STROKE IMPACT ON THE COGNITIVE FUNCTIONS OF MOROCCAN NEURO-LESIONED PATIENTS IN THE GHARB REGION

Stroke (ischemic and hemorrhagic stroke) is a sudden-onset neurological deficit resulting from focal vascular lesions. This is due to a clot-induced obstruction of a vessel (ischemic stroke) or a rupture of a vessel causing haemorrhage (hemorrhagic stroke). The management of neuro-injured patients (AVC) is a major public health problem. The principal aim of this study is to evaluate the short and long term neuropsychological sequences following a neurological accident of neuro-injured patients hospitalized at the Kenitra Provincial Center (Morocco) in comparison with the control group. We tested 34 stroke patients, with an average age sample of 59.12 years, for a standard deviation of 14.35 with extremes between 32 and 82 years. Of these patients, 20 were female (58.82%) and 14 male (41.18%). The sex ratio is 0.7 in favour of the female sex. Both the neuro-lesioned patients and the control group benefited from neuropsychological tests. In the neurocognitive evaluation we used three neuropsychological tests: (a) The bell test or non-verbal bell dam test allows for a selective, visuospatial and strategic attentional evaluation; (b) Raven's test focuses on the nonverbal neuropsychological intelligence where the subject is led to analyze and solve each test problem based on inductive reasoning; (c) The digit memory test is a test to evaluate the short-term verbal memory and working memory capabilities of stroke patients. Our results showed through the various neurocognitive tests that our stroke patients obtained lowers score, compared to the control group (p <0.05). Raven Standard Progress Matrix Test Scores (SPMR):(Mean-Patients = 32.49, SD = 7.43 < Mean-Controls = 42.01, SD = 3.98). Digit Memory Test scores: Forward digit span (Mean-Patients = 2.21, SD = 0.5 <Mean-Controls = 2.65, SD = 0.49). Backward digit span (Mean-Patients = 1.74; SD = 0.6 <Mean-Controls = 2.41, SD = 0.5). Bell test (Mean-Patients = 24.35; SD = 2.62 < Mean-Controls = 30.18; SD= 2.52). The patient quality of life is consequently highly affected. Rehabilitation was effective and is very important because it improves cognitive functions such as nonverbal skills, visuospatial and strategic attention, along with the digit memory equally improving patient quality of life.

Background: stroke is a disorder of nervous system function that occurs suddenly and is caused by brain bleeding disorders that can affect the quality of life physical dimensions, social dimensions, psychological dimensions, environmental dimensions. Based on the result of Lumbu study (2015) the number of samples were 71 people collected data using the (WHOQOL-BREF). There were 56 people (78,9%) had the poor quality of life of post stroke. The mean of post-stroke quality of life domain was physical domain (45,27%), psychological domain (49,87%), social relations domain (48,15%) and environmental domain (50.01%). Objective: the purpose of the study was know the quality of life of the stroke patients in Outpatient Polyclinic of Private Hospital in Yogyakarta. Methods: used descriptive quantitative by using questionnaire test of purposive sampling system based on patients who have been affected of ischemic or hemorrhagic stroke before, number 30 respondents. Result: quality of life of stroke patient of medium physical dimension (67%), psychological dimension (71%), social dimension (67%), dimension good environment (63%). Conclusion: the quality of life of stroke patients of physical dimension, psychological dimension, and moderate social dimension, while the quality of life of stroke patients were good environmental dimension.   Keywords: Hemorrhagic stroke, ischemic stroke, quality of life

Older people with dementia always experience the cognitive difficulties that are believed to impact their daily life significantly. However, information regarding occupation-based intervention toward older people with mild dementia in the institution is inconclusive. The results of this randomized controlled trial indicate a significant difference in cognitive functions, social relationship, and the quality of life. Participants in the experimental group improved significantly compared to the participants in the control group. Engagement in occupational activities should be encouraged to older people with mild dementia in institution to facilitate the cognitive functions, social relationship, and quality of life. To study and analyze the actual clinical effect of nursing intervention in stem cell therapy for stroke patients. A total of 9 stroke patients treated with stem cell therapy in our hospital from September 2018 to September 2019 were selected as the research and analysis objects. All patients were divided into observation group 5 stroke patients and control group 4 stroke patients by random number method. Both groups of patients were treated with stem cell therapy. Patients in the control group were treated with conventional nursing methods, and patients in the observation group were treated with nursing intervention methods. The treatment compliance, psychological status, quality of life and daily living ability before and after intervention were compared between the two groups. There were no significant differences in exercise training, disease monitoring and compliance with regular life between the two groups before intervention (P > 0.05), and the patients in observation group were significantly better than those in control group after intervention (P < 0.05), with significant differences. There was no significant difference in anxiety and depression scores between the two groups before intervention (P > 0.05), and after intervention, the patients in the observation group were significantly lower than those in the control group (P < 0.05), and the difference was statistically significant. There was no significant difference in quality of life and ability of daily living between the two groups before intervention (P > 0.05), and the patients in the observation group were significantly better than those in the control group after intervention (P < 0.05), the difference was statistically significant. The application of nursing intervention in stem cell therapy for stroke patients has a significant effect, which significantly improves the patient's compliance with the treatment, alleviates the negative emotions of patients, improves the psychological status of patients, improves the quality of life and daily living ability of patients, and is worthy of popularization and application in clinical nursing.

