Technology is expanding at an unprecedented rate. Because patients value the speed and convenience of the internet, there is an increasing demand for telemedicine.  Practitioners must therefore adapt their clinical skills to evolving online technologies. This paper presents a series of three case studies in which a physical therapist first assessed and treated musculoskeletal disorders via a live, secure video. The basis of the mechanical assessment was observation of movement rather than palpation. In each case, the virtual mechanical assessment identified a specific sub-classification with a directional preference.  All patients reported improvements in symptoms and function in less than four visits and all maintained a reduction in symptoms after three months. Given the “hands-off” role of the evaluator, this approach can become an effective tool in the evolving healthcare platform of telerehabilitation.

Abstract Objective Virtual reality (VR) technologies are increasingly used in physical rehabilitation; however, it is unclear how VR interventions are being delivered, and, in particular, the role of the therapist remains unknown. The purpose of this study was to systematically evaluate how commercially available VR technologies are being implemented in gait, posture, and balance rehabilitation, including justification, content, procedures, and dosage of the intervention and details of the therapist role. Methods Five databases were searched between 2008 and 2018. Supervised interventional trials with >10 adult participants using commercially available VR technologies to address mobility limitations were independently selected by 2 authors. One author extracted reported intervention characteristics into a predesigned table and assessed methodological quality, which was independently verified by a second author. A total of 29 studies were included. Results Generally, minimal clinical reasoning was provided to justify technology or activity selection, with recreational systems and games used most commonly (n = 25). All but 1 study used a single interventional technology. When explicitly described, the intervention was delivered by a physical therapist (n = 14), a therapist assistant (n = 2), both (n = 1), or an occupational therapist (n = 1). Most studies reported supervision (n = 12) and safeguarding (n = 8) as key therapist roles, with detail of therapist feedback less frequently reported (n = 4). Therapist involvement in program selection, tailoring, and progression was poorly described. Conclusion Intervention protocols of VR rehabilitation studies are incompletely described and generally lack detail on clinical rationale for technology and activity selection and on the therapist role in intervention design and delivery, hindering replication and translation of research into clinical practice. Future studies utilizing commercially available VR technologies should report all aspects of intervention design and delivery and consider protocols that allow therapists to exercise clinical autonomy in intervention delivery. Impact Statement The findings of this systematic review have highlighted that VR rehabilitation interventions targeting gait, posture, and balance are primarily delivered by physical therapists, whose most reported role was supervision and safeguarding. There was an absence of detail regarding complex clinical skills, such as tailoring of the intervention and reasoning for the choice of technology and activity. This uncertainty around the role of the therapist as an active ingredient in VR-based rehabilitation hinders the development of implementation guidelines. To inform the optimal involvement of therapists in VR rehabilitation, it is essential that future studies report on all aspects of VR intervention design and delivery.

Devika Hovell raises deeply significant questions about the role of due process in the legitimacy of the United Nations Security Council (UNSC). Hovell gives us a fine-grained analysis of what exactly makes due process so compelling; in her approach, the reasons why it is compelling will vary in different contexts, depending upon the particular value and function it serves. In particular, she discusses three ways of articulating the values underlying due process, and the models of due process that would follow from each. She then discusses how her analysis would play out in two situations: The Council’s use of asset freezes, and the role of the UN in the cholera epidemic in Haiti. In her case studies, she looks at situations where due process has been insufficient, and discusses some of the UN’s attempts to remedy this, and the organizational difficulties in doing so.

This paper presents a theoretical framework that explores visual meaning in the design and use of interior space. It is comprised of three main parts. The first outlines the framework and draws on several key theories. The second introduces three very different constructs as case studies that in#uence (or are a product of) spatial quality, namely: buildings, faces, and songs of alienation. The third part is a discussion about how each of these three constructs are linked to each other as well as to the idea of interiority. While architectural forms are containers of meaning, the way in which interior space is curated is driven by deeper meaning–one that transcends form and function because people ultimately produce the meaning. And because each person is different, the conditions of interiority (in this case, the meaning that resides within each person) drives the meaning of external constructs that act as enclosures of meaning (buildings and their interiors). The findings are that the mind and body can be projected beyond the facade and into the spaces contained in the buildings we occupy. The role of technology is also important because changes in technology help mediate the process of linking the meaning inside with the meaning out there.

While repressions are seen to be a backbone of authoritarian rule, there is a lack of case studies of repressions and repressive policies in different kinds of authoritarian regimes and their interaction with other mechanisms of authoritarian sustainability. As Russia has demonstrated a transition from ‘soft’ electoral authoritarianism to its more ‘hard’ version during Putin’s third term in office, the role of repressions has increased. What are their scope and functions in Russia during this reverse transition? This article offers an analysis of the causes, types and mechanisms of repressions, and presents various ways of measuring their scale as well as the sources and means of their legitimation within the framework of an electoral regime. It shows that the regime prefers to demonstrate its high repressiveness—its willingness and propensity to repress—but in a limited number of cases; it also describes the role of repressive populism, namely presenting repressions as a necessary response to multiplying threats, as well as the scope and function of counter-elite repressions. The latter are seen as no less important than political repressions in the regime’s reverse transition, and as the main leverage of redistribution of power and institutional rearrangement in its course.

