Health and well-being are a function of familial relationships between Native Hawaiians and their land. As a result of settler colonialism, Native Hawaiians face systemic and social barriers, which impede their relationship to land, with implications of adverse health outcomes. This qualitative study explores changes in health among Native Hawaiians, with a specific focus on food systems and the environment. Community-engaged research approaches were utilized to recruit 12 Hawaiian adults. The major themes include the following: (1) health as holistic and a harmonious balance, (2) nutrition transition and current connections to ‘āina (land extending from the mountain to the sea; that which feeds or nourishes), and (3) food sovereignty and community solutions to uplift the Lāhui (Nation of Hawai‘i). Consideration of cultural values, community strengths, and traditional lifestyle practices may address health inequities and changes in food systems related to health that stem from colonization, determinants of health, and environmental changes.

Abstract Purpose of Review The purpose of this review is to describe the combined impacts of the nutrition transition and climate change in Nigeria and analyze the country’s national food-related policy options that could support human and planetary health outcomes. Recent Findings This paper uses a food systems framework to analyze how the nutrition transition and climate change interact in Nigeria affecting both diets and the double burden of malnutrition, resulting in what has been termed the syndemic. Interactions between climate change and the nutrition transition in Nigeria are exacerbating diet-related inequities and will continue to do so if food systems continue on their current trajectory and without significant transformation. Siloed policy actions that attempt to mitigate one aspect of food system risk can create a negative feedback loop in another aspect of the food system. Our analysis finds that Nigeria has five national policies that include actionable steps to address food system insufficiencies; however, each of these policies is constrained by the boundaries of singular nutrition, climate change, and agricultural objectives. The country should consider a coherent policy environment that explicitly identifies and links underlying systemic and institutional drivers between climate change and malnutrition that simultaneously and comprehensively address both human and planetary health outcomes of food systems. Summary The systemic and institutional outcomes of this emerging syndemic—undernutrition, obesity, and climate change—are inexorably linked. Nigeria lacks a coherent policy environment taking on this challenging syndemic landscape. The analysis in this paper highlights the need for Nigeria to prioritize their national nutrition and agricultural and climate policies that uncouple feedback loops within food systems to address climate change and malnutrition in all its forms.

AbstractThe nutrition transition transforms food systems globally and shapes public health and environmental change. Here we provide a global forward-looking assessment of a continued nutrition transition and its interlinked symptoms in respect to food consumption. These symptoms range from underweight and unbalanced diets to obesity, food waste and environmental pressure. We find that by 2050, 45% (39–52%) of the world population will be overweight and 16% (13–20%) obese, compared to 29% and 9% in 2010 respectively. The prevalence of underweight approximately halves but absolute numbers stagnate at 0.4–0.7 billion. Aligned, dietary composition shifts towards animal-source foods and empty calories, while the consumption of vegetables, fruits and nuts increases insufficiently. Population growth, ageing, increasing body mass and more wasteful consumption patterns are jointly pushing global food demand from 30 to 45 (43–47) Exajoules. Our comprehensive open dataset and model provides the interfaces necessary for integrated studies of global health, food systems, and environmental change. Achieving zero hunger, healthy diets, and a food demand compatible with environmental boundaries necessitates a coordinated redirection of the nutrition transition. Reducing household waste, animal-source foods, and overweight could synergistically address multiple symptoms at once, while eliminating underweight would not substantially increase food demand.

The COVID-19 pandemic has provoked a range of economic shocks, food systems shocks, public health crises and political upheavals across the globe, prompting a rethink of associated global systems. Prepandemic anticolonial movements that challenged hierarchies of race, space, gender and expert knowledge in global health took on new meaning in the context of the unequal impacts of the SARS-CoV-2 virus as it moved through different kinds of spaces and distinct political contexts. In light of these dynamics, and the desire of many current practitioners in global health to reimagine the future, the need for critical analyses of the recent past have become more urgent. Here we challenge linear understandings of progress in global health—with a focus on the field of nutrition—by returning to consider a previous cycle of dramatic social, political and economic change that prompted serious challenges to the dominance of Western powers and US-based philanthro-capitalists. With a ‘global’ health and nutrition audience in mind, we put forward considerations on why a better understanding of the continuities and divergences between this past and the present moment are necessary to challenge a status quo that was, and is, highly flawed.

