PURPOSE. To study the epidemiological features of glaucoma prevalence among the population of the Russian Federation and to assess the organization of primary specialized medical care for patients with this pathology.MATERIALS AND METHODS. The subject of this study was the population of the Russian Federation diagnosed with glaucoma. In order to assess the epidemiological features of glaucoma prevalence and to assess the organization of primary specialized medical care for patients with this pathology, according to the federal statistical reporting forms (Forms No. 12, No. 30), the following indicators were calculated: overall and primary incidence of glaucoma in the general population; proportion of patients with glaucoma who are registered with ophthalmologists; availability of ophthalmologists (individuals) per 100 thousand population; proportion of patients with glaucoma diagnosis established during preventive examinations; total number of registered patients with blindness and visual impairment. Statistical processing of the material involved methods of descriptive statistics; calculation of average and relative values with a preliminary assessment of the distribution of indicators for normality. Spearman rank correlation method was used to determine the correlation between quantitative variables.RESULTS. The studied data was used to calculate the following average values: total and primary glaucoma incidence among the population of the Russian Federation — 78.2 (55.8–105.3) cases per 100 thousand population; the proportion of patients with dispensary registered glaucoma — 80% (74.4–85.8%). The performed correlation analysis established a moderate direct statistically significant relationship between the indicators of total and primary glaucoma incidence among the population (rank correlation coefficient: r=0.72 at p<0.05). In its turn, the correlation between the indicators of glaucoma incidence among the population and the availability of ophthalmologists for the population has not been established (r=0.14, p>0.05). It was found that the indicators of dispensary observation of glaucoma patients do not depend on the indicators of total glaucoma incidence (r=0.16) and availability of ophthalmologists for the population (r=0.13). There is a weak inverse correlation between the proportion of glaucoma cases detected during preventive examinations and the indicator of primary glaucoma incidence among the population (r=-0.28; p<0.05). Also, the correlation analysis did not establish a relationship between the indicator of total glaucoma incidence among the population and the total number of registered patients with blindness and low vision (r=-0.008).CONCLUSION. The incidence rate of glaucoma in specific administrative territories should be assessed by comparison with the average values among the whole population, which are represented by the median and quarterly ranges. The incidence rate of glaucoma in the general population is not statistically related to the level of availability of ophthalmologists for the population. The proportion of follow-up observation of glaucoma patients does not depend on the availability of ophthalmologists and the level of glaucoma incidence. The incidence of blindness and low vision does not statistically correlate with the incidence of glaucoma in the population.

In recent years, there have been significant positive changes in the organization of ophthalmological care for children, formed the legal basis for the provision of medical care to children with ophthalmopathology, increased the availability and quality of medical care, detectability of ophthalmopathology (6%), increased the results of treatment, which reduced the level of primary disability per 10 thousand of the child population in the Russian Federation has declined from 1.4 in 2004. to 0.9 in 2016. Early treatment has helped reduce blindness and low vision in 2.6 times (from 58.4 in 2009 up to 22.7 in 2016). The rapid development of pediatric ophthalmology is due to the introduction of new highly informative objective methods of research, high-tech surgical and laser treatment in the leading ophthalmic centers of Russia: Federal clinical recommendations on diagnosis, monitoring and treatment of children with retinopathy of prematurity (active phase), myopia, congenital cataracts, retinoblastoma, concomitant strabismus, congenital glaucoma, uveitis associated with juvenile idiopathic arthritis, congenital pathology of lacrimal pathways in newborns and children of the first year of life have been developed and put into practice.

Article is devoted to the analysis of the Order of the Russian Ministry of Health of 19.03.2020 № 198n (an edition of 29.04.2020) “About a temporary order of the organization of work of the medical organizations for implementation of measures for prevention and reduction of risk of spread of a new koronavirusny infection of COVID19” regarding formation of the temporary staff list of the division of the medical organization providing medical care to citizens with a koronavirusny infection of COVID-19 and also the requirement with the staff of this division. The criteria recommended by the Ministry of Health of the Russian Federation for determining the number of rates of medical workers in these divisions, requirements for medical workers of the division have been considered, a practical example of calculation of rates has been given.

