Background: Housewives are synonymous with household chores. These activities are carried out regularly, such as washing clothes, cooking, shopping, sweeping floors, washing dishes, cleaning the house. Housewives also carry out regular activities such as mopping, ironing, sweeping the yard, and gardening. Intensive activities and inappropriate positions performed by housewives have the potential to cause musculoskeletal complaints in the upper extremities such as pain or tingling sensation. Efforts that can be made to reduce these complaints are counseling on the application of the Joint Protection Technique (JPT). JPT aims to reduce pain and improve joint function through the use of an ergonomic approach such as changing movement patterns, modifying tasks and the environment. After participating in extension activities, it is hoped that housewives will be able to apply JPT in household activities without musculoskeletal complaints. Methods: Health education was carried out by distributing leaflets via the WhatApps group, lecturing on explaining images printed on leaflets and demonstrating the application of JPT in housekeeping activities. The counseling participants were housewives in RW 8, Kebakkalang, Kemiri Village, Kebakkramat District. The counseling activity took place at the house of one of the residents using equipment such as brooms, buckets of water, clothes, grocery bags, irons, and cooking utensils. Results: Counseling activities were carried out during March 2021 with 16 housewives as participants. Counseling activities still pay attention to health protocols, namely maintaining distance and wearing masks. Participants are able to demonstrate directly the application of the JPT principles in the activities of sweeping, washing clothes, shopping for vegetables, ironing, and cooking with real household context and equipment. Conclusion: Domestic work that is carried out by applying the JPT principle can be carried out without reducing the quality of the work, making the work stages simpler and not causing fatigue.

Occupational therapists (OTs) and physical therapists (PTs) are rehabilitation therapists with distinct but complementary roles who contribute to pain assessment and management in young people either as solo providers, or as a critical component of the interdisciplinary treatment team. Pain in infants, children, and adolescents interferes with their ability to engage in essential interactions with caregivers, acquisition of developmental milestones, and with participation in activities related to self-care, leisure, play, school, and work. OTs and PTs use specific treatment strategies, such as positioning, splinting, adaptive equipment, exercise, manual therapy, electrophysical agents, education on energy conservation, joint protection strategies, and pain self-management training to facilitate participation in valued life activities and occupations. This chapter provides an overview of important theoretical frameworks for rehabilitation therapists, reviews evidence for OT and PT interventions, and describes a framework for planning procedural pain management for rehabilitation therapists when working with pediatric clients.

Abstract Chronic tubulointerstitial nephropathy (CTIN) is one of the most common kidney diseases. However, treatment for CTIN has multiple limits. Adjuvant therapy through nutritional regulation has become a hot research topic at present. Icariin (ICA), an extraction of Chinese herbal medicine epimedium, has many pharmacological functions including anti-inflammation and tonifying kidney. Selenomethionine (SeMet) possesses the effects of antioxidant and lightening nephrotoxicity. However, little is known about the combined nephroprotection of them. This study was investigated to evaluate the joint effects of ICA and SeMet on CTIN and explore the mechanism. Based on a novel CTIN model developed in our previous study, mice were randomly divided into five groups (a: control; b: model; c: model+ICA; d: model+SeMet; e: model+ICA+SeMet). Renal tubule epithelial cells were treated with cyclosporine A and ochratoxin A without/with ICA or/and SeMet. The results showed ICA or/and SeMet ameliorated CTIN by inhibiting the uptrends of blood urine nitrogen, serum creatinine, urine protein, urine gravity, histopathological damage degree and collagen Ⅰ deposition. ICA or/and SeMet also increased cell proliferation and decreased apoptosis and the expression of TGF-β1 and α-SMA. Emphatically, ICA and SeMet joint had better nephroprotection than alone in most indexes including fibrosis. Furthermore, ICA and SeMet joint decreased the activation of toll like receptor 4 (TLR4)/NFκB pathway induced by CTIN. TLR4 overexpression counteracted the joint protection of ICA and SeMet. Therefore, ICA and SeMet in combination could protect against CTIN through blocking TLR4/NFκB pathway. The study will provide novel insights to explore an adjuvant therapeutic orientation.

ObjectivesThe objective of this survey was to investigate the barriers, facilitators, expectations and patient preferences regarding joint protection (JP) programmes in people with hand arthritis.DesignCross-sectional survey.SettingTertiary clinic.ParticipantsPatients with hand arthritis: osteoarthritis, rheumatoid arthritis, psoriatic arthritis and other forms of arthritis.Primary and secondary outcome measuresThis study used a survey among people with hand arthritis. Descriptive statistics and percentages were reported for all the data about the barriers, facilitators and preferences around JP.ResultsA total of 192 patients consented to participate. Most of the patients (82%) were unaware of JP. Factors that may act as barriers to participation and were regarded as ‘a very big concern’ were: cost of the programme (44%), time of offering the programme (39%), work commitments (36%) and having a centre/clinic close to the house (28%). Factors that may act as facilitators and rated as ‘extremely helpful’ were: research that shows that JP works (26%) and having the centre/clinic close to the house (25%). An online format for JP was the most preferred option (54%). Half (46%) preferred a timeframe of 1 hour, three times per week and 44% preferred a 2-hour programme, for three times per week.ConclusionsAwareness of the potential benefits of JP, and prior experience with JP programme were very low. Common potentially modifiable patient-reported barriers to participate in future JP interventions, included: cost, work commitments, distance from home to clinic and times that the intervention were provided. These barriers might be addressed with free and accessible forms of delivery of JP, which may lead to better uptake and participation in JP programmes.

Abstract Aim Base of thumb osteoarthritis (BTOA) is a common age-related disease which has a significant negative impact upon quality of life, while little is known about structure and pathways of interface services. Our aim was to assess disease burden, referral pathways, service structure and management pathways in UK interface services. Patients & Methods A structured questionnaire was carried out with a participating clinician at each centre to detail the local guidelines and management of BTOA. Five patients referred with BTOA were prospectively identified in each of 32 United Kingdom (UK) interface centres. Results Most centres (72%) had a local guideline and a standardised treatment regime consisting of education (100%), joint protection (100%), range of motion exercises (84%), strengthening exercises (88%), splintage (100%) and use of assistive devices (78%). No centre routinely offered a steroid injection at the first appointment and no centre had a specific threshold for offering an injection. Injection delivery was variable. Most patients had not been referred previously (82%). Most patients used analgesia (72%) but a minority of patients had been treated with a splint (46%), therapy (43%) and steroid injection (27%) prior to their latest attendance. Conclusion Most BTOA patients newly referred to interface services have been treated with analgesics and have not received comprehensive multimodal intervention. The management of BTOA at interface services is standardised in terms of education, splintage and therapy. However, there is a lack of standardisation in terms of both the threshold for, timing and mode of delivery of injection therapy.

New architectures or the protection of the fortifications in TarragonaThe conservation of the fortification in Tarragona was not an easy task. In 1884 the Roman wall was declared a National Monument, and in 1966 it was declared a historical and artistic complex. These measures distant in time did not achieve a joint protection of it. This study deals with the transformation of the bastion of Cadenas in the San Antonio Promenade. It is an example of urban speculation during Franco’s developmentalism. Our contribution is the result of an analysis and comparison of various types of documentation: military, engineering command headquarters as well as municipal and state. We demonstrate that the destruction of the bastions was due to the low value given to these defensive structures, which were considered an obstacle to urban expansion and development.