Abstract Background : The main purpose of this paper is to understand the multidimensional phenomenon of the nutrition transition in Malaysia, from 1980 to 2014, to inform future policies for a healthier nation. Methods : Food and health data were obtained through Food Balance Sheets, Malaysian Adult Nutrition Survey (MANS) and National Health and Morbidity Surveys (NHMS) for year-to-year review. Interaction between changes in food supplies and dietary trends and the progression of diet-related diseases and risk factors in tandem with demographic and socioeconomic transitions were observed using quasi-historical approach. Results : The period-under-review has seen Malaysia becoming more affluent, urbanised and modernised. Energy supply for Malaysian population remained consistently in excess of average calorie needs by a minimum of 30%. There were significant signs of shifting food trends, particularly in the supply of wheat (+56.5%), rice (-23.7%), sugar and sweeteners (+23.9%), meat (+49.3%), fish and seafood (+38.7%), and eggs (+55.7%). The plant/animal protein ratio has decreased over time. Prevalence of NCD and associated risk factors has increased rapidly, some as high as 170%, despite various policy efforts to reduce them. Conclusion : The study highlights the importance of policymakers taking a relook into its policies and strategies, and formulate sustainable, comprehensive and multifaceted actions together with all relevant stakeholders to ensure a conducive, healthy and nutritious food systems and environment for its population.

Overview: South Asian countries have experienced a remarkable economic growth during last two decades along with subsequent transformation in social, economic and food systems. Rising disposable income levels continue to drive the nutrition transition characterized by a shift from a traditional high-carbohydrate, low-fat diets towards diets with a lower carbohydrate and higher proportion of saturated fat, sugar and salt. Steered by various transitions in demographic, economic and nutritional terms, South Asian population are experiencing a rapidly changing disease profile. While the healthcare systems have long been striving to disentangle from the vicious cycle of poverty and undernutrition, South Asian countries are now confronted with an emerging epidemic of obesity and a constellation of other non-communicable diseases (NCDs). This dual burden is bringing about a serious health and economic conundrum and is generating enormous pressure on the already overstretched healthcare system of South Asian countries.Objectives: The Nutrition transition has been a very popular topic in the field of human nutrition during last few decades and many countries and broad geographic regions have been studied. However there is no review on this topic in the context of South Asia  as yet. The main purpose of this review is to highlight the factors accounting for the onset of nutrition transition and its subsequent impact on epidemiological transition in five major South Asian countries including Bangladesh, India, Nepal, Pakistan and Sri Lanka. Special emphasis was given on India and Bangladesh as they together account for 94% of the regional population and about half world’s malnourished population.Methods: This study is literature based. Main data sources were published research articles obtained through an electronic medical databases search.

Overweight and obesity affect over half a billion people globally and represent major public health concerns because excess weight gain is a key risk factor for non-communicable diseases. This chapter presents an overview of global trends in obesity, considering both developed and developing country contexts. It describes how this pandemic is rooted in the “nutrition transition” taking place around the world as a result of a globalized agri-food system that is changing the quantity, types, and desirability of foods available for consumption. In some countries, this is contributing to a “double burden of malnutrition,” as the problem of undernutrition now coexists alongside an increasing prevalence of over-nutrition. An overview of key policy responses and policy instruments that governments can utilize to address obesity is provided, recognizing that a holistic food systems response is needed to address the global challenge of obesity. The remainder of the chapter focuses on the food security and obesity challenges facing the Caribbean Community (CARICOM) and profiles a “farm to fork” school feeding project in the island nation of St. Kitts-Nevis that was designed to reduce obesity and improve food security among children. Implementation and key results of this integrated project are presented, including the core components of agricultural production, procurement of local produce, and children’s consumption of nutritious foods. The chapter concludes by identifying lessons for supporting “farm to school”-type projects as a possible food systems response to addressing obesity and food insecurity.

The present report summarises the symposium ‘Nutrition Transition and the Global Burden of Type 2 Diabetes’ and a workshop on strategies for dietary interventions to prevent type 2 diabetes held by the Global Nutrition and Epidemiologic Transition Initiative, Boston, MA, USA in November 2011. The objectives of this event were to bring attention to the global epidemic of type 2 diabetes in light of the ongoing nutrition transition worldwide, especially in low- and middle-income countries, and to highlight the present evidence on key dietary risk factors contributing to the global diabetes burden. The meeting put forward ideas for further research on this topic and discussed practical recommendations to design and implement culturally appropriate dietary interventions with a focus on improving carbohydrate quality to help alleviate this growing health problem.
