An 88-year-old right-handed man was admitted to our hospital for cognitive impairment and right-sided paralysis. His head non-contrast computed tomography (CT) showed large low-density areas (LDA) and fibrous structures in the left occipital and temporal lobe regions. Despite the fact that it had been more than 10 years since his stroke, rehabilitation was effective. This is a rare case in which cerebellar culmen -substantia nigra tract assisted rehabilitation after stroke.

Cysts found in splenic stores, of parasitic or non-parasitic origin, are rare causes of recurrent abdominal pain, but with a significant increase in the number of diagnoses in recent years. This condition is caused by the derivation of the mesothelial cell lining of the spleen capsule, occurring in 0.5 - 2% of the world population. Clinically, patients tend to remain asymptomatic for years and usually have an incidental diagnosis by imaging exams or during the investigation of diffuse and recurrent abdominal pain, rarely found in females. The treatment of splenic cysts is performed through video laparoscopic splenic resection.

The authors present a modified endaural incision with a caudal extension for surgical access to the mandibular condyle. The approach combined the advantages of good scar cosmesis and sufficient access to the surgical site for optimal placement of the osteosynthesis with lesser risk of nerve damage.

Inflammatory myofibroblastic tumor (IMT), a benign and rare neoplasm, of mesenchymal origin, pseudoarcomatous, is responsible for about 0.7% of all neoplastic presentations in the pulmonary region, being the most frequent cause of pulmonary nodules in pediatric patients, approximately 50% of the presentations. Thus, the present study aims to report a rare case of IMT in the lung site in a 31-year-old patient undergoing treatment for breast cancer, according to a literary review, relating the data according to the clinical presentation. Due to scarce documentation in the medical literature, no cases were found similar to the one described in relation to the reported oncological primary condition of the patient, making the present case a clinical challenge in modern medical practice.

Bisphosphonates are widely used, whether in osteoporosis or malignant bone disorders. Therefore, it is necessary that there is sufficient knowledge about the adverse effects of this drug, especially in relation to Osteonecrosis of the Jaws Developed by Bisphosphonates, which is a pathology generated in the gnathic bones. So, the following question was asked in the research problem: What is the current situation regarding the use of bisphosphonates and the onset of osteonecrosis of the jaw? Searches were carried out on digital library websites in search of articles and research in books by renowned authors. Inclusion and exclusion criteria for articles found were defined. The verification of each article was carried out so that it could have perspective and follow-up on the subject discussed. The following databases were used: PUBMED, BIREME and LILACS, and SCIELO between the years 2007 to 2019. It was concluded then that Bisphosphonates play a very important role as a cause of osteonecrosis of the jaws, where the prognosis will be worse if the patient is treated with intravenous rather than oral types.

We herein report two unusual cases of osteochondritis dissecans (OCD) of the trochlea. The injury consisted of a focal alteration of subchondral bone secondary to repetitive micro trauma from overhead maneuvers (pitching) in two male adolescents. Only 23 other cases of OCD affecting the trochlea have been reported. We comment on this discrepancy, explore the radiographic features, and detail the non-operative management that was undertaken. We substantiate our report with a systematic review of all OCD cases of the trochlea and provide information on patient demographics, sport type, management, recuperation and finally significance of radiographic grading.

A dor causada pela inserção da agulha durante a anestesia local está ligada à ansiedade do paciente no momento do atendimento. Sendo assim, os anestésicos tópicos foram criados com o objetivo de diminuir a sensação dolorosa durante esse procedimento. O presente estudo teve como objetivo principal avaliar a utilização da anestesia tópica por cirurgiões dentistas, antes da administração da anestesia local. Avaliou-se o uso de anestésicos tópicos através de uma coleta de dados por meio de formulário eletrônico, respondido por 53 Cirurgiões Dentistas, professores de uma Universidade de rede particular do estado do Ceará. Por meio do estudo foi possível perceber que 47% dos Dentistas sempre utilizam a anestesia tópica, tendo preferência pelo tipo gel (89%), ao invés do Spray (7%). Múltiplos fatores podem ser responsáveis por uma confusão referente à eficácia do anestésico tópico: distinção da área da aplicação, secagem preliminar da superfície, o tempo de aplicação, método de avaliação da dor e tipo de produto. Concluiu-se que a maioria dos profissionais faz uso da anestesia tópica, principalmente na forma de gel, se destacando os endodontistas, dentistas restauradores, ortodontistas e protesistas. Além disso, indivíduos com mais tempo de formados utilizam anestésicos tópicos mais frequentemente e da forma mais correta.

Vulvar Verrucous Carcinoma (VVC) is a rare lesion, with few described cases. It has low metastatic potential with high morbidity due to the necessity of extensive resections, although. Previously, VVC was considered a synonym to the Buschke-Lowenstein Tumor (BLT) or Giant Condyloma Acuminatum (GCA). Lichen Sclerosus (LS) is associated with Vulvar Intraepithelial Neoplasia (VIN) and Vulvar Squamous-cell carcinoma (SCC); association with VVC is also described. The case of a 60-year-old menopausal woman is reported; she had chronic itching and an extensive verrucous lesion in vulva, initially diagnosed and treated as condyloma acuminatum; there was recurrence as verrucous carcinoma associated to LS. Excision with margins was performed and clobetasol and imiquimod were used. Patient had complete remission with no further recurrences. Distinction between VVC and BLT can be difficult; current literature considers them different entities. Human papillomavirus (HPV) infection and the presence of LS play a controversial role in these injuries.
