Liver disease has emerged as a critical issue in the management of human immunodeficiency virus (HIV) victims. This study was carried out to investigate the derangement in indices of liver function in HIV-infected patients in Makurdi, North Central Nigeria. One hundred and fifty males and females adults within the age range of 21- 50 years were enrolled for this study. One hundred of these participants were confirmed HIV positive, out of which fifty were on various antiretroviral drugs including the nucleoside / nucleotide reverse transcriptase inhibitors and the non-nucleoside transcriptase inhibitors classes of antiretroviral drugs. The remaining fifty persons who were HIV- negative served as the control group. Blood samples from the studied persons were analysed for alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, total protein, albumin, total bilirubin and conjugated bilirubin using automated clinical chemistry analyser, Hitachi 902 (Roche Diagnostic GMBH, Germany). The HIV- patients with or without antiretroviral drugs had their serum levels of alanine aminotransferase, aspartate aminotransferase and alkaline phosphatase significantly raised (p< 0.05) compared with the control subjects. A significant rise (p< 0.05) was observed in serum level of total protein in the HIV patients who were yet to commence antiretroviral drugs when compared to the control subjects. However, the albumin levels of the HIV patients on the antiretroviral drugs and those who were yet to commence the antiretroviral drugs decreased significantly (p< 0.05) when compared with that of the control. The total bilirubin level of the HIV patients on antiretroviral drugs showed significant increase (p< 0.05) compared with the control subjects and the HIV patients who were not on antiretroviral drugs. This study reveals that both HIV infection itself and the antiretroviral drugs cause hepatic malfunction

AbstrakLatar Belakang : Human Immunodeficiency Virus (HIV) merupakan jenis retrovirus yang menginfeksi sistem kekebalan tubuh manusia yang menyebabkan Acquired Immunodefiency Syndrome (AIDS),. Kehadiran kuman TB menyebabkan progresivitas kasus ko-infeksi TB-HIV bertambah buruk sehingga mengancam jiwa penderitanya. Penelitian ini bertujuan untuk mengetahui gambaran kesintasan satu tahun pasien ko-infeksi TB-HIV berdasarkan waktu awal pengobatan ARV.Metode : Penelitian ini menggunakan desain kohort retrospektif di RSPI Prof. Dr. Sulianti Saroso tahun 2013-2015. Sumber data yang digunakan berasal dari penelusuran pada register pra ARV dan ARV, Form TB 01, buku monitoring ARV, monitoring farmasi ARV, pelacakan ikhtisar ARV dan status rekam medis. Pengumpulan data melibatkan petugas Pokja HIV/AIDS dan dokter (validasi diagnosa dan kovariat) yang di blind atas hipotesis penelitian.Hasil : Probabilitas ketahanan hidup kumulatif satu tahun pasien ko-infeksi TB-HIV yang mendapatkan awal pengobatan ARV di RSPI Prof. Dr. Sulianti Saroso periode Januari 2011-Mei 2014 adalah 81,5%. Probabilitas ketahanan hidup pasien TB-HIV berdasarkan waktu awal menunjukan bahwa ketahanan hidup satu tahun pada pasien yang mendapatkan pengobatan ARV pada fase intensif adalah 89,1% dan pada pasien yang mendapatkan pengobatan ARV pada fase lanjut adalah 74,5%.Kesimpulan : Pasien ko-infeksi TB-HIV yang mendapatkan ARV pada fase intensif cenderung memiliki probalitas ketahanan hidup yang lebih besar di tahun pertama dibandingkan pasien ko-infeksi TB-HIV yang mendapatkan ARV pada fase lanjut. Abstract Background : Human Immunodeficiency Virus (HIV) is a type of retrovirus that infects the human immune system that causes Acquired Immunodefiency Syndrome (AIDS). The presence of TB germs cause progression of cases of co-infection of TB-HIV getting worse so threatening sufferers. This study aims to reveal the one-year survival rate of patients co-infected TB-HIV based on time start of antiretroviral treatment.Methods : This study used a retrospective cohort design in RSPI Prof. Dr. Sulianti Saroso years 2013-2015. The data used comes from searches on the register of pre ARV and ARV form, TB Form, the book ARV monitoring, monitoring of pharmaceutical drugs, ARV overview and status tracking of medical records. The data collection involves the officer HIV / AIDS and the doctor (validation diagnosis and covariates) were in blind on the research hypothesis. Results : The cumulative probability of survival for one year patients co-infected TB-HIV get antiretroviral treatment early in RSPI Prof. Dr. Sulianti Saroso period January 2011-May 2014 was 81.5%. The probability of survival for patients of TB-HIV based on the initial time showed that one-year survival in patients receiving antiretroviral treatment in the intensive phase was 89.1% and in patients receiving antiretroviral treatment in advanced phases was 74.5%.Conclusion : Co-infected TB-HIV patients get antiretroviral drugs in the intensive phase tend to have a probability of survival is greater in the first year compared to co-infection TB-HIV patients get antiretroviral drugs in the advanced phase