This chapter examines the role of the zombie within TV horror both in terms of a long established tradition of monster-of-the-week through to the increasingly prevalent place that the zombie plays within contemporary serialised television. This chapter challenges the dismissal of television as an appropriate space for horror and the political allegory often associated with Romero’s zombie films, by presenting a series of case studies in which the TV zombie serves such as narrative and thematic purpose. In particular it considers how the serialized nature of television, exemplified by the soap opera format, is well suited to the zombie narrative in which closure is traditionally denied. It also serves to structure the nature and function of allegory within the televisual zombie format. Case studies include: Buffy the Vampire Slayer, Supernatural, The Walking Dead and In the Flesh.

This article examines the role and function of author attributions in multi-text manuscripts containing Dutch, English, French or German short verse narratives. The findings represent one strand of the investigations undertaken by the cross-European project ‘The Dynamics of the Medieval Manuscript’, which analysed the dissemination of short verse narratives and the principles of organisation underlying the compilation of text collections. Whilst short verse narratives are more commonly disseminated anonymously, there are manuscripts in which authorship is repeatedly attributed to a text or corpus. Through six case studies, this article explores medieval concepts of authorship and how they relate to constructions of authority, whether regarding an empirical figure or a literary construction. In addition, it looks at how authorship plays a role in manuscript compilation, and at the effects of attributions (by author and/or compiler) on reception. The case studies include manuscripts from the thirteenth to fifteenth centuries, produced in a range of social and cultural contexts, and featuring some of the most important European authors of short verse narratives: Rutebeuf, Baudouin de Condé, Der Stricker, Konrad von Würzburg, Willem of Hildegaersberch, and Geoffrey Chaucer. The preliminary findings contribute to our understanding of author attributions in text collections from across northern Europe and point towards future lines of enquiry into the role of authorship in medieval textual dissemination.

The size and organization of collagen fibrils in the extracellular matrix is an important determinant of tissue structure and function. The synthesis and deposition of collagen involves multiple steps which begin within the cell and continue in the extracellular space. High-voltage electron microscopic studies of the chick embryo cornea and tendon suggested that the extracellular space is compartmentalized by the fibroblasts for the regulation of collagen fibril, bundle, and tissue specific macroaggregate formation. The purpose of this study is to gather direct evidence regarding the association of the fibroblast cell surface with newly formed collagen fibrils, and to define the role of the fibroblast in the control and the precise positioning of collagen fibrils, bundles, and macroaggregates during chick tendon development.

It is currently believed that ciliary beating results from microtubule sliding which is restricted in regions to cause bending. Cilia beat can be modified to bring about changes in beat frequency, cessation of beat and reversal in beat direction. In ciliated protozoans these modifications which determine swimming behavior have been shown to be related to intracellular (intraciliary) Ca2+ concentrations. The Ca2+ levels are in turn governed by the surface ciliary membrane which exhibits increased Ca2+ conductance (permeability) in response to depolarization. Mutants with altered behaviors have been isolated. Pawn mutants fail to exhibit reversal of the effective stroke of ciliary beat and therefore cannot swim backward. They lack the increased inward Ca2+ current in response to depolarizing stimuli. Both normal and pawn Paramecium made leaky to Ca2+ by Triton extrac¬tion of the surface membrane exhibit backward swimming only in reactivating solutions containing greater than IO-6 M Ca2+ Thus in pawns the ciliary reversal mechanism itself is left operational and only the control mechanism at the membrane is affected. The topographic location of voltage-dependent Ca2+ channels has been identified as a component of the ciliary mem¬brane since the inward Ca2+ conductance response is eliminated by deciliation and the return of the response occurs during cilia regeneration. Since the ciliary membrane has been impli¬cated in the control of Ca2+ levels in the cilium and therefore is the site of at least one kind of control of microtubule sliding, we have focused our attention on understanding the structure and function of the membrane.

Abstract The Popeye domain-containing gene family encodes a novel class of cAMP effector proteins in striated muscle tissue. In this short review, we first introduce the protein family and discuss their structure and function with an emphasis on their role in cyclic AMP signalling. Another focus of this review is the recently discovered role of POPDC genes as striated muscle disease genes, which have been associated with cardiac arrhythmia and muscular dystrophy. The pathological phenotypes observed in patients will be compared with phenotypes present in null and knockin mutations in zebrafish and mouse. A number of protein–protein interaction partners have been discovered and the potential role of POPDC proteins to control the subcellular localization and function of these interacting proteins will be discussed. Finally, we outline several areas, where research is urgently needed.
