The aim of the study was to study the compliance of the list of drugs for the treatment of malignant neoplasms in the territorial state guarantee programs (TSGP) with the list of vital and essential drugs (VED) in the constituent entities of the Russian Federation and the practice of purchasing drugs at the expense of regional preferential provision (RPP) funds. Material and methods. The analyzed territorial programs of state guarantees were compared with the list of drugs for RPP for the patients with malignant neoplasms with the VED. An assessment of medication procurement in 2019 and 2020 was carried out and compared with the current list of RLO in all constituent entities of the Russian Federation. Results. Only 14% of the Russian Federation regions in 2019 and 20% in 2020 met the territorial state program of guarantees requirement. The average number of medications included in the territorial state program of guarantees was 73 INNs in 2019 and 100 INNs in 2020. The number of regions with a complete list of drugs accounted for 7% to 33% in the corresponding Federal Districts. In 2020 the number of regions with a complete list of medications in a territorial program of state guarantees did not change. Discussion. Reduction of medications from the VED purchased at the expense of RPP reduces the number of possible treatment regimens, and limits oncologists’ ability to provide care on an outpatient basis. Conclusion. Most Russian Federation regions violate the requirements of the territorial program of state guarantees do not harmonize the list of medications in the Program with EDL. The practice of purchasing medications from the territorial program of state guarantees indicates the absence of finance mechanisms for providing medications to patients at the outpatient stage of treatment.

In the Russian Federation (RF) a steady growth of morbidity with oncologic diseases is observed. An important factor of reduction of negative influence of oncopathology on the parameters of public health is provision of the population with specialists and beds of oncological profile, as well as their effective use. Aim. To determine the status and identify dynamics of the main parameters of use of beds of oncological profile of the state healthcare system of RF, federal districts and constituent entities of RF in comparison with tendencies of parameters of morbidity with malignant neoplasms, and also of provision of the population with medical personnel in the period from 2010 to 2019. Materials and Methods. Using the data of the Federal statistical observation forms (№30, 47, 14LC, 7) based on the calculation of absolute and relative parameters by the descriptive statistics method, the analysis of the main parameters of the use of bed resources of oncology profile was carried out in comparison with the parameters of morbidity with malignant neoplasms and provision of the population with medical personnel in 24-hour and day-stay hospitals in the Russian Federation, federal districts and constituent entities of the Russian Federation in 2010-2019. Results. During the study period, with the underlying increase in the morbidity of the population with diseases of neoplasm class (primarily, malignant) in the Russian Federation, there was found a regular increase in: the absolute number of oncological beds for 24-hour stay by 5.216 beds (+16.8%), provision with these beds from 2.17 to 2.47 per 10 000 population (+13.8%), hospitalization rate from 6.1 to 9.6 per 1000 population (+57.4%); and a decrease in: average stay in an oncological bed by 3.7 days (-30.6%, from 12.1 to 8.4 days), and the average bed occupancy by 15 days per year (-4.3%, from 345 to 330 days). Mortality in cancer beds increased from 0.76% in 2010 to 0.95% in 2019 (by 25.0%). There was an increase in the provision of the population with oncological beds in day-stay hospitals by 3.4 times, in day-stay hospitals of polyclinics by 63.6%. Conclusion. With the underlying growth of oncological morbidity in the country, the bed capacity of the oncological profile of 24-hour and day hospitals has significantly increased, with a high level of disproportional development of the bed capacity both between federal districts and between the subjects of the Russian Federation.

Background. The analysis of the quality of medical care for children with cancer is based on statistical data. Evaluation of the results is also the basis of the strategy for the development of medical care for this category of patients.Objective. Our aim was to analysie the main indicators characterizing medical care for children with cancer in the North-Caucasian, Volga, Urals, Siberian and Far Eastern Federal Districts.Methods. In ecological study the operative reports for 2017 of the executive authorities in the sphere of health protection of 44/48 (91.6%) subjects of the Russian Federation that are part of the listed above federal districts have been analyzed (Chuvash Republic, Irkutsk Region, Republic of Sakha (Yakutia) and Khabarovsk Territory did not provide data).Results. The number of children’s population was 15 555 306 people (0–17 years), the number of children’s oncological beds — 1166 (0.7 per 10 thousand 0–17 years), the average number of days of berth employment in the year — 324.6 bed days. In 13 (29.5%) subjects of the department of pediatric oncology are absent, in 6 (13.6%) — there are no children’s oncological beds. The number of doctors providing medical care to children with cancer is 213, of which 132 (62%; 0.08 for 10 thousand 0–17 years) have a certificate of a pediatric oncologist. In 6 (13.6%) subjects there are no doctors-children oncologists (6 more (13.6%) subjects did not provide data). The incidence of malignant neoplasms was 13.1 (per 100.000 0–17 years), the prevalence of 93.1 (per 100.000 0–17 years), mortality rate 2.5 (per 100.000 0–17 years), one-year lethality — 6.8%. 10.1% of patients were actively detected. 781 (38.3%) of primary patients were sent to medical organizations for federal subordination, 39 (1.9%) of primary patients left the territory of the Russian Federation.Conclusion. Obvious accounting defects (low incidence rate) and lack of reliable follow-up data (estimation of mortality levels is difficult) are eliminated by introducing electronic accounting systems. For reliable estimation of the level of provision of the population with children’s cancer beds and the percentage of patients sent for treatment in medical organizations of federal subordination, audit of patients’ illnesses is necessary. The traditional problems — the deficiency of doctors and children’s oncologists and a low percentage of patients identified actively — must be addressed through the reform of the training program for medical personnel.

