statistically insignificant difference between two halves (p=0.08). Recurrence rate of melasma was higher (46.66%) with laser toning in comparison to glycolic acid peel (26.6% patients).</p><p class="abstract"><strong>Conclusions:</strong> Glycolic acid peel and laser toning were comparable in terms of outcome with laser toning showing slightly better results. In comparison to peels recurrence rate and complications like hypopigmentation in Asian patients were higher with laser toning.</p>
