The colostomized person's perceptions regarding sexuality and sexual activity is a major component of quality of life and nursing care, however due to several limitations, the questionnaire only approach, although validated for such applications, has not enough resolution, due to its qualitative approach, to capture more delicate aspects of the impact of an ostomy onto a otherwise healthy patient. In order to correct such bias, a exploratory-descriptive study of a qualitative approach based on Bardain’s content analysis was conducted.  80 colostomized participants were recruited from Stomatology Outpatient Clinics in the Hospital de Base, Regional Hospital of Asa Norte and the Regional Hospital of Gama, Distrito Federal, Brazil. Data was collected through a clinical and sociodemographic questionnaire, as well as an individual interview. The results are presented in five categories: self-concept, leakage issues, odor and gases, stigma, sexual activity and companionship. The analysis suggests that the fulfillment of nursing care ideals, through clear and objective communication, results in the colostomized person's care and can favor in the acceptance of their new way of living. The exchange of knowledge and practices between the colostomized patient and the medical care support crew, in special the nursing care, allows the establishment of strategies that contribute gradually to the full health care, promoting the improvement of the QoL of both the colostomized person as well his/her partner. It is concluded that health services should provide care process in health and nursing, with the purpose of guaranteeing means for the planning of a holistic implementation of health care. This planning should include the psychological support and the educational process that develops one's ability for self-care, which may interfere in the role of sexuality and sexual activity, in addition to the physiological, psychological and social issues.

Introduction: Current vocational secondary education (VSE) in Russia is characterized by a focus on practical training (as part of dual training) aimed at improving professional competencies of graduates. It also demonstrates negative trends in adolescent health, which requires optimization and development of specific algorithms of medical support for students aged 14–22 years and older appropriate to their academic and practical workload. Objective: To assess adaptation of college students to learning conditions, depending on their health status, and to substantiate ways of improving their health care. Material and methods: We retrieved and analyzed information from the database of multicenter studies conducted within the unified program of the National Medical Research Center for Children’s Health of the Russian Ministry of Health. Our sample included 197 first- and second-year college boys from the cities of Omsk and Moscow studying to become welders and auto mechanics. The quality of life, health and well-being indicators were evaluated according to the International Medical Outcomes Study 36-item short-form (MOS SF-36). The students were divided into subgroups based on their health status. The statistical processing complied with modern requirements and criteria of evidence-based medicine. Results: We established that vocational schools admitted a significant number of applicants with the above specialty preferences suffering from chronic diseases (21.1–26 %). Results of the questionnaire-based survey of future welders and car mechanics revealed difficulties in adaptation to the educational process of the students with chronic disorders expressed by lower quality of life indicators, frequent health complaints and feelings of fatigue, especially in second-year students who experience an increasing academic and practical workload. Conclusions: Students with chronic diseases mastering professions with hazardous working conditions represent population at risk, require health monitoring, determination of professional suitability, and control over the working conditions in industrial practice. The importance of protecting health of future professionals is determined by implementation of practice-based training in 42 % of vocational schools. Models of school medicine proposed to optimize health care in comprehensive schools can be adapted to conditions of secondary vocational facilities. Foreign studies have proved feasibility of an early onset of work-related diseases in certain occupations, even before completion of training, thus necessitating vocational guidance, professional medical advice, and development of an effective system of medical support for adolescents and students.

Introduction: Breast cancer is the transformation of healthy breast cells into malignant cells. The disease is characterized by metastases that can range from limited bone metastases to wide-spread and life-threatening metastases. The main goal of caring for these patients is to successfully control the symptoms of the disease, the side effects of the tumor while preserving the quality of life and surviving as long as possible. Objective was to show the specifics of interventions in the health care of a patient with breast cancer with metastatic changes in the endocranium Methods: We used a descriptive method and a conservative direct method: case study of a patient with breast cancer and metastatic changes in the brain according to the method of the Nursing Care Process (NCP). Discussion: Modern nursing care implies patient care according to a scientifically based method, universal in the nursing practice NCP, according to which care is focused on the patient/family, re-specting their preferences. NCP includes: continuous assessment of the patient's condition and defining problems and care needs, setting goals and selecting optimal care strategies. The most COM-mon collaborative problems for our patient were: pain, fatigue, anemia, insomnia. The problems that the nurses solved through independent interventions were: deficit in self-care, prevention of falls and injuries, lack of knowledge regarding the radiation pro-cedure, risk of infection, and the possibility of excess fluid volume. Conclusion: Patients with advanced breast cancer and their families have complex needs, the failure of which can result in impaired quality of life. An individual approach is needed in as-sessing the patient's condition, identifying needs, and planning interventions based on scientific evidence, available health re-sources, and preferences of patient/family. Patient care according to the NCP method promises a more comprehensive approach, improved patient experience as well as improved medical out-comes.

