The purpose of the study is to use the example of the Arkhangelsk region to identify conditions affecting the organization of medical care for victims of road accidents on the federal highway in a region with a low population density, and to determine the main directions of its improvement. Materials and research methods. Using the method of comparative analysis, the following review was carried out: review of the authors’ studies, the subject of which was the features of road traffic injuries on the federal highway M-8 «Kholmogory» within the Arkhangelsk region; review of domestic and foreign scientific research, review of regulatory documents on the provision of medical assistance to victims of road accidents. Scientific literature search was performed in eLibrary, PubMed, Scopus systems by keywords. Scientific articles published in 1990–2020 were selected for analysis. Using the method of an organizational experiment, a scientific substantiation of a set of measures to improve the efficiency of the provision of medical care in prehospital and hospital periods to victims of road traffic accidents at federal highway in regions of the Russian Federation with a low population density was carried out. Applying the method of system analysis, on the basis of the results of this study, conceptual provisions for a systemic register of health consequences of road accidents in the Russian Federation have been developed and scientifically substantiated. Research results and their analysis. The article describes road traffic injuries on the federal highway M-8 «Kholmogory» in the Arkhangelsk region. The factors that determine the effectiveness of the provision of medical care in prehospital and hospital periods to victims of road traffic accidents are considered. The ways of improving the provision of medical care to victims of road traffic accidents at federal highway in a region with a low population density are formulated and substantiated.

As a result of road accidents in the Russian Federation, up to 30 thousand people are killed every year, which causes significant demographic and socio-economic damage to the state. The World Health Organization considers road traffic injuries as one of the global problems associated with 1.25 million deaths. In many countries, the leading direction of development of medical care for victims of road traffic accidents is the creation of trauma systems. The main volume of medical care for victims of road traffic accidents and those with severe injuries under other circumstances is provided by trauma centers of the 2nd and 1st levels. The implementation of the federal targeted programs «Improving road safety in 2006-2012» allowed the creation of a network of trauma centers in a number of subjects of the Russian Federation, whose work reduced mortality and disability among victims with severe injuries, but the problem cannot be considered solved because there are no national database of the victims of road accidents (register), system for assessing the severity of injuries and recording long-term results of non-fatal injuries, which excludes the possibility to compare the clinical effectiveness of the Russian trauma centers among themselves and with foreign counterparts.

The article analyzes the informativeness of the main indicators of accidents, possible misconceptions about improving road safety while reducing the number of accidents, as well as the need to take into account the degree of road safety when choosing measures aimed at improving road safety, assessing such measures in combination with the degree of road network safety.

Relevance. The paper is devoted to problems of ensuring road traffic safety in Russia, which is the public task of great importance involving the implementation of the policy for protecting people's health, life, and property.Intention. To look for ways of optimizing and implementing the measures aimed at preventing the technogenic emergencies.Methodology. To analyze the indicators of road traffic accidents in the Russian Federation over five years as well as the federal laws in the field of road traffic safety.Results and Discussion. Risk factors of traffic accidents include: alcohol intoxication, speed limit exceeded, overtaking in the wrong place, driver talking, smoking while driving, driver fatigue. In the Russian Federation, there is a persistent downward trend in the number of accidents. In 2019, 164,358 traffic accidents (-2.2 %; all comparisons vs 2018) occurred, 16,981 (-6.8 %) persons died, 210,877 (-1.9 %) persons were injured. According to the Ministry of Internal Affairs, the number of accidents due violation of the Road Rules by drivers was 146,688 (-1 %); 14,420 (-5.7 %) persons died and 195,037 (-0.8 %) persons were injured. In 2019, road traffic accidents caused by drunk drivers tended to decrease (12,040; -3.5 %); 11,510 (-4 %) persons died and 160,725 (-0.4 %) persons were injured.Conclusion. Drivers' health is directly related to road traffic safety; health promotion would contribute to decreasing the number of road traffic accidents and traffic-related injuries.

Road traffic accidents cause enormous material and moral damage both to the society as a whole and to individual citizens. According to the data, the Russian Federation is among countries with highest road traffic mortality rates within European Region of the World Health Organization.The objective of the study was to attempt to identify possible causes of high mortality rates from traffic accidents on the roads of the Russian Federation and to reveal ways to reduce those rates.To that purpose a survey of citizens of the Russian Federation who are driver’s license owners and who are experienced drivers was conducted in different regions of the country. The results identified low medical literacy in terms of provision of first aid to the injured persons as one of the causes.Other factors, potentially capable to reduce the number of fatalities in road traffic accidents, once their performance indicators improve, have also been considered. Those indicators comprise time of arrival of ambulance crews at the site of an accident with enough medical equipment and medicines, interaction of emergency services, growth of efficiency of training in providing primary medical care at driving schools.

Introduction: The increasing number of vehicles and the proliferation of two-wheeled vehicles accentuate the frequency of fractures in our country. The aim of this study is to describe the epidemiology of fractures in Cotonou. Materials and Methods: This prospective two-year study focused on all patients admitted to the emergency department of the National Teaching Hospital of Cotonou for a fracture injury. Results: 1794 fractures were collected in 1432 patients. Fractures accounted for 24.9% of surgical emergencies. The average age was 33.7 years (29 days - 90 years). The dominance was male with a sex ratio of 3.62. Etiological circumstances were dominated by road traffic accidents (75.8%), The etiological circumstances were dominated by road accidents (75.8%), motorcyclists being the most affected (48.5%). The admission average was 27.33 hours (15 minutes - 29 days). Fractures preferentially sat on the lower limbs (64.4%). In descending order of frequency, were distinguished the fractures of leg (32.2%), of the femur (19.8%) and of the forearm (14.8%). Conclusion: fractures are common. The etiologies are dominated by traffic accident. The lesions predominate in the lower limb. The establishment of good road safety policy (improving track conditions, extension of the highway code ...) would reduce their frequency.

The research object is social relations connected with the admission of novice drivers to traffic on roads and the issue of driver’s licences. The research subject is the set of normative acts of the Russian Federation regulating the admission of drivers to traffic on roads, and the statistical data of the State Road Safety Unit on road accidents with novice drivers whose driving experience is up to 2 years. The authors analyse road accidents with novice drivers, identify the main reasons of accidents and offer the solutions to these problems. The scientific novelty of the research consists in the fact that the analysis of traffic accidents helped to outline the problems that should be solved within the shortest possible time in order to reduce the number of road accidents caused by novice drivers. In this context, the authors suggest introducing changes into the legislation of the Russian Federation which would allow for the declaration of driver’s licences issued in other states invalid, as well as the exchange of non-Russian driver’s licences for Russian ones only after adequate training and passing the exams. Besides, the authors suggest introducing the possibility for juveniles who passed the exams to drive a vehicle before they arrive the age of 18 with particular restrictions and accompanied by an experienced driver.