Introduction Motor imagery training delivered at home via telerehabilitation is a novel rehabilitation concept. The aim was to investigate the effects of telerehabilitation-based motor imaging training (Tele-MIT) on gait, balance and cognitive and psychosocial outcomes in people with multiple sclerosis (pwMS). Methods This randomized, controlled pilot trial included pwMS and healthy individuals. pwMS were randomly divided into two groups, intervention and control. The intervention group received Tele-MIT twice a week for 8 weeks. The control group was a wait-list group without any additional specific treatment. Healthy participants served as a baseline comparison. The Dynamic Gait Index, used to assess dynamic balance during walking, was the primary outcome. Secondary outcomes included assessments of walking speed, endurance and perceived ability, balance performance assessed by a computerized posturography device, balance confidence, cognitive functions, fatigue, anxiety, depression and quality of life. Results Baseline comparisons with healthy individuals revealed that motor imagery abilities were preserved in pwMS ( p > 0.05). The intervention group exhibited significant improvements in dynamic balance during walking ( p = 0.002), walking speed ( p = 0.007), perceived walking ability ( p = 0.008), balance confidence ( p = 0.002), most cognitive functions ( p = 0.001–0.008), fatigue ( p = 0.001), anxiety ( p = 0.001), depression ( p = 0.005) and quality of life ( p = 0.002). No significant changes were observed in the control group in any of the outcome measures ( p > 0.05). Discussion Tele-MIT is a novel method that proved feasible and effective in improving dynamic balance during walking, walking speed and perceived walking ability, balance confidence, cognitive functions, fatigue, anxiety, depression and quality of life in pwMS.

Background: The present investigation was designed to determine cognitive performance and quality of life (QoL) in a group of elite athletes who sustained spinal cord injury (SCI). Methods: nine participants suffering a SCI participated in the study. Different cognitive functions were evaluated through the following tests: COWAT, Digit Span, Stroop color–word and QoL through the WHOQoL-BREF scale. Results: Generally, participants positively assessed their overall quality of life and health status. Although the tests conducted indicate reduced cognitive function among the athletes, it did not affect the reduction in QoL. Single correlations between the results of cognitive tests and QoL could be treated as coincidental. Conclusions: Despite the observed decline in selected cognitive functions, the participants positively assessed their quality of life and physical health.Reduced cognitive functioning could be influenced by the impact of sleep-disordered breathing, pain, depressive disorders and medication. This indicates the need for an individualized approach to define the patient’s deficits, needs and best care. Further studies with a larger group of participants are needed.

Abstract Background: Statistics show that every year, 5.4 million people in the world suffer a stroke. Post-stroke cognitive impairment (PSCI) is one of the most common complications after stroke with a rate of 75%, which leads to decreased functions for independent living and reduced quality of life (QOL). Exercise training has been reported to be useful to improve the cognitive functions of post-stroke patients. Yijinjing, a traditional Chinese Qigong exercise characterized by an integration of mind and body in moderate exercise intensity, can improve cognitive functions of PSCI patients. This study aims to explore the feasibility and effectiveness of the Yijinjing exercise in this regard.Methods: A single-blind, randomized controlled trial will be employed with evaluations at three and six months. 72 PSCI patients will be recruited and randomly assigned to the Yijinjing exercise intervention group or the control group (1:1). Participants in the control group will receive routine rehabilitation therapies, including occupational therapy, physical therapy, acupuncture therapy, and health education 5 times a week for 3 months. The intervention group will receive a 12-week routine rehabilitation therapy combined with the Yijinjing exercise intervention for 40 minutes each session and 3 sessions a week. The primary outcome of cognition will be measured by the Montreal Cognitive Assessment scale (MoCA). Secondary outcomes include executive function, memory function, visuospatial function, sleep quality, gait and motor function, activity of daily living (ADL), and quality of life (QOL).Discussion: Current evidence has reported the effectiveness of traditional Chinese exercise in improving the post-stroke population’s motor functions. This research is a randomized controlled trial that evaluates traditional Chinese exercise’s effectiveness for PSCI patients. It is expected to expand the traditional Chinese exercise scope and provide a new treatment approach for stroke populations with cognitive impairments.Trail registration: Chinese Clinical Trial Registry, ChiCTR1900026532.
