Angka kekambuhan gangguan jiwa di Jawa Tengah pada tahun 2018 mencapai 2,2% per 1.000 penduduk. Puskesmas Bandarharjo sendiri merupakan Puskesmas dengan kasus kesehatan jiwa tertinggi di Semarang. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan Program Kesehatan Jiwa di wilayah kerja Puskesmas Bandarharjo. Desain penelitian ini adalah kualitatif dengan pendekatan deskriptif analitik. Data dikumpulkan dari wawancara mendalam dan observasi berdasarkan kriteria purposive sampling. Subjek dalam penelitian ini terdiri dari dua orang dokter eksekutif, seorang koordinator Program Kesehatan Jiwa, dan seorang perawat. Keempat subjek ini dikategorikan sebagai informan utama. Perwakilan dari keluarga pasien gangguan jiwa dan kader kesehatan juga diikutsertakan sebagai informan triangulasi. Hasilnya, pelaksanaan program kesehatan jiwa di Puskesmas Bandarharjo belum optimal. Hal ini dikarenakan jumlah pasien gangguan jiwa berat yang mendapatkan pelayanan kesehatan jiwa sesuai standar hanya 94,7%. Oleh karena itu, masih ada sekitar 5,3% pasien yang belum mendapatkan pelayanan kesehatan jiwa. Masih kurangnya Standard Operation Procedure (SOP) untuk setiap tahapan pelaksanaan program, terutama tahap pengelolaan awal (pemberian obat) dan sosialisasi tentang Program Kesehatan Jiwa. Sebaiknya sosialisasi dan promosi Program Kesehatan Jiwa di Puskesmas Bandarharjo ditingkatkan. Perlu adanya perbaikan sistem pendistribusian obat dari Dinas Kesehatan Kota ke Puskesmas agar tidak terjadi keterlambatan serta review terhadap aturan BPJS terbaru terkait sistem rujukan berjenjang yang masih menjadi permasalahan di lapangan

Abstract At the end of the lifespan of this onshore oilfield development, all wells and oil production system, transmission pipelines and all surface infrastructures shall be decommissioned in accordance with international guidelines for abandonment of oil and gas facilities. The development and production period relevant to Block 1/2/4 shall be ceased in 2033 as the PSC law approved by South Sudan Government. Considering that South Sudan is located along the Nile River in Africa, necessary measures shall be taken with respect to activities in the field to ensure effective protection for the environment. In addition, difficulties which have negative impact on decommission assessment shall be taken into account, such as inadequate wells data, the destruction of the facility by war, high security risks of Block 1/2/4, costly decommissioning and abandonment fees and so on. This paper combines the practice of Block 1/2/4 in South Sudan to introduce the decommissioning plan, abandonment working process, standard operation procedure for wells and surface facility disposal, and the calculation of the decommissioning fund. The decommissioning may include complete removal or abandonment in-situ which based on an evaluation of cost, safety and environmental impact. While a detailed decommissioning and abandonment proposal has been compiled and updated during the production phase of the project. When the final decommissioning and abandonment plans have been developed for upstream and midstream, the costs have been updated accordingly. These estimates had utilized by the operators to fund the decommissioning and abandonment obligations in accordance with the provisions of the oil company. The D&A task was successful and finally approved by the authority of government. Due to the successful completion of the decommissioning task of Block1/2/4, a completed decommissioning and abandonment management system has been established, including D&A cost calculation and standard operation procedure for wells and facility disposal, which can lower the environmental risk and reduce the capital expenditure at the end of the project. It can be regarded as a classic practice in petroleum industry and has a strong reference value.

