Rekam Medis merupakan berkas yang berisi tentang identitas pasien, pemeriksaan, pengobatan, tindakan serta pelayanan yang diberikan kepada pasien. Data rekam medis seiring waktu akan terakumulasi setiap hari seiring dengan aktivitas di rumah sakit. Selama ini data rekam medis yang tersedia di Rumah Sakit Umum Daerah Bintan hanya menampilkan informasi berupa data statistik maupun grafik. Sedangkan, pola dan kecenderungan penyakit yang diderita oleh pasien masih belum di gali untuk dijadikan acuan untuk penyuluhan ke masyarakat maupun sebagai pencegahan penyakit, terutama data rekam medis yang berasal dari Poliklinik Penyakit Dalam. Poliklinik Penyakit Dalam merupakan salah satu Poliklinik yang banyak melayani pasien di Rumah Sakit Umum Daerah Bintan, sehingga data rekam medis yang berasal dari Poliklinik Penyakit Dalam belum bisa digali secara maksimal. Pada penelitian ini penulis melakukan penerapan Algoritma Apriori dalam menentukan aturan asosiasi berdasarkan jenis kelamin, umur, kelurahan, dan kode ICD10. Dengan menggunakan data sebanyak 1668 dan menggunakan minimal support sebesar 6% dan minimal confidence sebesar 50%, kemudian didapatkan hasil aturan asosiasi sebanyak 22 aturan. Aturan asosiasi yang dihasilkan pada penelitian ini dapat digunakan oleh pihak rumah sakit maupun pihak dinas kesehatan untuk melakukan sosialisasi dan penyuluhan kesehatan kepada masyarakat.

Abstract Neonates with apnea of prematurity (AOP) deteriorate clinically because CPAP often provides inadequate support during apnea. NAVA provides proportional ventilator support from the electrical activity of the diaphragm. When NAVA level is 0 cmH20/mcV (NN0), patients receive minimal support when breathing and backup ventilation when apneic. This study addresses feasibility of NN0 and time spent in back up ventilation. This was a prospective, two center, observational study of preterm neonates on NN0 for AOP. Ventilator data were downloaded for 24 hours. Number of Clinically Significant Events (CSEs) and switches to back up were collected. Statistics were paired t-test. There were twenty-eight subjects with gestational age of 25 + 1.8 weeks; study age 28 ± 23 days. Number of events were 4±4.39 per 24 hours. Subjects were on NN0 approximately 90% each minute, back-up mode 2.5±1.1 times/minute with 10.6±7.2% in back-up. Conclusion: Preterm neonates on NAVA level 0 had few clinically significant events with minimal time in back up ventilation.

UFGLI students comprise 34% of the students enrolled in four-year universities. Unlike some students, UFGLI students face internal and systemic barriers throughout their educational experience and their struggles are often dismissed and disregarded. Working and raising a family while taking courses, minimal support systems, and financial struggles require students to optimize their resources. We explore the issues of UFGLI students and the importance of their spiritual and religious supports using a literature review and a case study. Religious and spiritual identities are resources that should be explored and supported by staff at university counselling centers. Affirming UFGLI students’ religious and spiritual identities and understanding how religion and spirituality work in their lives can assist these students in their acclimation to and success at university.

Social policy was immensely important in Weimar Germany. In this area the republic, defining itself as a welfare state, frequently clashed with the expectations of its citizens. In fact, Weimar’s social policy was progressive and ambitious, for instance implementing unemployment welfare and collective bargaining agreements as two of its key pillars. But the tight economic constraints in which the Weimar state was operating also meant that the minimal support for pensioners, disabled war veterans, and war widows during the hyperinflation created true and lasting hardship. Furthermore, many citizens experienced the assessment of their claims as unfair and humiliating. Social policy in Weimar Germany was severely burdened by the limited financial possibilities the state had available. Ultimately, the republic failed in adequately communicating why it could not match public expectations.

