CPX-351 Induces Remission in Newly Diagnosed Pediatric Secondary Myeloid Malignancies

Secondary myelodysplastic syndromes and acute myeloid leukemia (sMDS/AML) are rare in children/adolescents and have a dismal prognosis. The mainstay therapy is hematopoietic cell transplantation (HCT) but there has been no innovation in cytoreductive regimens. CPX-351, a fixed 5:1 molar ratio of liposomal cytarabine/daunorubicin, has shown favorable safety and efficacy in elderly individuals with sAML and children with relapsed de novo AML. We report the outcomes of seven young patients (six with newly diagnosed sMDS/AML and one with primary MDS/AML) uniformly treated with CPX-351. Five patients had previously received chemotherapy for osteosarcoma, Ewing sarcoma, neuroblastoma, or T-ALL; one had predisposing genomic instability disorder (Cornelia de Lange); and one MDS-related AML and multi-organ failure. The median age at diagnosis of myeloid malignancy was 17 (13-23) years. Patients received 1-3 cycles of CPX-351 (cytarabine 100mg/m2 plus daunorubicin 44mg/m2) on days 1, 3, and 5, resulting in complete morphologic remission without overt toxicity or treatment-related mortality. This approach allowed for adding FLT3 inhibitor as individualized therapy in one patient. Six patients were alive and leukemia-free at 0.5-3.3 years after HCT. One patient died from disease progression before HCT. Concluding, CPX-351 is an effective and well-tolerated regimen for cytoreduction in pediatric sMDS/AML warranting prospective studies.

The purpose of this study was directed to examine the quality of service for determining less paper-based automatic pensions for Civil Servants in the North Sulawesi Provincial Government. This research is descriptive qualitative with data collection techniques in the form of interviews, observations and documentation. Implementation of the determination of less paper-based automatic pensions for Civil Servants in the North Sulawesi Provincial Government has not gone well, due to the absence of Standard Operating Procedures (SOP) for determining less paper-based automatic pensions, lack of transparency, weak socialization, weak data reconciliation, lack of human resources. The apparatus, there is no innovation in the development of less paper-based pension processing, the condition of civil servants who will enter the retirement age limit has not responded to the implementation of the less paper-based automatic pension determination. Therefore, it takes a strong commitment and desire from the North Sulawesi Provincial Government, especially the Regional Personnel Board of North Sulawesi Province in making innovations in developing less paper-based automatic pension determinations to create automatic pension service flows that meet service quality standards, making it easier for pension recipients to receive pension decisions.

Perusahaan tahu di Indonesia pada umumnya, masih menggunakan cara manual, dalam pembuatannya khususnya pada proses penyaringan bubur kedelai yang membutuhkan waktu yang, lama. Hal tersebut didasarkan pada belum ditemukannya mesin pemeras bubur kedelai. Dewasa ini telah ditemukan inovasi mesin, pemeras bubur kedelai yang bisa meningkatkan kuantitas dan, kualitas produksi dibanding dengan cara manual. Penerapan, teknologinya adalah bubur kedelai diletakkan pada tabung, penyaring kemudian tabung tersebut diputar menggunakan motor, yang dihubungkan melalui fanbelt dan pulley. Ketika motor, diputar, tabung akan ikut berputar sehingga menimbulkan gerak, sentrifugal dimana air kedelai akan terpisah dari ampas. Kecepatan putar motor yang dikontrol adalah 750 rpm dengan, nilai Kp 0,108 , Ki 0,83 sehingga mendapatkan air sari kedelai, sebesar 1,3 liter dengan perbandingan 1 kg kedelai : 1 liter air. Parameter dari penerapan metode PI ini meliputi rise time 4 detik, settling time 4,5 detik, overshoot 0 dan error steady state 2,4%. Dengan penerapan metode PI maka hasil perasan kedelai dari, peyaringan menjadi semakin banyak dan waktu yang dibutuhkan 4 menit lebih singkat dibandingkan dengan cara konvensional. When the motor is rotated, the tube will rotate, causing centrifugal motion where the soybean water will separate from the pulp. The speed of the motor controlled in 750 rpm with the Kp 0,108, Ki 0,83, to get soybean essence up to 1,3 liter within comparison 1 kg soybean : 1 liter water. The parameter PI method including rise time 4 second, settling time 4,5 second, overshoot 0 and error steady state 2,4%. By applying PI method, the result of filtering is 4 minute faster comparing with conventional method.

Responses to the Problem of OD Stagnation: A Tribute to Warner Burke

As our most qualified inspector, sage, and futurist for organizational development (OD), Warner has set the standard for what great organizational change and development should be. He has recently accused the field of being stagnant and in need of new approaches to organizational change. In his view, there has been no innovation in OD since appreciative inquiry was introduced in 1987. This article takes seriously Warner’s challenge and discusses positive organizational scholarship (POS) as a step forward in innovative OD scholarship. POS highlights some phenomena that have largely been overlooked in mainline OD articles, and the article provides illustrations of successful POS interventions relying on a POS–OD approach to change. A sampling of new OD-type intervention practices is introduced.

Training on the Utilization of Citrus Fruits in Making Dodol to Improve the Welfare of Citrus Farmers in Lau Riman Village, Tanah Karo Regency

Citrus fruit is one of the typical Indonesian fruits that is cheap and easy to obtain, as well as one of the daily dessert fruit of the community. One of the Citrus fruit producer locations in North Sumatra is Tanah Karo Regency, where the Citrus orchards are the main livelihood of the community. Citrus Fruit itself was developed in the era of 1990 in Tanah Karo which is able to eradicate poverty and improve the education of Tanah Karo youths. Lau Riman Village, Tiga Panah Sub-district, within +/- 17 km from Kabanjahe, is a producer of Citrus orchards in besides the Merek and Munthe areas. The Citrus orchard in Lau Riman Village surrounds the village with a total of approximately eighty eight Family Heads. Siamese honey citrus or Karo itself is known as a famous sweet citrus, has a sweet smell or aroma, rough citrus flesh, rind that is easy to peel and the appearance of the fruit is quite attractive. The purpose of Community Partnership Program activities is to assist partners in improving the skills and innovation of citrus to packaging technology so that it is expected to create economic independence and increase business income. Based on observations of partners, there are several problems encountered, including: 1) There is no innovation in citrus crop yields, 2) Conventional marketing that is only awaiting the arrival of buyers, 3) Skills and limited knowledge of the utilization of citrus crops. Specific targets in the Community Partnership Program activities that result; 1) Fruit Dodol innovation 2) Marketing Strategy 4) scientific articles published in ISSN journals. The method used to achieve the Community Partnership Program activity goals is the method of education, production training, and packaging and marketing assistance. This activity was carried out for citrus fruit farmers in Lau Riman Village, Tiga Panah Sub-district, Tanah Karo Regency, which is approximately 75 km from the University of Medan Area. The choice of location is because in the village there are communities that depend on the economic life of citrus farming and there has been no innovation to citrus fruits other than just planting, harvesting, and selling fresh citrus fruits.

Penile Rings: No Innovation without Evaluation

Penile strangulation is a rare clinical entity which if left untreated can cause serious urogenital problems. We present the case of 45-year-old male patient who presented with penile strangulation after applying a metallic ring. Strangulation of the external male genitalia is a serious problem that requires urgent intervention in order to avoid serious complications. If left untreated, it can result in gangrene and penile amputation. Management of penile strangulation is a real challenge for the treating surgeon and should be treated as an emergency in order to avoid vascular compromise.
