Chondromyxoid fibroma; a rarely found tumor, contributing <1% of all primary bone neoplasm. We reported a 4-year female child with a 1-year history of nasal obstruction and facial swelling. Large enhanced lesion with amorphous densities spreading into the right cribriform plate and floor of sphenoid sinus, laterallyinto right lamina papyracea, inferolaterally into medial wall of maxillary sinus, posteriorly into nasopharynx and superior aspect of oropharynx was appreciated in CT scan. Mass was excised by Caldwell Luc’s endoscopic medial maxillectomy via the sublabial approach. CMF was confirmed histologically in post-operative biopsy. A 69-year-old hypertensive woman with a hyperdynamic, left brachio-basilic dialysis fistula presented with a long history of throbbing in her head, swelling of the left side of the face and two months of right visual loss with gross swelling of the right optic disc. Tight stenosis of left brachiocephalic vein was found to be causing retrograde flow into the left jugular vein which normalised after dilatation and stenting with resolution of the papillodema.

Lipoma, a benign neoplasm of adipocytes, is the most common mesenchymal soft tissue tumor of adulthood, but rarely present with huge sizes in their cutaneous localization. Liposarcomas are the second most common soft tissue sarcoma following malignant brous histiocytoma. They comprise approximately 16-18% of all malignant soft tissue tumours. It is most common in the trunk followed by lower extremity, upper extremity and head and neck.. While lipomas can be clinically diagnosed , transformation of lipomas into liposarcoma is rare. Here we report the case of a 55 gentleman, who came with a history of swelling over the right shoulder with a probable malignant transformation due to history of long duration of presence of swelling. Management is usually by wide excision. Local radiation and chemotherapy may be necessary for high grade lesions.

Aggressive Fibromatosis (AF) is a rare, benign neoplasm originating from musculoaponeurotic stromal structures characterized by aggressive growth and infiltration of local tissues. To date, there are only six previously reported cases of AF involving the larynx. Our case involves a 70-year-old female with a 5-year history of hoarseness and an enlarging neck mass consistent with aggressive recurrent fibromatosis. MRI showed a large, solid mass arising from the left anterior cervical space displacing the trachea and upper airway to the right. A year after the initial radical resection, the patient presented with recurrence. A second radical excision was performed not including a laryngectomy. Radiation therapy was consulted for possible local treatment to prevent subsequent recurrence of tumor. Literature has reported up to 40-70% recurrence rates of AF within 18 months. There is minimal literature to guide treatment. Surgery seems to be the treatment of choice for head and neck AF, but management of recurrence is controversial.

<p>Rhinosporidiosis is a chronic granulomatous disease of mucous membranes caused by <em>Rhinosporidium seeberi</em>. Most frequently affected sites are nose and nasopharynx followed by eye.<strong> </strong>Report a rare case of 13-year-old female with rhinosporidiosis in a bizarre way of presentation as a cyst in left cheek. A 13-year-old female child hailing from West Bengal presented to ENT OPD with complaints of swelling in the left cheek for 6 months gradually increasing in size. On examination left side of cheek a cystic swelling extending from angle of mouth to lateral side of face was seen. Magnetic resonance imaging (MRI) neck plain and contrast showed large well defined T2 hyperintense cystic lesion in subcutaneous plane measuring about 5.7 cm (transverse)×6.0 cm (cephalocaudal)×5.5 cm (anteroposterior) with post contrast wall enhancement. Patient planned for excision and histopathological examination was consistent with rhinosporidiosis. Rhinosporidiosis continues to be a conundrum necessitating further literature and research on this. The difficulty in culturing the organism and its preternatural presentations add up to difficulties in diagnosing. Emphasise the quaint feature of its presentation as a cyst in cheek as primary site without any focus of infection in the respiratory tract and with negative history of traumatic inoculation.</p>

Cemento-ossifying fibroma is a rare benign neoplasm occurring in the tooth bearing areas of mandible and maxilla. Here we report a case of an adolescent boy who presented with swelling in the right side of mandible. It was excised surgically and histopathological diagnosis of cement-ossifying fibroma was made. His mother also had history of cemento-ossifying fibroma 10 years back. This case is being reported for its rare incidence and familial occurrence.