Purpose This paper aims to evaluate hematological parameters, blood glutathione (GSH), serum glucose-6-phosphate dehydrogenase (G6-PD) and liver function in favism animals' models after oral intake with a mixture of pickled olives and Vicia faba (V. faba) seeds. Design/methodology/approach Favism is a life-threatening hemolytic crisis. It results from the ingestion of V. faba by the individuals. The hemoglobin (Hb), hematocrit (Hct), red blood cells (RBCs), white blood cells (WBCs), serum glucose, blood GSH, serum G6-PD, serum thiobarbaturic acid reactive substances (TBARS), liver protein and liver function were evaluated after oral administration with a mixture of pickled olives with V. faba seeds in favism animals' models. Findings The favism-, vicine- and convicine-treated animals showed a significant decrease (p < 0.01) in Hb (6.42, 7.23 and 5.96 g/dl), Hct (25.4, 26.4 and 25.1%), RBCs (2.56, 2.45 and 2.60 106 cells/mm3, WBCs (4.35, 4.25 and 4.30 103 cells/mm3), serum glucose (95.8, 97.1 and 96.5 mg/dl), blood GSH (24.7, 26.6 and 23.8 mg/dl), serum G6-PD (15.8, 15.9 and 15.7 U/L), serum aspartate aminotransferase (6.35, 6.59 and 5.97 U/L), alanine aminotransferase (4.49, 4.61 and 4.50 U/L), total protein (6.54, 6.59 and 6.40 g/dl), albumin (3.84, 3.91 and 3.75 g/dl), globulin (2.70, 2.48 and 2.65 g/dl) and liver protein (3.37, 3.10 and 3.42 g/g tissue) but a significant increase (p < 0.01) in serum TBARS (38.7, 38.9 and 39.4 nmol/dl), alkaline phosphatase (275, 271 and 281 U/L) and total bilirubin (0.93, 0.89 and 0.91 mg/dl) compared to Hb (16.3 g/dl), Hct (45.3%), RBCs (5.80 106 cells/mm3), WBCs (9.45 103 cells/mm3), serum glucose (96.5 mg/dl), blood GSH (39.7 mg/dl), serum G6-PD (36.1 U/L), serum TBARS (18.0 nmol/dl), serum aspartate aminotransferase (19.8 U/L), alanine aminotransferase (9.23 U/L), alkaline phosphatase (214 U/L), total bilirubin (0.57 mg/dl), total protein (8.76 g/dl), albumin (4.85 g/dl), globulin (3.91 g/dl) and liver protein (6.28 g/g tissue) in control group. The oral administration with pickled olives + V. faba, pickled olives + vicine and pickled olives + convicine into favism animals' models, vicine-treated animals and convicine-treated animals, respectively pushed all the above-mentioned parameters to near the control levels. Originality/value V. faba contains vicine and convicine glycosides. Vicine and convicine glycosides in V. faba are hydrolyzed by intestinal microflora to aglycones divicine and isouramil, respectively. Divicine and isouramil are highly reactive compounds generating free radicals where divicine and isouramil are the main factors of favism. The ß-glucosidase in pickled olives converts both vicine and convicine glycosides into aglycones divicine and isouramil, respectively, in aerobic condition outside the human body (in inactive forms) and prevent these glycosides to cause favism.

AbstractIntroductionHuman immunodeficiency virus and protein energy malnutrition are still prevalent in Nigeria and the occurrence of the two conditions together confers a poor prognosis. The aim of this study was to determine the current categories of malnutrition amongst under 5 children in Lagos, document their HIV status and determine any peculiarities in the clinical features, haematological and some biochemical profile in these children.MethodsThe study was a prospective crossectional study conducted at the Paediatric department of the Lagos University Teaching Hospital and the Massey Street Children’s Hospital over a 6 month period. All the subjects had anthropometry, HIV testing, full blood count and serum proteins done. The factors associated with HIV status were determined with the logistic regression analysis.ResultsTwo hundred and fourteen (214) malnourished children ≤5 years were recruited into the study and 25(11.7%) were HIV positive. One hundred and five (49.1%) of the participants had moderate malnutrition while 25.2% had severe forms of malnutrition. Fever, cough and diarrhea were the commonest symptoms. Severe wasting, oral thrush, dermatoses and splenomegaly were seen more commonly in the HIV positive subjects. The haematological indices were comparable in the two groups, however, the total protein was significantly higher in the HIV positive subjects compared to the negative group (p=0.042). Multivariate analysis showed that the total protein (p=0.001) and platelet count (p=0.016) could significantly predict the occurrence of HIV in the malnourished childrenConclusionThe presence of severe wasting, oral thrush, diarrhea, splenomegaly, thrombocytopenia and high total proteins in malnourished children should heighten the suspicion of possible underlying associated HIV infection. This study reinforces the recommendation by the World Health Organisation (WHO) that all malnourished children should have mandatory HIV screening.