Background. An effective system for management of pediatric dermatological care in Russia requires taking into account the peculiarities of regional morbidity. According to modern literature, this issue is not sufficiently developed. Our aim was to study the characteristics of variability in the incidence of skin and subcutaneous tissue diseases of children aged 0–14 years in the Russian Federation. Methods. The data of the state medical statistics on 83 territorial entities of the Russian Federation for 2009–2015 was used. The analysis implemented the risk aggregation method. The degree of variability in regional morbidity was assessed by the range of variation and the coefficient of variation. Region rankings were carried out by the annual incidence. The Spearman’s rank correlation coefficient was used to study the stability of regions belonging to a certain ranked place. Results. In 2009, the incidence of skin and subcutaneous tissue diseases of children aged 0–14 years in the Russian Federation had a pronounced variability: the range of variation was 7.7 times, the coefficient of variation was 28%. The annual region rankings were carried out. In 2010–2015, the annual variability indices remained high. The coefficients of rank correlation when distributing regions by morbidity were in the range of 0.75–0.94 compared to 2009. Quintiles of regions with different levels of pediatric dermatological morbidity were singled out. Conclusion. The incidence of skin and subcutaneous tissue diseases in children aged 0–14 years in the territorial entities of the Russian Federation during 2009–2015 had a pronounced variability. Regions had a stable preference for a certain ranking place. The division of territorial entities of the Russian Federation into groups of pediatric dermatological morbidity are presented.

In the presented article there was executed a retrospective analysis of legionellosis in the Russian Federation for the period of its official registration. Currently, the prevalence rate of legionellosis in Russia is 0.01 per 100 thousand of the population, while in Europe and the United States this figure is 1.1-1.2 per 100 thousand of the population. Probably in the Russian Federation there is a problem of diagnosing of the disease and its official registration.

Objective. To analyze legislation regulating medical care for children with rheumatic diseases in the Russian Federation. Materials and methods. We analyzed regulatory documents of the Ministry of Health of Russia, including the rules of medical care for children with rheumatic diseases, the resolution regulating the distribution of medicines at a discount or for free, and regional legal acts regulating routing of children with rheumatic diseases. Results. The legislation regulating medical care for children with rheumatic diseases is imperfect and needs to be updated. We identified shortcomings in the system regulating preferential provision of certain groups of patients with medicines. The results of our analysis can be used as a rationale for changes and amendments to existing regulatory legal acts and for the development of new regulatory documents. Conclusion. This analysis of legal documents allows us to suggest amendments to the existing legal framework in order to reduce tensions (including legal tensions) associated with distribution of preferential medicines, reconsider establishment of disability in patients with juvenile idiopathic arthritis, expand opportunities to prescribe biologicals for non-disabled patients with systemic scleroderma, juvenile dermatomyositis, and systemic vasculitis in regions. We justified detailed description of regional legal documents regulating distribution of preferential medicines among patients with rheumatic diseases. According to the State program of the Russian Federation «Health care development» till 2020 approved by the decree of the Russian Federation government dated 24.12.2012 N 2511-p, the principle of the cluster approach became the one of the directions of informatization of healthcare. Subsystem perinatal cluster is the continuous monitoring of pregnant women. Its main advantage is the formation of a unified information space of the hospitals in the region that allows you to reliably group out pregnant females according to the risk of complications of pregnancy and childbirth, to determine the routing for all the patients. The goal of the system is a significant improvement of the assistance within a cluster (group), the formation of a standardized approach to the medical care, management of emergency situations in the occurrence of life-threatening conditions, the reduction of maternal and perinatal mortality rate. Computerization of medical care in obstetrics was implemented in the automated system «Program monitoring for pregnant females».
