Evaluating Health Promotion in Rio de Janeiro, Brazil: An Integrated Local Development Project

Obesity is an issue for young adults in the U.S. This population is particularly vulnerable to weight gain as they move from adolescence to young adulthood, especially as they transition from high school to college. Adopting a health promotion approach, a university-based cluster of researchers, community advocates, and a technology partner embarked on a two-year seed development project that focused on development, implementation, and evaluation of a web-based healthy lifestyle intervention for college students. Using a mixed-method design, two convenience samples of residential university students were recruited to participate in a 4-week intervention called Eat, Move, Live, in which they interacted with a newly-created comprehensive website about management of a healthy lifestyle. Participants’ post-intervention readiness for change increased by 15% (eating and life balance behaviors) to 23% (moving behaviors). Participants reported increased awareness of eating behaviors, and feelings of engagement in tracking their fruit and vegetable consumption. Findings suggest that technology may be utilized to enhance the effectiveness and efficiency of achieving students’ individual goals related to healthy living. These preliminary findings have implications for increasing the development and implementation of technological approaches to health promotion for young adult students.

In observational epidemiology it is usual to select a control group to study the effects of certain exposures on human health. Intervention studies are well known among epidemiologists but it is not very frequent in other areas of research. In this paper we propose the same idea of intervention studies and the use of three methods for a health promotion research control group selection: Propensity score, Mahalanobis' distance and Mahalanobis within Propensity Calipers. In the original project, "Health and Local Development: a progress review towards the millennium goals with relation to health in the Brazilian cities which develop social agendas", cities with social agendas from Brazil were matched separately by state. In the state of Paraná there are 397 cities. Of these, 34 presented social agendas implemented and active since, at least, 2004. Five variables measured in 2000 were considered for the matching: population size, human development index of income, human development index of education, percentage of literacy and vaccine coverage. As a result, among these three methods, the Mahalanobis by itself was considered the less efficient. In conclusion, the propensity, which is a very simple linear score, presented very good matched sample. However, the Mahalanobis within Calipers was the method that provided the best result.

The analysis of management efficiency is an important element in evaluating the functioning of public administration from an economic point of view. In order to achieve greater efficiency of the management process, and thus the quality in public administration, it is important to analyze and evaluate its elements. Modern research usually covers individual elements, parts of the management process. However, the current study proposes a comprehensive approach to this process. The objective of the article is the evaluate levels of management efficiency of local government offices using a single synthetic indicator and also to identify areas that hinder management efficiency. The study applies the institutional analysis methodology, adjusting it to the needs of the article. The article postulates that areas that call for immediate changes in Poland's local government offices should include such areas as strategic and financial management, invigorating economic development, project management and public service offers. Any changes thus introduced in these areas may enhance improvements in management processes, effectiveness and efficiency of activities, the quality of the office's functioning, organizational development, which togethr indirectly affects local development.

Introduction: Brazil’s HIV burden has greatly increased over the past decade, especially for socially marginalized and vulnerable groups such as adolescents, women, and men who have sex with men. The reasoning for worsening HIV outcomes is complex, but ongoing economic and political crises have placed extreme operational and financial burdens on both the public health system and HIV-related civil society, affecting both treatment and prevention efforts and delivery. Context: Community-based HIV-related health-promotion activities have continued in Nova Iguaçu, Rio de Janeiro, despite these setbacks. These efforts have been led by a semi-independent community advisory board and engagement group based at the Hospital Geral de Nova Iguaçu with support from researchers based at the Oswaldo Cruz Foundation. Methods: The research team supported, documented, and participated in various activities led by the community advisory board and engagement group from 2017–2018 including meetings, community workshops/lectures, production of health promotion materials, and the dissemination of research findings. Results: The research team utilized the concepts of vernacular knowledge and critical pedagogy to describe and document the ongoing, bottom-up approach, community-led efforts of the community advisory board and engagement group. In particular, we describe the process of stakeholder engagement, popularization of research results, and resource sharing spearheaded by the community advisory board in Nova Iguaçu. Conclusion: The community advisory board demonstrates how community-led efforts are essential to HIV and AIDS response efforts in light of worsening HIV burdens and global shifts towards biomedicalization. Their HIV-related activities rely on existing community networks and resources with secondary support from a research team. This illustrates a key intervention point between traditional research and an empowering community mobilization that can inform similar efforts in other low-resource settings.

ABSTRACT Objective: identifying the factors related to the mental health of women in a prison in the Statey of Rio de Janeiro. Method: a descriptive, exploratory and qualitative study conducted between October 2014 and January 2015 in a female prison in the State of Rio de Janeiro. Forty (40) incarcerated women were interviewed. The information collected was discussed based on content analysis, using a thematic based modality. Results: the following factors that affect the mental health of incarcerated women have been identified: anxiety, stress, depression, altered sleep patterns, misuse of psychotropic medication, sexual abstinence, and interruption of family relationships, in addition to the precarious conditions of confinement. Conclusion: we emphasize the need for a multiprofessional team in mental health within the prison system that meets the needs of this population, ensuring applicability of the National Policy of Comprehensive Healthcare for Persons Deprived of Freedom, reinforcing dialogue with Humanization Policies and Health Promotion, with the intention of performing healthcare that contributes to comprehensive healthcare for incarcerated women.

