Observation on the Preventive Effect of Hydrocolloid Dressing on High-risk Pressure Ulcers in Patients with Cerebral Apoplexy in Convalescent Period

The Mehran risk score (MRS) was used to classify patients with coronary heart disease and evaluate the preventive effect of alprostadil on contrast-induced nephropathy (CIN) after percutaneous coronary intervention. The patients (n = 1146) were randomized into an alprostadil and control group and then divided into 3 groups on the basis of the MRS: low-risk, moderate-risk, and high-risk groups. The primary end point was the occurrence of CIN (alprostadil + hydration vs simple hydration treatment); secondary end points included serum creatinine, blood urea nitrogen, creatinine clearance rate, cystatin C, interleukin-6, C-reactive protein, proteinuria, and differences in the incidence of major adverse events. In the low-risk, moderate-risk, and high-risk groups, the incidence of CIN in the control and alprostadil group was 2.9 versus 2.6% ( P = .832), 11.4 versus 4.9% ( P = .030), 19.1 versus 7.7% ( P = .041), respectively. Multivariate logistic regression analysis showed that alprostadil treatment was a favorable protective factor for moderate-risk and high-risk CIN patients (OR = 0.343, 95% CI: 0.124-0.951, P = .040). Alprostadil can be used as a preventive treatment for moderate- and high-risk CIN patients classified by the MRS. The reduction of CIN by alprostadil may be related to an anti-inflammatory effect.

Background: The aim of this research was to use the Mehran risk score to classify elderly diabetics with coronary heart disease to assess the preventive effect of trimetazidine on contrast-induced nephropathy (CIN) after percutaneous coronary intervention (PCI) in different risk population. Methods: An uncompromised of 760 elderly diabetics that went through PCI were included in this research. The patients were first divided into three groups in the light of MRS: low-risk, moderate-risk, and high-risk group, then randomized into trimetazidine group and the control group respectively. The first endpoint was the amount of CIN, which is described as a rise in serum creatinine levels by ⩾44.2 μmol/L or ⩾25% ratio within 48 or 72 hours after medication. Second endpoint included differences in creatinine clearance rate (CrCl), blood urea nitrogen (BUN), serum creatinine (Scr), cystatin-C (Cys-C), and the incidence of major adverse events after administration. Results: In the three groups, the incidence of CIN in trimetazidine and control group was 5.0% versus 4.9%(χ2 = 0.005, p > 0.05), 8.0% versus 18.0% (χ2 = 7.685, p < 0.05), 10.4% versus 27.1% (χ2 = 4.376, p < 0.05), respectively. The multivariable logistic regression result demonstrated that trimetazidine intervention was a profitable element of CIN in moderate and high-risk groups (OR = 0.294, 95% CI 0.094-0.920, p = 0.035). Conclusion: Our study confirmed that trimetazidine can be considered for preventive treatment of CIN occurrence in elderly diabetics with moderate and high-risk population, while there is no obvious advantage compared with hydration therapy in low-risk patients.

Objective: To explore the nursing effects of medical treatment combination on patients at high risk of developing pressure ulcers at rural homes. Methods: From January 2018 to December 2018, 86 patients at high risk of developing pressure ulcers at rural homes who were in Wenjiang Hospital were randomly divided into a control group and an observed group. The control group received the routine publicity and education and visiting, while the observed group adopted the cooperative nursing model of medical treatment combination. Then compare the healing status and nursing satisfaction of the two groups. Results: Of the 43 patients with pressure ulcers in the observed group, 18 had been in the second stage of pressure ulcers and 16 had been healed; 14 had been in the third stage of pressure ulcers and 8 had been healed; 11 had been in the fourth stage of pressure ulcers and 5 had been healed. The nursing satisfaction of the patients in the observed group was 95.35%. Conclusion: Conducting medical treatment combination on patients at high risk of developing pressure ulcers at rural homes can effectively improve the patients’ and their families’ awareness of pressure ulcer nursing, standardize their nursing behaviour, help recover the patients themselves and promote the patients’ satisfaction.

OBJECTIVE: To evaluate the risk for pressure ulcers in elderly in their homes, after a period of hospitalization. METHODS: A longitudinal prospective study conducted in the homes of 40 elderly identified with risk for pressure ulcer (PU) at hospital discharge, using the Braden Scale. The monitoring was conducted over four home visits, in the period between June and August of 2010, in Fortaleza (CE) and its metropolitan region. RESULTS: The majority of the elderly were female (65%) with a medical diagnosis of cerebral vascular accident (55%). In the first and second visits, 55% and 40% of the elderly, respectively, presented high risk for PU, and the incidence of PU was 22.5%. The association of the risk scores presented significant association in the first three visits. CONCLUSION: The risk for PU development was higher in the first two weeks, after hospital discharge, but diminished for the remainder of the visits.

AbstractObjective:To evaluate the incidence of pharyngocutaneous fistula after pharyngolaryngectomy with and without a Montgomery salivary stent.Design:Retrospective analysis of patients with factors that predispose to the development of pharyngocutaneous fistula (i.e. disease extending to the supraglottic region, base of the tongue or pyriform sinuses, and/or radiochemotherapy).Subjects:Between 2002 and 2008, 85 pharyngolaryngectomies were performed in our clinic. Of these patients, 31 were at increased risk of fistula development, of whom 45 per cent developed fistulas post-operatively. This subgroup of 31 patients was compared with a second subgroup of 22 patients at high risk of fistula development, treated between 2009 and 2011 with pharyngolaryngectomy and with a Montgomery salivary stent placed in advance during closure of the neopharynx.Results:Statistical analysis showed a significant reduction in the rate of fistula development, from 45 to 9 per cent (p < 0.01), with application of the salivary stent.Conclusion:These data confirm the preventive effect of a salivary stent placed during pharyngolaryngectomy, for patients at high risk of fistula development.
