AbstractVisible light enters our body via the pupil. By changing its size, the pupil shapes visual input. Small apertures increase the resolution of high spatial frequencies, thus allowing discrimination of fine details. Large apertures, in contrast, provide a better signal-to-noise ratio, because more light can enter the eye. This should lead to better detection performance of peripheral stimuli. Experiment 1 shows that the effect can reliably be demonstrated even in a less controlled online setting. In Experiment 2, pupil size was measured in a laboratory using an eye tracker. The findings replicate findings showing that large pupils provide an advantage for peripheral detection of faint stimuli. Moreover, not only pupil size during information intake in the current trial n, but also its interaction with pupil size preceding information intake, i.e., in trial n-1, predicted performance. This suggests that in addition to absolute pupil size, the extent of pupillary change provides a mechanism to modulate perceptual functions. The results are discussed in terms of low-level sensory as well as higher-level arousal-driven changes in stimulus processing.

Abstract Introduction Non-alcoholic fatty liver disease (NAFLD) is one of the main causes of chronic liver disease worldwide. Flavonoids, a group of natural compounds, have garnered a great deal of attention in the management of NAFLD because of their profitable effects on glucose and lipid metabolism, inflammation, and oxidative stress which are the pivotal pathophysiological pathways in NAFLD. Naringenin is a citrus-derived flavonoid with a broad spectrum of potential biological effects including anti-inflammatory and antioxidant properties, which may exert protective effects against NAFLD. The present clinical trial aims to examine the efficacy of naringenin supplementation on plasma adiponectin and neurogulin-4 (NRG-4) concentrations, metabolic parameters, and liver function indices in overweight/obese patients with NAFLD. Methods and analysis This is a double-blind, randomized, placebo-controlled clinical study that will investigate the impacts of naringenin supplementation in overweight/obese patients with NAFLD. Liver ultrasonography will be applied to diagnose NAFLD. Forty-four eligible overweight/obese subjects with NAFLD will be selected and randomly assigned to receive naringenin capsules or identical placebo (each capsule contains 100 mg of naringenin or cellulose), twice daily for 4 weeks. Participants will be asked to remain on their usual diet and physical activity. Safety of naringenin supplementation was confirmed by the study pharmacist. The primary outcome of this study is changes in adiponectin circulating levels. The secondary outcomes include changes in NRG-4 levels, liver function indices, metabolic parameters, body weight, body mass index (BMI), waist circumference (WC), blood pressure, and hematological parameters. Statistical analysis will be conducted using the SPSS software (version 25), and P value less than 0.05 will be regarded as statistically significant. Discussion We hypothesize that naringenin administration may be useful for treating NAFLD by modulating energy balance, glucose and lipid metabolism, oxidative stress, and inflammation through different mechanisms. The current trial will exhibit the effects of naringenin, whether negative or positive, on NAFLD status. Ethical aspects The current trial received approval from the Medical Ethics Committee of Tehran University of Medical Sciences, Tehran, Iran (IR.TUMS.MEDICNE.REC.1399.439). Abstract Background Sarcoidosis is an inflammatory disease characterized by the formation of granulomas, which involve the heart in up to 25% of patients. Cardiac sarcoidosis can lead to life threatening arrhythmias and heart failure. While corticosteroids have been used as a treatment for over 50 years, they are associated with hypertension, diabetes, and weight gain, further increasing cardiovascular risk. Interleukin-1 (IL-1) is the prototypical proinflammatory cytokine that works to activate the nuclear transcription factor NF-kB, one of the targets of glucocorticoids. IL-1 also plays an important role also in the pathophysiology of heart disease including atherosclerosis, myocardial infarction, and myocarditis. Methods Building on a network of research collaborators developed in the Cardiac Sarcoidosis Consortium, we will investigate the feasibility and tolerability of treatment of CS with anakinra at two National Institute of Health Clinical and Translational Science Award (CTSA) hubs with expertise in cardiac sarcoidosis. In this pilot study, up to 28 patients with cardiac sarcoidosis will be recruited to compare the administration of an IL-1 blocker, anakinra, 100 mg daily on top of standard of care versus standard of care only for 28 days and followed for 180 days. Utilizing surrogate endpoints of changes in systemic inflammatory biomarkers and cardiac imaging, we aim to determine whether IL-1 blockade with anakinra can combat systemic and cardiac inflammation in patients with cardiac sarcoidosis. Discussion The current trial demonstrates an innovative collaborative approach to clinical trial development in a rare, understudied disease that disproportionately affects females and minorities. The Einstellung effect was first described by Abraham Luchins in his doctoral thesis published in 1942. The effect occurs when a repeated solution to old problems is applied to a new problem even though a more appropriate response is available. In Luchins' so-called water jar task, participants had to measure a specific amount of water using three jars of different capacities. Luchins found that subjects kept using methods they had applied in previous trials, even if a more efficient solution for the current trial was available: an Einstellung effect. Moreover, Luchins studied the different conditions that could possibly mediate this effect, including telling participants to pay more attention, changing the number of tasks, alternating between different types of tasks, as well as putting participants under time pressure. In the current work, we reconstruct and reanalyze the data of the various experimental conditions published in Luchins' thesis. We furthermore show that a model of resource-rational decision-making can explain all of the observed effects. This model assumes that people transform prior preferences into a posterior policy to maximize rewards under time constraints. Taken together, our reconstructive and modeling results put the Einstellung effect under the lens of modern-day psychology and show how resource-rational models can explain effects that have historically been seen as deficiencies of human problem-solving.

