Genetically Engineered Biologic Drugs in Management of Children with Bronchial Asthma

Current article represents the modern clinical guidelines on management of severe bronchial asthma (BA) in children and practical use of genetically engineered biologic drugs. Clinical efficacy and safety of omalizumab has its special role. Efficacy analysis was carried out in real-life' clinical setting (considering high economical expenses of biological treatment) to estimate effective response predictors and principles of patients selection for such therapy. Two years of anti-IgE treatment experience in inpatient pediatric department settings demonstrates that omalizumab inclusion to treatment of children with severe asthma resistant to standard therapy allows to solve asthma symptoms, to forgo high doses inhaled glucocorticosteroids, to improve lung function parameters, and to increase significantly quality of life in 95% of our patients.

The thesis examines the role, efficacy and influence of the five national English-language independent film and television production sector unions in the Canadian broadcasting policy network. While labour is typically classified as a civil society organization within policy networks studies, this thesis will examine the blanket applicability of this typology in analysing labour's engagement with issues that involve both their vested economic/industrial interests as well as broader social/cultural goals, using the unions' engagement with the issue of Canadian dramatic programming from 1998 to present as a case study.

Asthma Phenotypes and Current Biological Treatments

Asthma is a heterogeneous disease characterized by bronchial hyperreactivity, chronic airway inflammation, and reversible airflow obstruction, and it affects individuals in all age groups. In recent years, the concept of intrinsic and extrinsic asthma as per the former classification has been replaced by endotypic and phenotypic definitions. However, the two main asthma endotypes described and have simplified its classification. These endotypes, “Th2-high” and “Th2-low”, are based on various measurements obtained for different biological materials, including blood, bronchial and sputum samples. The definitions of asthma is useful for targeted and individualized treatments, estimating the treatment response and prognosis. In the field of respiratory medicine, biological drugs (BDs) have shown rapid evolution and positive developments in the last 10 years, particularly for the treatment of asthma, interstitial lung disease, and lung cancer. However, because of the increasing number of BDs and associated studies, it has become very difficult to update treatment guidelines on a regular basis. BDs are used for patients with difficult-to-treat, moderate to severe, and/or uncontrolled allergic asthma. Here we present a review of current asthma phenotypes and the role, efficacy, and side effects of BDs used for the treatment of these conditions.

Pelatihan Intentional Change Model untuk Meningkatkan Leader Effectiveness Pengurus PPGT

Efektifitas pemimpin bukanlah hal yang mudah untuk dicapai oleh seorang pemimpin. Pemimpin itu harus membangun kesadaran untuk memperlengkapai diri dan anggotanya demi tercapainya tujuan. Keyakinan intrapersonal dalam kapabilitasnya untuk berfungsi sebagai pemimpin disebut leader role-efficacy (LRE), kemampuannya untuk menavigasi dan mengelola hubungan interpersonal dengan anggotanya disebut leader trust in subordinate (LTS), keduanya merupakan elemen kunci dari leader effectiveness. Penelitian ini bertujuann untuk mengetahui apakah pelatihan intentional change model (ICM) dapat meningkatkan leader effectiveness pengurus Persekutuan Pemuda Gereja Toraja (PPGT) jemaat Pajalesang Palopo. Subjek penelitian yaitu pengurus organisasi PPGT berjumlah limabelas orang sebagai one group pre and post-test design. Kuesioner ­pre-test diberikan seminggu sebelum perlakuan dan kuesioner post-test diberikan dua minggu setelah perlakuan. Alat ukur yang digunakan oleh peneliti adalah Leader effectiveness scale dari Ladegard dan Gjerde (2014) dan Psychological Capital Quetionnaire (PCQ) dari Luthans, Youssef, dan Avolio (2015). Dari pengukuran menggunakan teknik analisa t sample t-test. Hasil penelitian bahwa terdapat perbedaan signifikan nilai sebelum dan setelah diberi perlakuan pelatihan intentional change model dengan nilai signifikansi sebesar p=0,000 (p?0,05). Study about Ambivalence and Desire for Child Birth by Working Mothers: Focusing on Mediation Effect of Multiple-role Efficacy

This study examined the relationship between ambivalence of Korean working mothers for children and their future desire for child birth. This study examined multiple-role efficacy as a mediator in the relationship between ambivalence for their children and future desire for child birth by Korean working mothers having more than one pre-school aged child. This study examined the mediation effect of multiple-role efficacy on the relationship between ambivalence and desire for child birth by working mothers. On-line and off-line survey data from 291 Korean working mothers with more than one pre-school aged child was used for this study. Collected data were analyzed using the PASW 18.0 program and descriptive statistics analyses. Structural equation modeling and bootstrapping were conducted by AMOS 20.0 program. Results showed that ambivalence for children of working mothers having pre-school aged children had no direct influence on future child birth desire. However, their ambivalence had an indirect influence on future child birth desire through multiple-role efficacy and multiplerole efficacy by working mothers had significant effect on child birth desire. In conclusion, the working mothers’ multiple-role efficacy had a mediation influence on the relationship between ambivalence for children and desire of child birth. These results were discussed for designing a policy to counter low birth.

Updates in the management of chronic lymphocytic leukemia/small lymphocytic leukemia

Chronic lymphocytic leukemia is one of the most common lymphoid malignancies. Often treatment modalities are tailored to individual patients based on age, presence of comorbidities and cytogenetics. The advent of ibrutinib has significantly changed the management of the disease in all patient groups and has had the largest impact on clinical practice to date. Over the last 15 years, a series of trials have established that chemoimmunotherapy improves both progression-free survival and overall survival compared to chemotherapy alone. Despite its proven role, efficacy of ibrutinib has not been well established in young, fit patients and in comparison with standard care and as combination therapy with other agents such as venetoclax. New data have strengthened the role of ibrutinib in the front-line setting and establish its place in therapy. In addition, combination therapies are geared to achieve negative minimal residual disease and allow patients to potentially be off of therapy. The management of this leukemia has extensively changed over the past years, and this review article will aim to highlight key trials that have changed practice and led to guideline updates. It is not unlikely that treatment modalities will continue to improve in light of new data.
