Охрана репродуктивного здоровья предполагает множество направлений деятельности, в том числе мероприятия, направленные на коррекцию пищевого статуса мужчин и женщин репродуктивного возраста, особенно беременных и кормящих женщин. Объектом исследований являлся паштет печеночный профилактической и лечебной направленности с включением пищевой системы на основе альбумина пищевого, жмыха зародышей пшеницы и фасоли. Исследованы органолептические, функционально-технологические, реологические показатели качества и безопасности, микроструктура паштета, обоснованы параметры и режимы подготовки пищевой системы и готового изделия с ее применением. Установлено, что массовая доля таких нутриентов, как Fe, Zn, Na, P и водорастворимых витаминов (В и В), находилась на уровне 20 % и более от суточной потребности. Отмечено увеличение содержания в диапазоне от 20 до 50 % суточной потребности фолиевой кислоты, витаминов В, В, С и D, что позволяет отнести разработанный паштет к функциональному продукту. При этом содержание некоторых компонентов (Mn, Cu, витаминов В, В) превысило уровень 50 % суточной потребности, благодаря чему паштет также можно отнести к витаминизированным продуктам. Выявлено улучшение сбалансированности состава полиненасыщенных жирных кислот, в разработанном продукте соотношение омега-6 к омега-3 кислот соответствовало рекомендациям НИИ питания РАМН (5-10:1) и составляло соответственно (омега-6 : омега-3) 7:1. Биологическая ценность спроектированного паштета находится на высоком уровне и достигает значения 69,8 %, показатель КРАС 30,2 %, что свидетельствует о незначительном количестве избыточных аминокислот в белковой фракции продукта. The protection of reproductive health involves many areas of activity, including measures aimed at correcting the nutritional status of men and women of reproductive age, especially pregnant and lactating women. The object of research was liver pate of preventive and therapeutic orientation with the inclusion of a food system based on food albumin, wheat germ cake and beans. Organoleptic, functional-technological, rheological, quality and safety indicators, the microstructure of the pate are studied, the parameters and modes of preparation of the food system and the finished product with its use are justified. It was found that the mass fraction of nutrients such as iron, zinc, H, P and water-soluble vitamins (B and B) was at the level of 20 % or more of the daily requirement. There was an increase in the content of folic acid, vitamins B, B, C and D in the range from 20 to 50% of the daily requirement, which allows us to attribute the developed pate to a functional product. At the same time, the content of some components (manganese, copper, vitamins B, B) exceeded the level of 50 % of the daily requirement, so that the pate can also be attributed to fortified products. An improvement in the balance of the composition of polyunsaturated fatty acids was revealed, the ratio of omega-6 to omega-3 acids in the developed product corresponded to the recommendations of the Research Institute of Nutrition of the Russian Academy of Medical Sciences (5-10:1) and was respectively (omega-6 : omega-3) 7:1. The biological value of the designed pate is at a high level and reaches a value of 69.8 %, the RED indicator (30.2 %), which indicates an insignificant amount of excess amino acids in the protein fraction of the product.

The iliopsoas muscle abscess is rare condition, with increasing recognition, that can be either primary, by hematological or lymphatic dissemination, or secondary to infections in the gastrointestinal, genitourinary tract or musculoskeletal system. Its early identification is highlighted, for an adequate therapeutic orientation, contributing to a good prognosis.

Abstract Background Glioblastoma multiform with primitive neuronal component (GBM-PNC) has been recently defined as a rare variant of glioblastoma multiform (GBM), which shows characteristically pathological pattern of less differentiated areas with small blue cell morphology and neuroectodermic immunophenotype. New studies emphasize its characteristics and differences, which have become vitally important due to the changes in therapeutic management. Case presentation We present the case of 57-year-old male patient who onset symptoms were secondarily widespread partial seizures and expression aphasia. Brain magnetic resonance imaging (MRI) reported left enhanced temporal infiltrating lesion, requiring surgery twice throughout two years. At first surgery, pathological samples revealed embryonic tumor of the central nervous system (grade IV, WHO 2016), so PACKER protocol consisting of CSRT (craniospinal radiation) plus weekly vincristine followed by 8 cycles of cisplatin, lomustine and vincristine usually used for medulloblastomas or other primitive neuroectodermal tumors was started. However, due to reappearance of symptoms and progression in MRI, reoperation was performed with definitive diagnosis of GBM-PNC (Grade IV, WHO 2016) and switched to STUPP protocol. Conclusions It is important to take into account the chance of this entity when histological, radiological and intraoperative findings orient toward a primitive neural tumor since the presence of GBM could be overlooked leading to mistakes in diagnosis and the therapeutic orientation.

This chapter explores how gendered social institutions shape the custody process and experiences of noncustodial mothers. It elaborates that work and employment, psychotherapy, and the law are discursively gender-neutral social institutions that rest on and reproduce existing gender hierarchies that shape a noncustodial mothers' experiences. It also explains what gendered understandings and assumptions about work and employment or home inform mothers' understandings of child custody and custody-determination processes. The chapter describes the role that the therapeutic orientation of the custody process plays in making mothers lose custody. It demonstrates the manner in which gender matters in child custody as the concept of parenting is deeply gendered, regardless of the actual genders of parents.

