TÓM TẮT Đặt vấn đề: Tổn thương phần mềm là loại tổn thương da, tổ chức dưới da, cân, cơ... do nhiều nguyên nhân khác nhau như loét tỳ đè, bỏng, viêm da cơ địa, viêm da tiếp xúc, Zona, viêm da bọng nước,… xảy ra trên bệnh nhân nhiễm Covid-19 nặng, các tổn thương này có thể có từ trước hoặc sau khi bị nhiễm COVID-19. Chúng tôi thực hiện đề tài này nằm mục tiêu (1) nhận định đặc điểm chung của bệnh nhân nhiễm COVID-19 nặng có tổn thương phần mềm, (2) bước đầu nhận định tình trạng có tổn thương phần mềm xảy ra ở bệnh nhân nhiễm Covid-19 nặng và (3) tìm hiểu mối tương quan giữa quá trình chăm sóc vết thương và thời gian lành vết thương của bệnh nhân. Đối tượng và phương pháp: 25 bệnh nhân có tổn thương phần mềm trên tổng số 1.094 bệnh nhân nhiễm COVID-19 nặng, trung tâm hồi sức tích cực người bệnh COVID-19 bệnh viện Trung ương Huế tại thành phố Hồ Chí Minh từ 15.09 - 01.11 năm 2021 (khảo sát nhanh trong 45 ngày). Kết quả: Khảo sát nhanh trong 45 ngày trên 1.094 bệnh nhân bị COVID-19 nặng đang điều trị tại trung tâm hồi sức tích cực Người bệnh COVID-19 bệnh viện Trung ương Huế tại thành phố Hồ Chí Minh có 25 trường hợp bị tổn thương phần mềm: Hầu hết những bênh nhân này đều có bệnh lý nền nặng kèm theo, nam gặp ít hơn nữ (44%/56%), nam ở lứa tuổi 30 - < 50 tuổi chiếm cao nhất, nữ từ 50 tuổi trở lên là chủ yếu chiếm 92,86%. Đa số bệnh mắc phải lớn tuổi, trên 50 tuổi. Loét tỳ đè chiếm đa số 76% (19/25 trường hợp) so với các thương tổn khác, trong khi đó bệnh nhân bị tổn thương trong quá trình điều trị tại trung tâm chiếm 36,8% (7/19 trường hợp) còn phát hiện trước lúc vào viện là 63,2% (12/19 trường hợp). Tỷ lệ thương tổn phần mềm trên tổng số bệnh nhân điều trị tại thời điểm khảo sát chiếm 2,29% (25/1.094 bệnh nhân), do LDTĐ chiếm 0,64% bị tại trung tâm (7/1.094 bệnh nhân). Điều trị nội khoa chiếm đa số 76% nhiều hơn so với phẫu thuật, tỷ lệ phẫu thuật chiếm 24% (6/25 trường hợp). Thời gian điều trị liền vết thương trước 14 ngày chiếm đa số 56%. Kết luận: Quá trình chăm sóc và điều trị bệnh có mối tương quan với quá trình lành vết thương và dự phòng các thương tổn phần mềm. Chiếu tia Plasma lạnh góp phần trong quá trình liền vết thương. Chăm sóc và điều trị các tổn thương phần mềm có hiệu quả, tái tạo biểu bì mô nhanh chóng, tỷ lệ thương tổn thấp, góp phần giúp bệnh nhân sớm hồi phục, giảm tỷ lệ tử vong. ABSTRACT SURVEY OF COVID-19 PATIENTS HAVINGSOFT TISSUE DAMAGE AT THE COVID-19 INTENSIVE CARE CENTER OF HUE CENTRAL HOSPITAL IN HO CHI MINH CITY Backgrounds: Soft tissue damage is damage to the skin, subcutaneous tissue, scales, muscles caused by various causes such as pressure ulcers, burns, atopic dermatitis, contact dermatitis, shingles, bullous dermatitis occurring in patients with severe COVID-19 infection. These lesions may be present before or after a COVID-19 infection. This study aims to (1) identify the general characteristics of severe COVID-19 patients with soft tissue damage, (2) assess the status of soft tissue damage in patients with severe COVID-19 infection, and (3) find out the correlation between the wound care process and the healing time of the wound. Methods: Twenty - five patients have soft tissue lesions in a total of 1.094 patients with severe COVID-19 infection at COVID-19 Intensive Care Center operated by Hue Central Hospital in Ho Chi Minh city from September 15 to November 1, 2021 (quick survey in 45 days). Results: A quick 45 - day survey of 1.094 severe COVID-19 patients showed that there were 25 cases with soft tissue damage: most of these patients have severe underlying medical conditions, men less than women (44%/56%, men aged 30 - < 50 years old account for the highest number, women aged 50 years and older accounted for the majority of cases ( 92.86%) Most of the patients are over 50 years old. Pressure ulcers (PU) accounted for the majority of cases at 76% (19/25 cases) compared with other lesions, in which patients injured during treatment at the center accounted for 36.8% (7/19 cases). Meanwhile, PU detected before hospital admission was 63.2% (12/19 cases). The rate of soft tissue lesions on the total number of patients treated at the survey time accounted for 2.29% (25/1.094 patients), due to pressure ulcers accounting for 0.64% at the center (7/1.094 patients). Internal Medicine treatment accounted for most cases (76%), more than surgery treatment, 24% (6/25 cases). The time of wound healing before 14 days accounted for most cases at 56%. Conclusion: The care and treatment process correlates to the healing process and the prevention of soft tissue injuries. Adequate care and treatment of soft tissue injuries help to quickly heal epidermal tissue, reduce injury rates, lead to early recovery and low mortality. Abstract Scoring systems have been implemented in veterinary practice to indicate the severity of pathologies, such as lameness and gastric ulceration. The need for a scoring system of equine dentition in relation to digestive health has been identified. A scoring system would allow veterinarians, owners, and researchers to more accurately assess dental health and the resulting impact it may have on chewing ability. A proposed system, the Equine Dental Scoring System (EDSS), was developed via collaboration of a team of veterinarians and equine nutritionists familiar with equine dental abnormalities. The EDSS was designed to assign higher scores corresponding to increasing severity of dental abnormalities that would impede proper chewing. The proposed scoring system ranges from 0 to 5 as follows: 0) no sharp enamel points, soft tissue damage, or malocclusion, (1) sharp enamel points, but no soft tissue damage or malocclusion, (2) sharp enamel points and soft tissue damage, but no malocclusion, (3) mild malocclusion with all aligned teeth meeting level (ex. ramps, hooks), (4) moderate malocclusion with all teeth meeting but not level (ex. wave, smile, diagonal, frown), (5) major malocclusion with one or more teeth not meeting or inhibited temporomandibular joint movement (ex. step, shear, retained cap), and/or infection, and/or pain while chewing. The EDSS was validated by assessing agreement via the Cohen’s kappa statistic between four trained professionals scoring ten images of horse dentition. Both the weighted (к = 0.62) and unweighted (к = 0.73) kappa statistics indicated substantial agreement between scorers, signifying reliable repeatability of the EDSS. Presenting dental health in the form of a score would indicate severity of dental pathologies and allow for quantitative and statistical evaluation of dental health in nutrition research and veterinary medicine.