One of the malignant neoplasms, which is associated with exposure to only one factor - asbestos fibers, is malignant mesothelioma. At the same time, today there is already convincing evidence of its polyetiology. This indicates the urgent need to expand research in this direction in order to develop targeted preventive measures.

Purpose of the study. Healthcare budget impact analysis in the Russian Federation of providing adult patients with chronic obstructive pulmonary disease (COPD) with basic therapy for outpatient treatment. Material and methods. The hypothesis of reducing costs for the treatment of exacerbations of COPD was tested while providing all patients with basic therapy on an outpatient basis. The model calculates the direct medical costs of drug provision and treatment of exacerbations in patients with COPD in the current situation and when all patients with COPD are covered with basic therapy as part of a program of preferential drug provision. The simulation period is 1 year. The analysis does not take into account social losses associated with disability, payment of disability certificates, reduced labor productivity and mortality. Results. The cost of providing all patients with COPD with the recommended basic drug therapy on an outpatient basis is substantially offset by a decrease in the cost of treating exacerbations: drug costs should increase by 14.1 billion rubles compared with the current situation, while the cost of providing medical care to patients with exacerbations is reduced by 9.7 billion rubles. As a result, the difference in direct medical costs, or the necessary additional investments, is 4.4 billion rubles. The price of drugs used for basic therapy has the greatest impact on the size of the required additional investments. Conclusion. Providing all patients with COPD with the recommended basic drug therapy on an outpatient basis, we can expect a decrease in the number of moderate exacerbations per year by 14.7% and severe exacerbations by 31.2%. Taking into account not only medical, but also socio-economic costs can not only offset additional investments, but also demonstrate significant savings. When implementing a pilot project of preferential drug provision for patients with COPD in a specific region of the Russian Federation, it is possible to reduce the cost of purchasing medicines when forming a municipal order.

The article presents the results of a comparison of the Orphan Drugs Register approved for use in the United States and the 2020 Vital and Essential Drugs List approved on October 12, 2019 by Order of the Government of the Russian Federation No. 2406-r. The comparison identified 305 international non-proprietary names relating to the main and/or auxiliary therapy for rare diseases. The analysis of the market of drugs included in the Vital and Essential Drugs List, which can be used to treat rare (orphan) diseases in Russia was conducted.

The author offers mapping and geoecological analysis of the Russian Federation regions presence in the state program “Environmental Protection”. The unequal distribution of the program’s targets and activities in different regions is revealed. A considerable number of relevant environmental problems for several mentioned regions have not been reflected in the program. It is important to increase the area of specially protected natural areas for a significant number of subjects of the Russian Federation. The status “part of the territory occupied by specially protected natural territories of Federal value in the total area of the subject of the Russian Federation” is recommended to be assigned all regions of Russia. Identification and elimination of objects of accumulated environmental damage that threat to the Volga river is relevant, at least for all the regions in which the Volga flows. Not all regions with a high level of air pollution and/or large masses of air pollutants have the indicator “reduction of total emissions for the reporting year”. It is necessary to increase the Program of measures for the protection of rare and endangered species of plants and to expand the list of regions in which it is planned to protect rare and endangered species of animals.

Проанализированы отчеты фирмы «Барт» и годовые отчеты International Hop Growers’ Convention в период 2007-2020 гг. Приведены и систематизированы данные Федеральной службы государственной статистики Российской Федерации, по Чувашской Республике и официального интернет-портала органов власти Чувашской Республики по формированию внутреннего рынка хмеля. Предложены меры регионального регулирования отрасли, которые согласованы с утверждённой Концепцией развития хмелеводства в Чувашской Республике на 2020-2025 гг. в рамках государственной программы. The article presents the results of a comprehensive study on the market of hop production and processing. The reports of the company «Bart» and the annual reports of International Hop Growers'are analyzed Convention in the period 2007-2020. The data of the Federal State Statistics Service of the Russian Federation on the Chuvash Republic and the official Internet portal of the authorities of the Chuvash Republic on the formation of the domestic hop market are presented and systematized. The measures of regional regulation of the industry are proposed, which are coordinated with the approved Concept of hop growing development in the Chuvash Republic for 2020-2025 within the framework of the state program.
