Energy based devices have been developed for the purposes of tissue contraction and skin tightening. Its application in the face and neck have been explored using lasers, temperature controlled monopolar and bipolar radiofrequency, and ultrasound. The purpose of this chapter is to explore the various applications for the face and neck using Renuvion™, a unique energy driven device based on plasma generated from the combination of helium gas and radiofrequency energy. The advantage of this technology is its ability to offer precise delivery of heat to tissue with minimal thermal spread, in part due to the rapid cooling aided by the helium gas. We will explore the options in which this technology can be incorporated to rejuvenate the face and neck, the patient selection considerations in choosing method of approach, surgical technique, anticipated outcomes, potential concerns and or complications associated with this and expected perioperative care. Applications in the face and neck include: (1) Subdermally in the neck as a stand alone procedure with or without liposuction. (2) Subdermally in a limited incision, non-excisional technique with a concomitant platysmaplasty either with an open approach or percutaneous use of suture suspension for the platysmal muscle. (3) Subdermally in conjunction with an open traditional rhytidectomy involving skin excision. (4) Ablative resurfacing—fractional or pulsed and full continuous modalities (non-FDA cleared at the time of this writing). It is the authors’ experience that with appropriate patient selection this can be a powerful tool that can deliver skin tightening and rhytid reduction not seen by other technologies available.

Treating patients with heavy or thick features comes with challenges not present in those patients lacking these physical characteristics. The authors report our experience with cool atmospheric plasma for facial contouring and skin rejuvenation of the heavy face and neck including rhinophyma. Cool atmospheric plasma is generated by running helium gas over radiofrequency energy. The resulting plasma is a fourth state of matter and has enhanced clinical effects for ablation and thinning of skin and soft tissues as well of contouring and tightening of deeper soft tissues and fascia. Cool helium plasma has been a very effective tool for skin rejuvenation and skin tightening as well as using it as a tool for nonexcisional microinvasive face and neck rejuvenation. Future research may indicate that it can help treat primary or recurrent superficial cutaneous malignancies.

Abstract Background During peroral endoscopic myotomy (POEM), use of traditional instruments leads to an increase in overall time of procedure due to constant exchange of instruments. Speedboat-RS2 (Creo-Medical, UK) is a novel device which has shown promising results for endoscopic submucosal dissection of gastrointestinal lesions, but its feasibility, and safety for POEM is unexplored. Methods Data from patients who had undergone POEM for achalasia cardia utilizing Speedboat-RS2 was reviewed. Results Two patients with mean age of 46 years and mean symptom duration of 3 years were included. Both patients had type II achalasia diagnosed on esophageal manometry. One patient underwent anterior myotomy and the other posterior approach. Submucosal tunneling (~15 cm) was achieved in a mean of 20 minutes, myotomy (~ 9 cm) in 8 minutes and the entire procedure was completed in a mean of 30 minutes with 100 % technical success. Both patients were discharged the day after the procedure without any immediate adverse events. At 6 weeks, endoscopy was uneventful and the patients were asymptomatic. Conclusions Early experience shows that Speedboat-RS2 is feasible for performing POEM with good safety profile. Submucosal tunneling was relatively quick and coagulation was effective. Long term studies with a larger patient cohort are warranted.

Traditionally, liposuction of the upper arms addresses only the posterolateral aspect. We evaluated the efficacy of circumferential arm liposuction in a larger series of patients. Power-assisted tumescent liposuction in local anesthesia was performed on 79 female patients with enlarged upper arms. All patients showed an immediately visible reduction of the circumference of the upper arms and there was a significantly improved skin tightening effect. There were no complications. Circumferential liposuction of the upper arms is a not very well known, superior technique to reduce arm fat, which obviates skin excision in most cases. Using this approach, surgeons can expand their client base by including elderly and obese patients with skin ptosis who seek leaner and athletic looking arms without scars.