Aim. To explore the mechanism of resveratrol in reducing the soft tissue damage of osteoarthritis (OA) based on network pharmacology. Methods. Pharmmapper was used to predict the target of resveratrol, OMIM and Genecards were used to collect OA-related disease genes, and David ver 6.8 was used for enrichment analysis. Then, animal experiments were carried out for verification. The rat OA model was established and the rats were randomly divided into 4 groups: model group, resveratrol low-dose group, resveratrol high-dose group, and blank control group for follow-up experiments. Hematoxylin-eosin (HE) staining was used to detect the degree of pathological damage of rat bones and joints. Enzyme-linked immunosorbent assay (ELISA) was used for the content of inflammatory factors. Western blot was used to detect the expression of Toll-like receptor 4 (TLR4), Myeloid differentiation factor 88 (MyD88), nuclear factor kappa B protein (NF-κB), cysteine protease-9 (CASP-9), Bcl-2 protein, and Bax protein. Results. Through network pharmacological analysis, this study found that resveratrol may regulate the TLR4 signaling pathway, PI3K-Akt signaling pathway, FoxO signaling pathway, Osteoclast differentiation, Rheumatoid arthritis, etc. Animal experiments showed that compared with the model group, the pathological damage of bone and joint in the resveratrol low-dose and high-dose groups was significantly improved. Compared with the model group, the serum levels of IL-1beta, IL-6, IL-17, TNF-α, and MCP-1 in the resveratrol low-dose and high-dose groups were significantly reduced ( P  < 0.05); protein levels of TLR-4, MyD88, and NF-κB p65 were significantly reduced ( P  < 0.05); caspase-9 and Bax protein levels were significantly reduced ( P  < 0.05), and Bcl-2 was significantly increased ( P  < 0.05). Conclusion. Resveratrol may inhibit the activation of the TLR4-mediated NF-κB signaling pathway and has a repairing effect on soft tissue damage in OA.

