National Health Insurance (NHI) Program has been implemented in Indonesia on January 2014. That program definitely brings some changes into managerial aspect in public health center (PHC). This study aimed to determine an impact of NHI policy in the implementation of health promotion programs at PHC in South Tangerang City, Indonesia. This study was conducted using qualitative method during February and March 2016. The impact of NHI is seen on policy, budget, equipment, human resource and implementation of health promotion program. With purposive sampling method, six policy makers, eight service providers and eight service users were selected for in-depth interview. As many as 17 documents were analyzed. Observation conducted at four selected PHC. Data analysis used thematic content analysis. There was no difference of PHC’s functions before and after NHI period. Budget expended for health promotion programs had increased after NHI implemented which it could be opportunity for PHC to make innovations, procure materials and implement better health promotion programs. Capitation budget which could be used for executing health promotion program and some recently implemented health promotion programs in NHI era becomes an evidence that NHI policy has a positive impact in the implementation of health promotion program at PHCAbstrakJaminan Kesehatan Nasional (JKN) telah diimplementasikan di Indonesia pada 1 Januari 2014. Hal tersebut membawa beberapa perubahan pada aspek manajerial pada pusat kesehatan masyarakat (puskesmas). Penelitian ini bertujuan untuk mengidentifikasi dampak dari kebijakan JKN terhadap implementasi program promosi kesehatan di puskesmas di Kota Tangerang Selatan, Indonesia. Penelitian menggunakan metode kualitatif. Data dikumpulkan pada Februari – Maret 2016. Dampak program dilihat dalam hal kebijakan, pendanaan, fasilitas, sumber daya manusia dan pelaksanaan program promosi kesehatan. Dengan menggunakan metode purposive sampling, enam pengambil kebijakan, delapan pemberi layanan dan delapan penerima layanan diambil sebagai informan dalam penelitian ini. Pada analisis dokumen, 17 dokumen telah dianalisis. Observasi dilakukan dengan melihat kegiatan yang dilakukan di empat puskesmas. Analisis data menggunakan analisis konten tematik. Tidak terdapat perbedaan dari fungsi puskesmas sebelum dan setelah adanya JKN. Dana yang digunakan untuk kegiatan promosi kesehatan telah mengalami peningkatan setelah implementasi kebijakan JKN dimana dana tersebut dapat digunakan untuk berinovasi, memberi peralatan dan melakukan promosi kesehatan dengan lebih baik. Dana kapitasi yang dapat digunakan untuk melaksanakan program promosi kesehatan dan beberapa kegiatan promosi kesehatan yang baru dilaksanakan pada saat era JKN menjadi bukti bahwa kebijakan JKN memiliki dampak positif terhadap pelaksanaan program promosi kesehatan di puskesmas.

Health promotion services at public health center is essential to support healthy behavior but still many officers did not have the competence and quality to conduct standard health promotion. West Java implemented health promotion training to 238 officers from 10 districts. The research objective was determining changes in participants’ knowledge and its implementation after training as a basis for sustainable technical guidance. Mixed methods design explanatory sequential as results from training analysis and in-depth interviews of officers. The results showed significant change in participants’ knowledge after training by district, no significant difference between districts and education level, and no significant increase in service coverage. Continuous technical direct guidance for officers by district officials and professional organizations is needed to strengthen service management, coordination, partnerships, integration and recording-reporting. Guidance through social media can be considered for use. Further training is needed to strengthen advocacy, partnership, evaluation. Participants were grouped into four as multivariate cluster analysis result of knowledge level after training and education level.

Acrylamide, which forms in heat-treated foods with high carbohydrate content, is a probable human carcinogen. This study aimed to evaluate the association between dietary acrylamide intake and lung cancer using data from the Japan Public Health Center based Prospective Study. Our study included 85,303 participants who completed a food frequency questionnaire. Cox proportional hazards regression models were used to assess hazard ratios and 95% confidence intervals (CIs) after adjusting for confounders. After 14.3 years and 15.4 years of mean follow-up period, 1187 and 485 lung cancer cases were identified in men and women, respectively. The multivariable-adjusted hazard ratios of 10-µg/day increment in acrylamide intake were 1.01 (95% CI, 0.99–1.02) in men and 0.98 (95% CI, 0.95–1.02) in women. Compared with the lowest quartile of acrylamide intake, the hazard ratios for the highest quartile were 1.13 (95% CI, 0.95–1.33; p for trend = 0.12) in men and 1.03 (95% CI, 0.78–1.36; p for trend = 0.86) in women in the multivariable-adjusted model. Moreover, there was also no significant association observed in the stratified analysis for histological subtypes of lung cancer. This study demonstrated that dietary acrylamide intake was not associated with increased lung cancer risk in the Japanese population.