Abstract Chronic tubulointerstitial nephropathy (CTIN) is one of the most common kidney diseases. However, treatment for CTIN has multiple limits. Adjuvant therapy through nutritional regulation has become a hot research topic at present. Icariin (ICA), an extraction of Chinese herbal medicine epimedium, has many pharmacological functions including anti-inflammation and tonifying kidney. Selenomethionine (SeMet) possesses the effects of antioxidant and lightening nephrotoxicity. However, little is known about the combined nephroprotection of them. This study was investigated to evaluate the joint effects of ICA and SeMet on CTIN and explore the mechanism. Based on a novel CTIN model developed in our previous study, mice were randomly divided into five groups (a: control; b: model; c: model+ICA; d: model+SeMet; e: model+ICA+SeMet). Renal tubule epithelial cells were treated with cyclosporine A and ochratoxin A without/with ICA or/and SeMet. The results showed ICA or/and SeMet ameliorated CTIN by inhibiting the uptrends of blood urine nitrogen, serum creatinine, urine protein, urine gravity, histopathological damage degree and collagen Ⅰ deposition. ICA or/and SeMet also increased cell proliferation and decreased apoptosis and the expression of TGF-β1 and α-SMA. Emphatically, ICA and SeMet joint had better nephroprotection than alone in most indexes including fibrosis. Furthermore, ICA and SeMet joint decreased the activation of toll like receptor 4 (TLR4)/NFκB pathway induced by CTIN. TLR4 overexpression counteracted the joint protection of ICA and SeMet. Therefore, ICA and SeMet in combination could protect against CTIN through blocking TLR4/NFκB pathway. The study will provide novel insights to explore an adjuvant therapeutic orientation.

The subject of the article are the therapeutic competence of the teacher. The subject of the article is the therapeutic competence of the teacher. It is based on the therapeutic orientation in humanistic psychology. These competences allow the teacher to see in time the difficulties and abilities of a child, recognize their individual psychophysical possibilities as well as development and educational needs. The concept of therapeutic approach in teaching is aimed at taking actions favouring the development of students’ potential in order to increase the effectiveness of learning and improve their functioning. The article emphasizes the teacher’s skills particularly valuable in the area of assistance and support for student development.

Therapeutic Relationship has been regarded as a profound element of the psychotherapeutic procedure and crucial for the outcome of therapy. Research evidence so far claims that both therapist’s and client’s personality can impact the Therapeutic Relationship. Based on the well-researched Attachment Theory there has been some research mostly focused on how clients’ attachment patterns can affect the Alliance. Limited research, though, on how therapists experience the Relationship in relation with their Attachment Styles do exist. The current study investigates trainee therapists’ experience of the Therapeutic Relationship in the light of their Attachment Style. A qualitative approach was used for this research to investigate in depth the experience of five trainee Integrative psychotherapists who were recruited to take part in semi-structured interviews. Interviews were transcribed and verbatim was divided into superordinate and subordinate themes and analyzed by Interpretative Phenomenological Analysis method. Four master themes were identified. Findings were then compared and discussed according to existing literature. All participants referred to the relevance of Attachment Theory in psychotherapy. They also agreed on the activation of their Attachment Styles during the therapeutic process. The securely attached therapist confirms previous studies on her capability to offer a secure base for her clients. On the other hand therapists with insecure Attachment Styles appeared sensitive in their collaboration mostly with insecure clients. Anxiously attached therapists proved to experience difficulty in the initial stages of the Therapeutic Relationship while the combination of avoidant therapist-avoidant client appeared to be the most demanding one, regarding the concept of trust and relationship ruptures. We suggest that the exploration of trainee therapists’ Attachment Styles as well as the study of the Attachment Theory should be added to all Counseling and Psychotherapy curricula regardless the therapeutic orientation because of its contribution to forming positive Therapeutic Relationships.

The COVID-19 pandemic has dramatically affected the mental health field. The unique distress caused by the pandemic along with the need for many to transition their therapy practices to TeleHealth/Telepsychology modalities caught many therapists, including existential–humanistic therapists, unprepared to make the necessary adjustments to treat clients in a manner that is safe and effective. Existential–humanistic therapy often is neglected with trauma and disaster relief work, despite its relevancy and important contributions. All therapies must adjust in crisis situations, particularly when adjustments to how services are offered are disrupted; however, the impact of these changes varies by therapeutic orientation. As an approach that values genuineness, empathy, and presence, existential–humanistic therapy faces unique challenges when employed with disaster response. However, when these challenges are faced, existential–humanistic therapy has much to offer disaster response, including consideration of existential guilt, existential shattering, existential anxiety, and existential perspectives on self-care.

Abstract Background Contrasting historical views represent the asylum as a manifestation of humanitarian and therapeutic progress or as an institution of social control designed to bolster the capitalist economic order. More extreme critics suggest it was used to incarcerate people exhibiting only political or social deviance. Methods Case notes of 200 consecutive male and female admissions to the Essex County Asylum in 1904 were inspected. The nature of presentations was classified in contemporary terms into broad categories of disorder. Outcomes were identified and differences between men and women were explored. Results We found no evidence that patients were admitted without signs of significant mental and behavioural disturbance. In total, 44% of admissions had signs of an organic condition, and these were more frequent among men. Women were admitted at a faster rate and were 1.6 times more likely to have mania or a psychotic disorder. Overall, 45.5% of patients were discharged, with 62% of patients with non-organic disorders discharged recovered or improved. Conclusions Evidence partially supports both views of the asylum. In line with other studies, there is no evidence that the asylum was used to incarcerate people who did not show significant signs of disorder, but it did provide care and containment for those who could not be accommodated elsewhere, including many with organic conditions. The asylum also had a therapeutic orientation, however, and encouraged discharge where possible. In contrast to some other studies, women were more likely to be institutionalised than men, possibly reflecting their greater economic dependency.