Resumo: No final da década de 1990, o Arranjo Produtivo Local (APL) de moda íntima de Nova Friburgo foi identificado por atores de política do estado do Rio de Janeiro, e passou a ser alvo de iniciativas de promoção. Uma delas foi o Programa de Desenvolvimento de Distritos Industriais do Serviço Brasileiro de Apoio à Micro e Pequena Empresa (SEBRAE), do Instituto Promos (PROMOS) e do Banco Interamericano de Desenvolvimento (BID), que representou um ponto de inflexão na trajetória de políticas de desenvolvimento do arranjo, pois a partir desse programa um número considerável de outras instituições passou a desenvolver ações de apoio. Essa capacidade de inspirar outras experiências, através da visibilidade e do efeito-demonstração, torna a política em questão um objeto de análise relevante. O intuito deste trabalho é investigar se essa política teve um caráter sistêmico ou não. Acredita-se que esse tipo de análise contribui para a compreensão dos desafios mais amplos das políticas estaduais de APLs no Rio de JaneiroPalavras-chaves: Arranjo produtivo local. Desenvolvimento local.Abstract: At the end of the 1990s, the Local Productive and Innovative System (LIPS) of intimate fashion in Nova Friburgo were identified by political actors from the state of Rio de Janeiro, and became the target of promotional initiatives. One of those was the Industrial Districts Development Program, promoted by SEBRAE, PROMOS and IDB, which represented a turning point in the development policies path of the system, since a considerable number of other institutions began to promote support actions. This ability to inspire other experiences through its visibility and demonstration-effect makes the policy in question a relevant object of analysis. The purpose of this paper is to investigate whether this policy were systemic or not. This type of analysis is helpful to the understanding about the broader challenges of the Rio de Janeiro LIPS policies.Keywords: Local productive arrangement. Systemic policy. Local development.

No estado do Rio de Janeiro, o setor cervejeiro acumula diariamente toneladas de resíduos cervejeiros, com destaque para o bagaço de malte, com índice protéico favorável ao beneficiamento do material composto para ração animal. Além de suprir a demanda do produto pelos proprietários de rebanho bovino fluminense, incentivar o beneficiamento destes itens favoreceria a eliminação do risco ambiental que os resíduos poderiam acarretar ao meio, caso fossem eliminados na natureza, e ao mesmo tempo gerar riqueza, emprego e desenvolvimento local. O objetivo geral deste trabalho foi avaliar práticas de gestão ambiental de uma fábrica de cervejas na cidade do Rio de Janeiro, e se tais práticas colaboram na existência de uma parceria com o mercado local produtor de leite. Para tal, determinou-se um estudo de caso em que foi empregado método de investigação indutivo, técnicas de observação direta intensiva, entrevistas não-padronizadas e questionários. Conclui-se que, a empresa pesquisada possui atuação ambientalmente responsável. Entretanto, não são traçados e executados com pura preocupação e responsabilidade para com a sociedade, mas, fundamentalmente elaborados em razão de aspectos econômicos e legais, como o cumprimento da legislação vigente e da redução de custos e/ou geração de receita. Abstract In the State of Rio de Janeiro, the beer industry daily accumulates tons of waste beer, especially malt, which has high protein index, favourable to milling material composed for animal feed. Besides meeting the demands of the state cattle owners, encouraging the processing of these items would facilitate the elimination of environmental risk that residues in half if expunged in nature and, at the same time, it would generate wealth, employment and local development. The overall objective of this work was to evaluate environmental management practices of a plant of beer in the city of Rio de Janeiro and check whether such practices are collaborating on the existence of a partnership with the local producer of milk market. To do so, a case study was carried out, applying inductive research, direct intensive observation techniques, non-patterned interviews and questionnaires. It was concluded that the company researched has doggie environmentally responsible. However, they are not pure strokes and run with concern and responsibility towards society, but essentially drawn up by economic and legal aspects, such as the enforcement of existing legislation and the reduction of costs and/or revenue generation. Purpose Implementing health promotion programmes in schools is key to improving children’s health and well-being but difficulties in achieving expected results are often reported in the research literature. Discrepancies between expected and achieved outcomes can originate from differences in contexts. Understanding how interactions between contexts and programmes generate variable outcomes is, therefore, critical. The purpose of this paper is to explore the outputs of a programme implemented in different school contexts. The focus is to pinpoint outputs, understand the involvement of combinations of contextual factors and identify recurrences in these combinations. Design/methodology/approach This retrospective study covers a period from 2006 to 2016. Data collection includes two sets of data in eight high schools in the Rhône-Alpes Region in France: written documents and interviews with school staff. Realist evaluation is used to attempt to pinpoint outputs and relating contextual factors. Findings Results highlight the limited outputs of the programme. Differences between schools appear to originate from existing school policy prior to participation, existence of a project team, identification of the issue as priority and staff turnover. Analysis of contextual factors led to considering the implementation process as enabling health capacity building and enhanced the capacity of settings and communities to promote health. Research limitations/implications The data provided remain partial as there was high staff turnover, reluctance to participate due to failure to implement the project, and schools being over burdened with other requests. Originality/value Previous research suggests that top-down implementation of a standard programme is not an efficient strategy for all schools to engage in the development of suitable health promotion policies. A potential way forward is to base support for the local development of health promotion in schools on a better understanding of the contexts in which implementation occurs.