Sanitation clinic is an innovative health promotion program that is useful to tackle environmental-based diseases. Sanitation clinic activities such as counseling, home care and health interventions. This study aims to know of sanitation clinic activities at Gucialit and Gambut Public Health Center in tackling environment-based illness (PBL). Descriptive research type, using secondary data from sanitation clinic and health profi le of district health offi ce of Lumajang and Banjar 2014-2016, accompanied by indepth interview on information. Sanitation clinic activities obtained at Gucialit and Gambut Public Health Center such as counseling, home care and health intervention are going well. The internal referral system functions optimally, so patient visits at sanitation clinics increase. The existence of sanitation clinics in the Gucialit and Gambut Public Health Centers is very benefi cial in overcoming environmental-based diseases. Every Public health centers is important to carry out sanitation clinics, to support promotive services in tackling environment-based diseases. Abstrak Klinik sanitasi merupakan inovatif program promosi kesehatan yang bermanfaat untuk menanggulangi penyakit berbasis lingkungan. Kegiatan klinik sanitasi seperti konseling, kunjungan rumah (home care) dan intervensi kesehatan. Penelitian ini bertujuan untuk mengetahui kegiatan klinik sanitasi di Puskesmas Gucialit dan Puskesmas Gambut dalam menanggulangi penyakit berbasis lingkungan. Jenis penelitian deskriptif, menggunakan data sekunder laporan klinik sanitasi dan Profil Kesehatan Dinas Kesehatan Kabupaten Lumajang dan Kabupaten Banjar tahun 2014–2016, disertai indepth interview pada informan. Kegiatan klinik sanitasi yang diperoleh di Puskesmas Gucialit dan Puskesmas Gambut seperti konseling, pada tindak lanjut (home care) dan intervensi kesehatan berjalan dengan baik. Sistem rujukan internal berfungsi optimal,sehingga kunjungan pasien di klinik sanitasi meningkat. Keberadaan klinik sanitasi di Puskesmas Gucialit dan Gambut sangat bermanfaat menanggulangi penyakit berbasis lingkungan. Setiap Puskesmas penting untuk melaksanakan klinik sanitasi, untuk mendukung pelayanan promotif dalam menanggulangi penyakit berbasis lingkungan.

 The Indonesian government has set 60 priority districts/cities for stunting handling. Based on this determination, Sumedang is included in the priority district because in 2018 the stunting prevalence rate reached 32%. The selection conducted by Bappeda Sumedang contained 10 villages where the prevalence rate of stunting was high and three of them were villages in Rancakalong. To overcome this, the Rancakalong Community Health Center initiated the “Gempur Stunting” Health promotion Program which has succeeded in reducing the prevalence of stunting from 27.7% to 19.8%, making it an exemplary health promotion program. This research was conducted to determine the success factors of the "Gempur Stunting" health promotion. The results showed that reducing the highest stunting prevalence rate in Sumedang was due to the following supporting factors: (1) variations in community-based activities; (2) Good collaboration and coordination between related sectors, and (3) Reliability of the stunting-fighting health promotion program.

The purpose of this study was to examine the changes in physical activity (PA), physical fitness and psychosocial well-being in early adolescents following implementation of a school-based health promotion program in secondary schools. Methods: Six municipalities in Telemark County, Norway, were recruited into intervention (6 schools) or control groups (9 schools). A total of 644 pupils participated in the study (response rate: 79%). The schools in the intervention group implemented the Active and Healthy Kids program, where the PA component consisted of (1) 120 min/week of physically active learning (PAL) and (2) 25 min/week of physical active breaks. Furthermore, both the intervention and control schools carried out 135 min/week of physical education. The primary outcome was PA. Secondary outcomes were sedentary time, physical fitness, subjective vitality and health-related quality of life (HRQoL) in five domains: physical health, psychological well-being, parent, peers and school. Results: There was a group x time effect on school-based PA (p < 0.05), but not total PA, as well as on physical fitness (p < 0.05) and vitality (p < 0.01). In girls, there also was a group x time effect on three out of the five domains on HRQoL (p < 0.05). Conclusions: A multi-component, school-based health-promotion program with emphasis on the use of PAL led to positive changes in school-based PA levels. Furthermore, positive changes were seen in physical fitness, vitality and HRQoL among early adolescents in a county with a poor public health profile. This might have implications for the development and promotion in schools of general health and well-being throughout adolescence.