Arm contouring is a desired goal of the arm esthetics. Brachioplasty is a developing and safe surgery to improve the arm silhouette but scar is a one of the most distracting factor for both patients and surgeons. In this chapter conventional liposuction with laser assisted liposuction (LAL) is proposed to yield satisfactory results in selected group of patients. Classification systems are helpful to decide for proper patient selection for this technique. It includes assessment of fat excess, skin excess, and location of the deformity (proximal, entire arm, arm and chest). Patients with minimal to extensive fat deposits plus minimal to moderate skin laxity are the best candidates for this approach. When properly performed, with realistic expectations, liposuction and LAL combination is considerably a procedure of choice for arm contouring in these selected cases. The contraction of arm skin is considerably consistent. Our approach, except in the most extreme cases, is to initially recommend liposuction and possibly even a second liposuction prior to recommending brachioplasty. An esthetically pleasant result can be obtained even in massive arms with good skin tone. Massive arms with poor skin tone, however, may not. Patient selection and preoperative planning are discussed in detail. A thorough description for patient positioning and anesthesia options are studied. The technique of the liposuction is widely described including specific regional contour goals with artistic attention to enhance the contour. Refinement regarding skin tightening and skin surface smoothness are also discussed. Arm contouring is a growing field of body contouring and can be practiced with low complication rates and high patient satisfaction. Available classification systems help to select proper patient group. Laser assisted fat removal combined with conventional liposuction are promising procedures for selected patients.

Abstract: Children ministry nowadays faces challenges both from inside and outside church. This article focuses on the external challenges, namely the postmodern influence on the paradigm of the generation Z and what the church can do to respond. Some challenges in as well as opportunities for the ministry of the generation Z include the center on technology and media, the rising of new morality that set asides the value of an absolute truth, the emphasis on experience and not only on fact, and lastly, the rejection of authority or figures with authority and the appreciation for personality that is real, authentic, and trustworthy. In the face of these challenges, this article proposes several thoughts to be considered by the church for its children ministry in current context, namely: first, including children in the common worship, together with the adult congregations, as a means of nurturing the children’s’ faith. Secondly, in preaching, the children minister needs to exercise open approach Bible study, not close one, where children are led to ponder and discover from the biblical narratives truths and values for their personal lives. Lastly, to encourage and equip both parents to regain roles that God has designed for them, to actively participate in the spiritual formation of their children.   Keywords: children ministry, worldview, generation Z.     Abstrak: Pelayanan anak dewasa ini mendapatkan tantangan baik dari dalam maupun dari luar gereja. Artikel ini secara khusus menyoroti tantangan dari luar, yaitu pengaruh pascamodernisme terhadap paradigma generasi Z dan apa yang dapat gereja lakukan dalam meresponinya. Beberapa tantangan yang sekaligus dapat menjadi peluang bagi pelayanan kepada generasi Z adalah: adanya pemusatan pada teknologi dan media, munculnya moralitas baru yang menggeser nilai kebenaran absolut, penekanan pada pengalaman dan bukan hanya fakta, dan terakhir adalah penolakan pada otoritas atau figur otoritatif dan apresiasi pada pribadi yang real, otentik, dan dapat dipercaya. Berhadapan dengan tantangan demikian, beberapa hal yang perlu dipertimbangkan oleh gereja dalam melakukan pelayanan anak dalam konteks kekinian yang diusulkan dalam artikel ini antara lain: pertama, melibatkan anak-anak dalam ibadah bersama dengan jemaat dewasa sebagai salah satu bentuk pengasuhan iman anak. Kedua, dalam hal pemberitaan firman, pelayan anak perlu menerapkan sistem studi Alkitab yang terbuka bukan yang tertutup, di mana anak dituntun untuk memikirkan dan menemukan kebenaran serta nilai-nilai pribadi mereka sendiri dari kisah-kisah Alkitab. Terakhir, mendorong serta memperlengkapi orang tua, yaitu ayah dan ibu untuk kembali kepada peran yang Allah rancang bagi para orang tua untuk berperan aktif dalam pembentukan spiritual anak-anaknya.   Kata-kata kunci: pelayanan anak, pandangan dunia (worldview), generasi Z.
