Background: Hemorrhoid is an anorectal disease due to mechanical disturbances in the anal cushion and hemorrhoid venous plexus blood flow obstruction. Symptoms that often accur are bleeding, pain, prolapse, itching, and mucus discharge. Acupuncture is non-pharmacological therapeutic modalities that has been shown to improve hemorrhoid symptoms. Case: A 37-year-old woman complained of hemorroid lump with pain since last 1 year. Physical examination showed a reddish hemorrhoid prolapse, about 1.5 cm in the posterior anal without bleeding. The pain score with NRS is 4. Acupuncture therapy was performed 3 times a week at acupoints GV20, TE6, LU6, ST25, BL34, ST36, EX-UE2 and BL57. Then given continuous wave electrical stimulation, 4 Hz, 30 minutes retention. After 11 treatments, pain disappeared with NRS 0 and lump was reduced to about 0.5 cm. Conclusion: Acupuncture can improve the symptoms of hemorrhoids, which are seen by the relief of pain and the reduction of lumps.   Keywords: Acupuncture, hemorrhoid, hemorrhoid pain, hemorrhoid prolapse

In the present era of 21st century, haemorrhoids have become the most common and distressing anorectal disease. It is a lifestyle disease due to excessive intake of spicy food non-vegetarian food, and lack of exercise. Inadequate consumption of water and the habit of taking alcohol are some of the main reasons for the manifestation of the disease. Perianal trauma due to the long journey and excessive two-wheeler riding particularly on uneven roads and terrenes are also main reasons. Constipation is also a common cause of this disease. Haemorrhoids are dealt with rationally under the concept of Arshas. Use of medicine, surgery, cauterization with kshara and Agni are the treatment principles recommended for Arshas by Acharya Susruta. Except medicine, the rest of the procedures are invasive. The basic principles for the treatment of Arshas are deepana and anulomana (digestive, carminative, and laxative). A medicine with the above properties which is used traditionally by tribes is selected for the study. It has been found using among the tribes for a long time with no side effects. The contents of this medicine are safe and are used in routine ayurvedic practice. In this case study, an effort was made to evaluate the role of Mustadimodaka and ethnomedicine in the management of Arshas. Keywords: 2nd-degree haemorrhoids, Arshas, Mustadimodaka, Ethnomedicine

Urinary retention after surgery is caused by inadvertent damage to the muscles and nerves around the bladder during the operation, which results in the suppression of the urination reflex, or the postoperative pain that causes sphincter spasm and dysuria. It is a common complication after anorectal disease, with an incidence rate of up to 52%, which seriously affects the daily life of patients. It has been clinically discovered that acupuncture at Xinbaliao point can effectively improve urinary retention, relieve swelling in the lower abdomen, and improve the quality of life of patients. In this report, we present a case of postoperative urinary retention that had been treated with acupuncture at the Xinbaliao point.

Perianal abscess is a relatively common disease in the anorectal department, and its incidence accounts for more than 20% of anorectal disease. Once diagnosed in the clinic, surgical drainage is needed immediately, which is of great significance for the clinical outcome. In this study, patients with anorectal abscess diagnosed by transmittal biplanar intracranial ultrasound and multi-slice spiral CT were selected in our hospital. They were randomly divided into the ultrasound group and the MSCT group. Transrectal biplanar intracranial ultrasound has a higher diagnosis rate for perianal abscess, and the localization has a unique advantage, which provides a powerful image basis for the choice of clinical treatment.

Coronavirus 2019 disease (COVID-19) has created major impacts on public health. The virus has plagued a large population requiring hospitalization and resource utilization. Knowledge about the COVID-19 virus continues to grow. It can commonly present with gastrointestinal symptoms; initially, this was considered an atypical presentation, which led to delays in care. The pandemic has posed serious threats to the care of anorectal diseases. Urgent surgeries have been delayed, and the care of cancer patients and cancer screenings disrupted. This had added to patient discomfort and the adverse outcomes on healthcare will continue into the future. The better availability of personal protective equipment to providers and standard checklist protocols in operating rooms can help minimize healthcare-related spread of the virus. Telehealth, outpatient procedures, and biochemical tumor marker tests can help with mitigation of anorectal-disease-related problems. There is limited literature about the clinical management of anorectal diseases during the pandemic. We performed a detailed literature review to guide clinicians around management options for anorectal disease patients. We also highlighted the health challenges seen during the pandemic.