Studies of neural dynamics in lateral orbitofrontal cortex (lOFC) have shown that subsets of neurons that encode distinct aspects of behavior, such as value, may project to common downstream targets. However, it is unclear whether reward history, which may subserve lOFC's well-documented role in learning, is represented by functional subpopulations in lOFC. We analyzed neural recordings from rats performing a value-based decision-making task, in which we previously documented trial-by-trial learning that required lOFC. We found five distinct clusters of lOFC neurons, either based on clustering of their trial-averaged peristimulus time histograms (PSTHs), or a feature space defined by their average conditional firing rates aligned to different task variables. We observed weak encoding of reward attributes, but stronger encoding of reward history, the animal's left or right choice, and reward receipt across all clusters. Only one cluster, however, encoded the animal's reward history at the time shortly preceding the choice, suggesting a possible role in integrating previous and current trial outcomes at the time of choice.

We investigated perceived timing in auditory rate perception using a reproduction task. The study aimed to test (a) whether central tendency occurs in rate perception, as shown for interval timing, and (b) whether rate is perceived independently on each trial or shows a serial dependence, as shown for other perceptual attributes. Participants were well able to indicate perceived rate as reproduced and presented rates were linearly related with a slope that approached unity, although tapping significantly overestimated presented rates. While the slopes approached unity, they were significantly less than 1, indicating a central tendency in which reproduced rates tended towards the mean of the presented range. We tested for serial dependency by seeing if current trial rate reproductions depended on the preceding rate. In two conditions, a positive dependence was observed. A third condition in which participants withheld responses on every second trial produced a negative dependency. These results suggest separate components of serial dependence linked to stimulus and response: Withholding responses reveals a negative perceptual effect, whereas making responses adds a stronger positive effect that is postperceptual and makes the combined effect positive. Together, these data show that auditory rate perception exhibits both central tendency and serial dependence effects.

Abstract Over the last 10 years since the introduction of non-vitamin K antagonist oral anticoagulants (NOACs) into routine clinical practice our experience with these drugs has increased tremendously, also in the context of patients undergoing electrophysiology procedures. While some open questions remain, the available evidence indicates that for the majority of cases, these interventions can safely be performed on NOACs if study-based standard operating procedures are in place and followed. This review summarizes the most current trial evidence and guidelines on the use of NOACs for patients undergoing cardioversion, atrial fibrillation ablation, and device implantations, based on previous work of the author and others.
