Aims: the assessment of individual competence in medical education is about finding a balance between having sufficient resources to make valid and reliable decisions and not using more resources than necessary. Sequential assessment, in which more resources are used for borderline performing candidates than for poorly or clearly satisfactorily performing candidates, can be used to achieve that balance. Although sequential assessment is commonly associated with larger groups of candidates to be assessed, in many practical settings numbers of candidates may be small.Objective: this article presents a single case design with a statistical model for the assessment of individual competence that can be used regardless of the number of candidates.Method: a worked example of a solution that can be used for an individual candidate, using simulated data in the zero-cost Open Source statistical program R version 4.0.5., is provided.Results: the aforementioned solution provides statistics that can be used to make pass/fail decisions at the level of the individual candidate as well as to make decisions regarding the length and timing of an exam (or parts thereof) for the individual candidate.Conclusion: the solution provided can help to reduce resources needed for assessment to a considerable extent while maximizing resources for borderline candidates. This facilitates both decision making and cost reduction in assessment.

The randomness of some irreversible quantum phenomena is a central question because irreversible phenomena break quantum coherence and thus yield an irreversible loss of information. The case of quantum jumps observed in the fluorescence of a single two-level atom illuminated by a quasi-resonant laser beam is a worked example where statistical interpretations of quantum mechanics still meet some difficulties because the basic equations are fully deterministic and unitary. In such a problem with two different time scales, the atom makes coherent optical Rabi oscillations between the two states, interrupted by random emissions (quasi-instantaneous) of photons where coherence is lost. To describe this system, we already proposed a novel approach, which is completed here. It amounts to putting a probability on the density matrix of the atom and deducing a general “kinetic Kolmogorov-like” equation for the evolution of the probability. In the simple case considered here, the probability only depends on a single variable θ describing the state of the atom, and p(θ,t) yields the statistical properties of the atom under the joint effects of coherent pumping and random emission of photons. We emphasize that p(θ,t) allows the description of all possible histories of the atom, as in Everett’s many-worlds interpretation of quantum mechanics. This yields solvable equations in the two-level atom case.

Abstract Background A new outcomes-based curriculum is soon to be implemented for UK surgical trainees. Performance will be evaluated against the standard expected of a new consultant. Accurate recording of operative experience and performance will therefore be crucial to demonstrate achievement of this standard. The current eLogbook system for recording surgical experience has many benefits including simplicity and accessibility, but may misrepresent actual experience because most operations are considered as a whole; unlike some colorectal operations, involvement in steps within many upper gastrointestinal (UGI) operations cannot be recorded. Methods Impact on training by the COVID-19 pandemic led to discussion and identification of cultural and logistical barriers to accurate recording of experience. To address these, a modification to enhance the current eLogbook system was developed by trainees and trainers at a university teaching hospital. An existing typology was used to deconstruct common UGI operations into their component steps, which can be recorded at this more detailed level.  Results The modified deconstructed logbook concept is described using a worked example, which can be applied to any operation. We also describe the integration of a component-based training discussion into the surgical team brief and debrief; this complements the deconstructed logbook by promoting a training culture. Conclusions Using the described techniques, trainees of all levels can comprehensively and accurately describe their surgical experience. Senior trainees will benefit from recording complex operations which they are not expected to complete in their entirety, whilst less experienced trainees will benefit from the ability to record their involvement in more basic parts of operations. The suggested approach will reduce misrepresentation of experience, encourage proactive planning of training opportunities, and reduce the impact of crises such as pandemics on surgical training.

Preregistration has been lauded as one of the solutions to the so-called ‘crisis of confidence’ in the social sciences and has therefore gained popularity in recent years. However, the current guidelines for preregistration have been developed primarily for studies where new data will be collected. Yet, preregistering secondary data analyses--- where new analyses are proposed for existing data---is just as important, given that researchers’ hypotheses and analyses may be biased by their prior knowledge of the data. The need for proper guidance in this area is especially desirable now that data is increasingly shared publicly. In this tutorial, we present a template specifically designed for the preregistration of secondary data analyses and provide comments and a worked example that may help with using the template effectively. Through this illustration, we show that completing such a template is feasible, helps limit researcher degrees of freedom, and may make researchers more deliberate in their data selection and analysis efforts.

During the past decades, many researchers have attempted to explore effective teaching methods for developing students’ descriptive writing performance. In this study, the worked example was implemented as an effective way of guiding students to provide step-by-step solutions to learning tasks. Moreover, a spherical video-based virtual reality (SVVR) environment was provided to place students in real-world situations which enabled them to experience the learning contexts in depth. A pretest-posttest quasi experimental study was conducted to explore the influence of the SVVR-supported worked example approach and engagement level on students’ Chinese descriptive writing performance. A total of 79 fourth-grade elementary school students participated in this study. The experimental group used SVVR with worked examples to complete Chinese writing assignments, whereas the control group used videos and worked examples. The results showed no significant effects of the SVVR-supported worked example approach compared with the conventional worked example approach regarding organization, sensory details, or creativity dimensions. As for the figurative expression dimension, students in the SVVR-supported worked example approach condition scored significantly higher. Moreover, high engagement students significantly outperformed low engagement students in all four writing performance dimensions. Additionally, a significant interaction effect between learning approach and engagement level on figurative expression was found.

Abstract Background Implementation science aims to embed evidence-based practice as ‘usual care’ using theoretical underpinnings to guide these processes. Conceptualising the complementary purpose and application of theoretical approaches through all stages of an implementation project is not well understood and is not routinely reported in implementation research, despite call for this. This paper presents the synthesis and a collective approach to application of a co-design model, a model for understanding need, theories of behaviour change with frameworks and tools to guide implementation and evaluation brought together with the Consolidated Framework for Implementation Research (CFIR). Method Using a determinant framework such as the CFIR provides a lens for understanding, influencing, and explaining the complex and multidimensional variables at play within a health service that contribute to planning for and delivering effective patient care. Complementary theories, models, frameworks, and tools support the research process by providing a theoretical and practical structure to understanding the local context and guiding successful local implementation. Results This paper provides a rationale for conceptualising the multidimensional approach for implementation using the worked example of a pregnancy, birth, postnatal and early parenting education intervention for expectant and new parents at a large maternity hospital. Conclusion This multidimensional theoretical approach provides useful, practical guidance to health service researchers and clinicians to develop project specific rationale for their theoretical approach to implementation projects.