Background: Significant articular depression, separation of both condyles, diaphyseal comminution and dissociation, and loss of soft-tissue envelope integrity are all associated with high-energy proximal tibia fractures (Schatzker VI). Over the past 50 years, there has been a lot of research on plating problems in these complicated fractures. For the care of these complex injuries, Ilizarov devised a new method (ring fixator). Aim of the Study: To examine the outcomes of patients who received a ring fixator for the treatment of high-energy proximal tibia fractures (Schatzker VI). Materials and Methods: Fourteen patients (mean age 36) were treated with the Ilizarov fixator and transfixion wires for high-energy fractures of the proximal tibia (Schatzker VI). Nine of the patients had open fractures, and five of them had significant soft tissue damage. They were all tracked for an average of 19.4 months. The result was analyzed using the criteria set by Honkonen & Jarvinen (1992). Results: Thirteen fractures healed in an average of 14.6 weeks, with one taking six months. Twelve patients recovered complete extension, while eight others regained more than 110 degrees of flexion. All of the patients knees were stable, except one who had a minor varus deformity. Nine patients walked normally, while four had a little limp. Except for one, all of the knees exhibited an articular step-off of less than 4 mm and normal axial alignment. Six knees were found to be outstanding, five to be decent, and three to be fair. There were no instances of postoperative skin infection or septic arthritis, however, three patients did have a pin tract infection that was effectively managed. Conclusion: The technique is suitable for the treatment of complex proximal tibia fractures when there is substantial comminution at the fracture site as well as soft tissue damage (Schatzker VI).

A cohort of patients presented to Queen Victoria Hospital, UK, with iatrogenic toe ischaemia following application of a different, newly available post-procedure dressing with different properties to those usually used. This resulted in ischaemia with extensive skin and soft tissue damage, requiring debridement surgery and, in some cases, skin grafting. We aim to highlight the risk of morbidity from dressing application to the digits. This is a key learning skill for anyone who may either perform dressings or evaluate dressings on digits in the community and across multiple specialties in hospital. This article follows a thorough root cause analysis and addresses other possible causes of an acutely painful erythematous toe post-Zadek's procedure.

Study Design: Retrospective cohort study. Objective: To develop a grading method for cervical paraspinal soft tissue damage after cervical spinal cord injury (CSCI) without major fracture based on the short T1 inversion recovery (STIR) mid-sagittal magnetic resonance image (MRI) for prediction of neurological improvements. Methods: This study included 34 patients with CSCI without major fracture, treated conservatively for at least 1 year and graded using the STIR-MRI Grade. This system consists of anterior grades; A0: no high-intensity area (HIA), A1: linear HIA, and A2: fusiform HIA, and posterior grades; P0: no HIA, P1: HIA not exceeding the nuchal ligament, and P2: HIA exceeding the nuchal ligament, within 24 hours postinjury. The American Spinal Injury Association impairment scale (AIS) and the Japanese Orthopedic Association (JOA) scores were examined. Results: Anterior grades were not significantly correlated with the AIS and JOA score. At both injury and final follow-up, the AIS in P2 patients was significantly more severe ( P = 0.007, P = 0.015, respectively) than that in P0 patients. At the injury, the AIS in P2 patients was significantly more severe ( P = 0.008) than that in P1 patients. Among P2 patients only, the JOA score at the injury (1.4 points) did not improve by the final follow-up (3.9 points). The final follow-up JOA score (3.9 points) in P2 patients was significantly lower than that (13.6 points) in P0 patients ( P = 0.016). Conclusions: Grade P2 led to poor neurological outcomes. The STIR-MRI Grade is a prognostic indicator for neurological improvements past-CSCI.

Abstract Background: Complications in treating comminuted tibial plateau fractures with extensive soft tissue damage are common. In this study, we discussed the effect of staged treatment in the reconstruction of the lateral column with autogenous iliac bone using case follow-up. Methods: A retrospective analysis of 18 patients with comminuted tibial plateau fractures and extensive soft tissue damage from October 2016 to February 2020 who underwent staged treatment in our hospital. After the soft tissue damage repair, a large autologous iliac bone was used to reconstruct the lateral column of the tibial plateau.Results: All 18 patients were followed up over the course of 8-40 months, with an average of 23.44 months. All patients reported zero postoperative infection, skin and soft tissue necrosis, and loosening/breakage of the internal fixation. Regular postoperative review revealed that all patients achieved complete bony union, as evidenced by X-ray film, at 6 months after operation. Knee function (HSS) scores averaged 86.72 points at 1 year after operation; 15 people with 85 points or more, 2 people with 70-84 points, and one with 68 points. The excellent rate, according to HSS was 90%.Conclusions: Complex tibial plateau fractures with extensive soft tissue damage can be effectively remedied with a staged treatment, wherein a lateral column reconstruction is performed using a large iliac bone.