Background: Schizophrenia is associated with a deficiency of dietary antioxidants like vitamin B6, B9, and B12 resulting in defective methylation leading to hyperhomocysteinemia. Hyperhomocysteinemia causes mitochondrial DNA damage, oxidative stress, vascular damage, and lipid peroxidation. Oxidative stress and increase in reactive oxygen species result in 8-oxodG production which induces apoptosis of both astrocytes and thyrocytes thus predisposing them to thyroid dysfunction and neurodegeneration. Furthermore, the presence of excessive free radicals increases thyroid thermogenesis causing hyperthyroidism or its excess may cause hypothyroidism by inhibiting iodide uptake. In the present study, we evaluated the various biomarkers associated with thyroid dysfunction in schizophrenics.Materials and Methods: 288 patients suffering from schizophrenia and 100 control subjects were screened for liver function tests (LFTs) such as alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), and total bilirubin (TB). Also, the stress markers, namely malondialdehyde (MDA), homocysteine, cysteine, methionine, the thyroid profile including triiodothyronine (T3), thyroxine (T4), thyroid-stimulating hormone (TSH), thyroxine peroxide antibody (TPO-Ab); TSH receptor-Ab (TSHr-Ab), dietary antioxidants, lipids, cytokines, aminoacids and hormones, vitamins and trace elements, and other biochemical parameters.Results: The LFTs showed elevated levels of ALT (45.57 ± 4.87 Vs. 26.41 ± 3.76 U/L), AST (40.55 ± 1.34 Vs. 21.92 ± 3.65 U/L), ALP (121.54 ± 4.87 Vs. 83.76 ± 5.87 U/L), and total bilirubin (2.63 ± 0.987 Vs. 1.10 ± 0.056 mg/dl), in schizophrenics than controls. Increased levels of MDA (3.71 ± 0.967 Vs. 1.68 ± 0.099) and homocysteine (17.56 ± 2.612 Vs. 6.96 ± 1.987 μmol/L were observed in schizophrenics compared to the controls, indicating increased stress. Levels of cysteine and methionine were decreased in schizophrenics than the controls (1.08 ± 0.089 Vs. 4.87 ± .924 μmol/L and 17.87 ± 1.23 Vs. 99.20 ± 5.36 μmol/L). The levels of TPO-Ab (IU/ml), Tg-Ab (pmol/L), and TSHr-Ab (IU/L) were observed to be higher in the patients’ group as compared to control subjects (9.84 ± 2.56 Vs. 5.81 ± 1.98, 55.50 ± 2.98 Vs. 32.95 ± 2.87 and 2.95 ± 0.0045 Vs. 1.44 ± 0.0023 respectively). Levels of Vitamin B6, B9, and B12 were also significantly decreased in the patients compared to the healthy controls.Conclusion: The schizophrenics, demonstrated altered liver function, increased stress markers, and decreased dietary antioxidants. Reduced primary and secondary antioxidant levels, may result in hyperhomocysteinemia and cause further DNA and mitochondrial damage. Therefore, homocysteine and/or prolactin levels may serve as candidate prognostic markers for schizophrenia. Also, both neurological symptoms and the susceptibility to thyroid disorders may be prevented in the initial stages of this debilitating disorder by appropriate dietary supplementation of antioxidants which can rectify a reduction in primary and secondary antioxidants, and disturbed prolactin-serotonin-dopamine interactions in schizophrenics.

Human immunodeficiency virus (HIV) causes breakdown of the immune system and predisposes patients to various opportunistic infections and neoplasms. However, many patients may not be aware of the HIV infection before the development of their first HIV related complications. We reported four unrecognised HIV patients presenting to our accident and emergency department with common complications of HIV infection and the acquired immunodeficiency syndrome (AIDS). Although not as common as in America, emergency physicians in Hong Kong still have to take care of patients with unknown HIV status. The common presentations of HIV patients will be discussed. A high index of suspicion and knowledge of common HIV/AIDS complications are required for managing these patients.