Many muscle diseases show common pathological features although their etiology is different. In primary muscle diseases a characteristic finding is myofiber necrosis. The mechanism of myonecrosis is unknown. Polymyositis is a primary muscle disease characterized by acute and subacute degeneration as well as regeneration of muscle fibers coupled with an inflammatory infiltrate. We present a case of polymyositis with unusual ultrastructural features indicative of the basic pathogenetic process involved in myonecrosis.The patient is a 63-year-old white female with a one history of proximal limb weakness, weight loss and fatigue. Examination revealed mild proximal weakness and diminished deep tendon reflexes. Her creatine kinase was 1800 mU/ml (normal < 140 mU/ml) and electromyography was consistent with an inflammatory myopathy which was verified by light microscopy on biopsy muscle. Ultrastructural study of necrotizing myofiber, from the right vastus lateralis, showed: (1) degradation of the Z-lines with preservation of the adjacent Abands including M-lines and H-bands, (Fig. 1), (2) fracture of the sarcomeres at the I-bands with disappearance of the Z-lines, (Fig. 2), (3) fragmented sarcomeres without I-bands, engulfed by invading phagocytes, (Fig. 3, a & b ), and (4) mononuclear inflammatory cell infiltrate in the endomysium.

Popliteal artery thrombosis may present as a complication of an osteochondroma located in the vicinity of the knee joint. This is a case report of a 26-year-old man with symptoms of the right lower extremity ischaemia without a previous history of vascular disease or trauma. Plain radiography, magnetic resonance angiography and Doppler ultrasonography documented the presence of an osteochondrous structure of the proximal tibial metaphysis, which displaced and compressed the popliteal artery, causing its occlusion due to intraluminal thrombosis..The patient was operated and histopathological examination confirmed the diagnosis of osteochondroma.

The challenges of including factual information in public policy and political discussions are many. The difficulties of including scientific facts in these debates can often be frustrating for scientists, politicians and policymakers alike. At times it seems that discussions involve different languages or dialects such that it becomes a challenge to even understand one another’s position. Oftentimes difference of opinion leads to laws and regulations that are tilted to the left or the right. The collaborative balancing to insure public and natural resource interests are protected ends up being accomplished through extensive litigation in the courts. In this article, the author discusses the history of environmental balancing during the past three decades from the perspective of a field biologist who has used the strength of our policies, laws and regulations to fight for the protection of our Commonwealth’s aquatic resources. For the past 7 years, the author has taken over the reins of “the most powerful environmental agency in Pennsylvania” and charted a course using science to properly represent natural resource interests in public policy and political deliberations.

SummaryA 5-month-old, 13.5 kg, female Corriedale sheep was referred to the Veterinary Medicine Teaching Hospital, with a history of traumatic injury of the cervical spine followed by non-ambulatoric tetraparesis that occurred 2 weeks before being admitted to the hospital. At admission, malalignment of the cervical spine with the cranial part of the neck deviating to the right was noted. Neurological examinations identified the absence of postural reactions in both forelimbs, mildly decreased spinal reflexes, and normal reaction to pain perception tests. Radiography revealed malalignment of the cervical vertebrae with subluxations at C1–C2 and C2–C3, and a comminuted fracture of the caudal aspect of C2. The sheep was euthanized due to a presumed poor prognosis. Necropsy and histopathological findings confirmed injuries of the cervical spine from C1 to C3, which were consistent with the clinical finding of tetraparesis in this case. This paper presents a rare case of multiple subluxations of the cervical spine caused by blunt force trauma in a young sheep. These results highlight the importance of an astute clinical diagnosis for such an acute cervical spine trauma and the need for prompt surgical correction for similar cases in the future.