AbstractBackgroundPeople experiencing homelessness who live in congregate shelters are at high risk of SARS-CoV2 transmission and severe COVID-19. Current screening and response protocols using rRT-PCR in homeless shelters are expensive, require specialized staff and have delays in returning results and implementing responses.MethodsWe piloted a program to offer frequent, rapid antigen-based tests (BinaxNOW) to residents and staff of congregate-living shelters in San Francisco, California, from January 15th to February 19th, 2021. We used the Reach-Effectiveness-Adoption-Implementation-Maintenance (RE-AIM) framework to evaluate the implementation.ResultsReachWe offered testing at ten of twelve eligible shelters. Shelter residents and staff had variable participation across shelters; approximately half of eligible individuals tested at least once; few tested consistently during the study.Effectiveness2.2% of participants tested positive. We identified three outbreaks, but none exceeded 5 cases. All BinaxNOW-positive participants were isolated or left the shelters.AdoptionWe offered testing to all eligible participants within weeks of the project’s initiation.ImplementationAdaptations made to increase reach and improve consistency were promptly implemented.MaintenanceSan Francisco Department of Public Health expanded and maintained testing with minimal support after the end of the pilot.ConclusionRapid and frequent antigen testing for SARS-CoV2 in homeless shelters is a viable alternative to rRT-PCR testing that can lead to immediate isolation of infectious individuals. Using the RE-AIM framework, we evaluated and adapted interventions to enable the expansion and maintenance of protocols.

ObjectiveTo describe the Australian adult public’s knowledge and experiences regarding substitute decision-making for medical decisions and their preferences for obtaining information about the substitute decision-maker (SDM) role.MethodsThis is a national cross-sectional online survey of the Australian adult public. The survey examined participants’ advance care planning (ACP) awareness and experience, SDM experiences and preferences for obtaining more information about SDM, and participant knowledge about SDM.ResultsOf 1586 people who opened the survey, 1120 (70.6%) were included in the final sample. 13% (n=142) of participants indicated they had acted as an SDM. A median score of two correct responses out of five showed low to moderate knowledge about the SDM role among all participants, with only 33% reporting awareness of SDM laws existing in Australia. While most (59%) participants ranked a health professional as their preferred source of obtaining information about supporting SDMs, few participants who had been an SDM (n=64, 45%) reported obtaining any support in making medical decisions. The median SDM knowledge scores for people who had discussed ACP (3.0 vs 2.0, U=1 45 222, z=6.910, p<0.001), documented their ACP preferences (3.0 vs 2.0, U=71 984, z=4.087, p<0.001) or acted in the SDM role (3.0 vs 2.0, U=56 353, z=−3.694, p<0.001) were significantly higher compared with those who had not.ConclusionsThe Australian public may have low to moderate knowledge about the SDM role and access only minimal support when making challenging medical decisions.

Perkembangan teknologi sangatlah cukup terasa bagi semua lini kehidupan tidak terkecuali dalam sektor dunia usaha perdagangan. Menjamurnya toko online merupakan salah satu dampak dari perkembangan teknologi yang merubah cara pola berpikir konsumen dalam bertransaksi. Walaupun banyak muncul toko online baru setiap tahun namun faktanya banyak juga toko online yang tidak bertahan lama, karena persaingan yang sangat ketat dan salah dalam strategi pemasaran. Kesalahan dalam promosi barang harus selalu diwaspadai oleh perusahaan supaya perusahaan tidak terlalu banyak mengeluarkan biaya promosi yang sebenarnya barang tersebut tidak disukai konsumen. Membaca pola transaksi konsumen merupakan salah satu cara dalam menghadapi masalah ini, oleh sebab itu pemanfaatan data mining tidak bisa dianggap remeh. Dalam proses data mining, metode asosiasi dengan algoritma apriori bisa menjadi solusi dalam masalah ini, dimana cara kerja algoritma ini yaitu mencari pola keterkaitan antar barang dari data transaksi penjualan, sehingga hasil dari penelitian ini dengan batasan minimal support 10% dan confidence 70% pada 30 data transaksi diketahui bahwa keterkaitan produk 34 dengan 33 sebesar 80% dan produk 49 dengan 51 sebesar 75% serta produk 51 dengan 49 sebesar 75% sehingga hasil informasi yang dihasilkan algoritma ini dapat dimanfaatkan dalam membuat sebuah promo barang yang sesuai oleh keinginan konsumen sendiri.