Abstract Background Knowledge of health system indicators are essential to the living situation of persons with disabilities. Assessing and collating the performance of health systems is important in order to identify areas of improvement. The main aim of this study is to assess the relationship between health system indicators and self-reported quality of life (QoL) among persons with spinal cord injury who accessed either public or private health care services in South Africa. Methods A cross-sectional survey was implemented to analyse and compare health system indicators and self-reported QoL for 200 persons over the age of 18 with either a traumatic or non-traumatic spinal cord injury. The survey further compared the impact of public or private health care access on these indices. Health system indicators include: access to disability pension, vocational rehabilitation, health and nursing care, public space, medications, assistive devices and transportation. Self-reported quality of life indicators include: satisfaction with health; ability to perform activities of daily living; yourself; personal relationships; and living conditions. A total score of Quality of life was calculated using these 5 domains. Results The highest scores for health indicators were access to a disability pension (76%) and access to required health care (72%). Indicators that negatively affected respondents’ lives were access to public spaces (70%), transport (56%), medications (56%) and nursing care (53%) which were reported to have made participants’ lives significantly harder. Participants who accessed private health care reported a higher QoL score in comparison with those in the public health system who reported more problems. There was a negative correlation between QoL and access to a disability pension and a positive correlation between QoL and access to public spaces, medication, transport and nursing care. Conclusion Performance of health system indicators influence living situation, and different healthcare insurances play a significant role in health system performance and self-reported QoL among persons with SCI in South Africa. Additionally, persons with SCI who have the ability to overcome the challenges encountered due to poor performing health systems report better quality of life.

Background Intervention with a mobile Health (mHealth) app can improve the efficacy of early detection of oral cancer and the outcomes for patients taking oral anticancer medications. The quality of life of oral cancer patients is significantly reduced within three months after surgery; also, their needs for nursing care and health information increase, mainly due to side effects and associated psychological problems. Objective This study aimed to evaluate changes in the care needs and quality of life of patients with oral cancer after receiving the intervention of a newly developed mHealth app. Methods After surgery, oral cancer patients were divided into an experimental group (n=50) who received the mHealth app intervention and a control group (n=50) who received routine health care and instruction. After 3 months of intervention, survey questionnaires were used to assess the patients’ quality of life, nursing care needs, and acceptance of the mHealth app. Results The physiological care needs were significantly decreased in the experimental group compared with the control group (P<.05). Although the differences were not statistically significant, the psychological needs, communication needs, and care support needs all improved after the mHealth app intervention. The overall improvement in quality of life was higher in the experimental group than in the control group (–7.24 vs –4.36). In terms of intention to use, perceived usefulness, and perceived ease of use, the acceptability scores of the mHealth app were significantly increased after 3 months of intervention (P<.05). Conclusions Compared with routine health care and instruction, for patients after surgery, the education/information intervention using the mHealth app significantly reduced their nursing care needs, improved their quality of life, and increased their acceptance of using an mHealth app on a mobile device. These findings can provide a theoretical basis for future health care app design and improvement. This study suggests that an mHealth app should be incorporated into the routine care of oral cancer patients to provide medical information quickly and improve their self-management abilities, thereby reducing the patients’ need for physiological care and improving their quality of life. Background / objectiveEvaluations of the health care quality of patients after abdominal operations are significant for the improvement of health care quality in all hospitals. However, there are a lot of discussions how to evaluate the patient’s opinion and attitudes. The aim of the present study was to analyze patients’ evaluations of the quality of abdominal surgical nursing care.MethodsA multicenter, cross-sectional, analytical study was performed. The study was conducted in 11 abdominal surgical wards at Lithuanian hospitals. The data were collected in June 2007 and January 2008. Patients (n = 1208) after abdominal operations participated in the study. A GNCS-P with the response rates of 74 % was used. The data were analyzed statistically.ResultsThe co-operation with significant others and the progress of the nursing and health care process were evaluated lowest by patients. The patient age and satisfaction were the factors related to the quality of nursing care.ConclusionsThe abdominal surgical nursing and health care is a specific surgical area in which different patients are involved; however, all of them expect the good quality of health care before, during, and after abdominal surgery. The process of nursing care in abdominal surgery is specific because of the limited time of patient hospitalization, usually multiple patients’ diagnosis, as well as multiple contacts and relationships with many different staff. Surgical patient participation in the process of health care should be based on the effective relationship and co-operation among patients, medical specialists, and significant others, which is necessary and imperative for increasing the quality of abdominal surgical nursing care.Key words: health and the quality of nursing care, surgical health care, abdominal surgery, patient evaluations.