The focus of this research is to ensure that the implementation of the Health, Safety and Environment (HSE) program which runs as the goals and the objectives that have been set, the factors that lead to the success or failure of the program and the benefits derived from the implementation of the HSE program for employees and companies. This research is a qualitative research using the method of evaluation of the Context Input Process Product (CIPP) approach. Data taken using the guidelines interviews, questionnaires and observation studies and in the analysis of documents with descriptive methods through discussions and triangulation. All the activites of decision data is performed to all stakeholders components into aspects of evalution in this study include: Context Evaluation (C): The importance of goals and objectives in the implementation of the program; Input Evaluation (I): Strategies, procedures and activities of the program; Process Evaluation (P): Implemention process of the program; Product Evaluation (P): Outcomes and benefits of the program. The results showed that the implementation of HSE program is generally in accordance with its performance indicators but the success of the program has not been fully fulfilled because the program strategy is not directed and incomplete so that the implementation process is not quite as it should be because of the absence of adequate standard operation procedure. The recommendation of this research is that if the company wishes to continue the HSE program successfully, the company should review the vision, mission, goals and objectives of the program (Context), then fix the program strategy completely and purposefully (Input), completing the operation procedures, so that the implementation process in accordance with the standard operation procedure (Process)) that will further ensure the success of this HSE program (Product).

To establish a standard operation procedure (SOP) for shelter management, this research analyzed current situations about shelter management in Japanese local governments, focusing on designation of shelters, operation manager and information sharing. The results indicate that some have non-designated shelters, which might lead to differences of support with designated shelters, local governments ask residents to operate shelters mainly, but this requirement is not shared among them, and system of information sharing is not developed primarily with the department of health and welfare. Besides, this research shows the importance of including the closing and after the closing of the shelters.

Abstrak--Proses perawatan kendaraan servis berkala ekternal (SBE) di bengkel Auto 2000 XXX yang merupakan bengkel yang memiliki customer yang cukup banyak, seharusnya sudah dilakukan sesuai dengan standard operation procedure (SOP), kecuali khususnya yang berkaitan dengan mengangkat beban berat seperti roda. Masalah yang terjadi dalam proses pemasangan roda adalah karena banyaknya pelanggan bengkel pengguna roda besar, sehingga teknisi harus memasang roda dengan prosedur tambahan seperti menaikkan atau menurunkan lift, memantul-mantulkan roda dan lainnya. Hal ini menambah lamanya waktu dan juga berbahaya bagi teknisi karena beratnya roda yang melebihi standar (> 20 kg). Untuk mengatasi masalah tersebut dilakukan penelitian dengan membuat Special Service Tool (SST) untuk mempercepat proses pemasangannya. Fokus dari perbaikan adalah mempercepat proses pemasangan roda menggunakan SST Little Helper. Efisiensi waktu meningkat sebesar 41%. Tingkat keamanan proses pemasangan roda meningkat dari sebelumnya 24% menjadi 80%. Tingkat kemudahan juga meningkat dari sebelumnya 62% menjadi 66%. Zusammenfassung Berichtet wird über die Erfahrungen mit einer interdisziplinären klinikinternen SOP (Standard Operation Procedure) zur Tracheostomie (TS) bei „Coronavirus-Disease“(COVID-19)-Patienten, unter Berücksichtigung der allgemeinen nationalen und internationalen Empfehlungen. Der interdisziplinär festgelegte operative Zeitpunkt der TS aufgrund einer prolongierten invasiven Beatmung und frustranen Weaning-Versuchen betraf Phasen sowohl hoher als auch niedriger Erkrankungsaktivität. Es wurden 5 TS bei Patienten mit einem Durchschnittsalter von 70,6 Jahren durchgeführt. Neben den Standard-COVID-19-Schutzmaßnahmen für das medizinische Personal zur Vermeidung einer nosokomialen COVID-19-Infektion führt die SOP-unterstützte Kommunikation während der TS zu einer periprozeduralen Sicherheit aller Beteiligten. COVID-19-Erkrankungen des medizinischen Personals der beteiligten Abteilungen sind bisher nicht bekannt.

Background of this study is due to the absence Standard Operation Procedure (SOP) in K Pharmacy, can operational chaos. Solving these problems can be done by using the service blueprint, to find out the error process. This research not only designed, but also developed and implemented Standard Operation Procedure (SOP) and Service Blueprint. This type of research is applied research with descriptive qualitative methods. The technique of collecting data this study using purposive sampling technique. The methods collection data of this study were observation and depth interview. This research design used iterative process (exploration, creation, reflection and implementation). The result of this study are need to implement an electronic medical record system when doing assessments, implementing an inventory management system, providing a queue serial number of customers, and implementing an electronic cash register system
