Performance appraisal is an evaluation and assessment carried out to employees to find out what their abilities are and measure productivity during work. XYZ University has an employee performance appraisal which is conducted annually to determine employee promotions. The assessment process is still using the manual method and using the Graphic Rating Scale method. Graphic Rating Scale is a performance appraisal method in which an employee is assessed based on criteria that are considered important and relevant to performance and productivity. To determine a promotion, the Graphic Rating Scale method cannot do it because it takes standard values, core factors and secondary factors that are used as standards to determine promotions. Therefore, In this employee appraisal application there is a combined method used to assist the assessment process, namely the Graphic Rating Scale method and the Profile Matching method. The Profile Matching method is used because there are standard values, core factors and secondary factors that can be used as standards to determine promotions. The results of this study are on testing the Graphic Rating Scale method with the combined method of Graphic Rating Scale and Profile Matching, the combined method of Graphic Rating Scale and Profile Matching has a higher average accuracy of 78,18%, while the Graphic Rating Scale method has an average accuracy of 68,45%.

Bulan Sabit Merah Indonesia Surabaya atau yang disingkat BSMI adalah sebuah organisasi yang bergerak dibidang kemanusiaan. Sebagai organisasi yang bergerak dibidang kemanusiaan, BSMI Cabang Surabaya terus berupaya untuk meningkatkan kualitas dari relawan yang dengan salah satunya melakukan penilaian kinerja. Saat ini BSMI Surabaya masih belum memiliki sistem penilaian kinerja relawan serta kurangnya evaluasi untuk penilaian kinerja yang bisa membuat para relawan dapat melakukan kesalahan-kesalahan atau tidak menerapkan tugas sesuai diklat atau pelatihan yang telah diajarkan sebelumnya tanpa memperhatikan aturan yang telah ditetapkan. Berdasarkan masalah tersebut, maka diusulkan sebuah solusi aplikasi penilaian kinerja relawan berbasis website dengan menggunakan metode Graphic Rating Scales (GRS) yang dapat mendukung proses penilaian kinerja serta mempermudah tim manajemen dalam melakukan proses memasukan data relawan yang ingin dinilai dimanapun dan kapanpun juga terjangkau. Aplikasi dapat menemukan nilai total untuk setiap 3 kali kejadian dari relawan tersebut dan memperlihatkan grafik kinerja relawan dari tahun ke tahun untuk mengevaluasi dan melihat peningkatan atau penurunan kinerja relawan serta terdapat fitur laporan akhir berupa rangking dari seluruh relawan sesuai urutan nilai akhir yang telah diperoleh guna mempermudah dalam pengambilan keputusan serta meningkatkan kinerja dari setiap relawan.

Abstrak: Akademi Komunitas Negeri Putra Sang Fajar Blitar (AKB) merupakan institusi perguruan tinggi negeri yang menyelenggarakan pendidikan vokasi. Pendidik pada AKB terdiri dari dosen dan instruktur. Penelitian ini menjelaskan fungsi dari sistem penilaian kinerja menggunakan 360-degree feedback untuk mengetahui kinerja dari pendidik dalam berbagai aktivitas pembelajaran yang meliputi aspek persiapan, implementasi dari aktivitas pembelajaran, dan evaluasi hasil pembelajaran. Penilaian kinerja dilakukan dengan pengisian formulir penilaian kinerja oleh beberapa pihak sebagai stakeholders, yaitu: mahasiswa sebagai konsumen, koordinator program studi sebagai atasan, tenaga pendidik sebagai teman sejawan dan bagian akademik sebagai rekan kerja. Formulir penilaian kinerja terdiri dari seperangkat pertanyaan yang mana penilai dapat memberikan penilaian berdasarkan skala 1-4 (graphic rating scale), yaitu: sangat baik, baik, cukup baik, dan tidak baik. Hasil penelitian ini menunjukkan bahwa kinerja tenaga pendidik pada AKB berdasarkan kriteria waktu, kerapihan, komunikasi, proses pengajaran, persiapan materi pembelajaran dan evaluasi proses pembelajaran sangat baik, meskipun dalam beberapa aspek masih ada yang perlu ditingkatkan, seperti ketepatan waktu dalam penyerahan nilai dan ketegasan dalam menindak mahasiswa yang tidak disiplin.

Purpose: The pandemic of COVID-19 has affected many countries and medical services including assisted reproductive treatment (ART) have been hampered. The purpose of the study was to assess the preparedness of ART clinics and staff to resume services and patients reasons to initiate treatment and key performance indicators (KPIs) of ART labs during pandemic. Methods: Semi-descriptive report of two private in-vitro fertilization (IVF) clinics in Maharashtra, India, when COVID-19 testing for asymptomatic people was unavailable. Time required for replenishing laboratory supplies, and staff preparedness to function under new norms of pandemic was documented. Infection mitigation measures at workplace and triaging strategy were evaluated. KPIs of the clinics were evaluated. Results: One-third of the patients consulted through telemedicine accepted or were eligible to initiate treatment upon clinic resumption. Lack of safe transport and financial constraints prevented majority from undergoing IVF, and only 9 percent delayed treatment due to fear of pandemic. With adequate training, staff compliance to meet new demands could be achieved within a week, but supply of consumables was a major constraint. 52 cycles of IVF were performed including fresh cycles and frozen embryo transfers with satisfactory KPIs even during pandemic. Conscious sedation and analgesia during oocyte retrieval was associated with reduced procedure time and no intervention for airway maintenance, compared to general anaesthesia. Self reported pain scores by patients ranged from nil to mild on a graphic rating scale. Conclusion: This study provides a practical insight and will aid in forming guidelines for resumption of IVF services during pandemic.

Competition increasingly competitive business trigger Muliadana company to be more innovative in improving the quality of the company. One technique used by management companies in improving the quality of human resources is to conduct employee performance appraisal. To the authors try to establish a scoring system using the Graphic Rating Scale (GRS), which is a methodology for assessing the scale of the graph by comparing the individual performance against an absolute standard. The purpose of this valuation method is to minimize the less objective assessment and can determine the development of the employee's performance over several periods. The results of the performance appraisal company can provide feedback to the employees so they can decide the steps taken in making decisions such as: promotion, promotion or class, mutation, degradation as well as training for employees in need.

<p class="Abstract">Spondylolisthesis is forward slipping of upper vertebra in relations to its lower one, commonest is L4-L5. The ideal surgical treatment of spondylolisthesis is still controversial. Posterior lumber interbody fusion with pedicle fixation is one of treatment choices for the lumber spondylolisthesis. Forty patient who operated through posterior lumber interbody fusion and pedicle screw fixation by single surgeon was included from January 2012 to March 2015. Periodical follow-up was done both clinically and radiologically up to 6 months. Patient outcome was completed based on pain relief graphic rating scale and Oswestry disability index. In our series, the excellent outcome were 25 patients (62.5%), good were 12 patients (30%), and fair were 3 patients (7.5%). There were no poor outcome and no new neurological deficit. This study concludes that posterior lumber inter-body fusion with pedicle is an effective treatment for the lumber spondylolisthesis. It helps to maintain the biomechanics, associated with less complication and improve the quality of life of patient.</p>
