The paper describes the methods used for a study of the safety management system and culture of a cold rolling mill in a steel plant. It uses data from diverse sources which can be validated against each other and combined to produce a rich picture of the current risk control system and its shortcomings. The paper also describes the proposed improvements and what the plant chose to implement. This step also throws light on the culture of the plant.

Given ubiquitous human exposure to ethylene oxide (EO), regardless of occupation or geography, the current risk-specific concentrations (RSCs: 0.0001–0.01 ppb) from the U.S. Environmental Protection Agency (EPA) cancer risk assessment for EO are not useful metrics for managing EO exposures to the general U.S. population. The magnitude of the RSCs for EO are so low, relative to typical endogenous equivalent metabolic concentrations (1.1–5.5 ppb) that contribute ~93% of total exposure, that the RSCs provide little utility in identifying excess environmental exposures that might increase cancer risk. EO monitoring data collected in the vicinity of eight EO-emitting facilities and corresponding background locations were used to characterize potential excess exogenous concentrations. Both 50th and 90th percentile exogenous exposure concentrations were combined with the 50th percentile endogenous exposure concentration for the nonsmoking population, and then compared to percentiles of total equivalent concentration for this population. No potential total exposure concentration for these local populations exceeded the normal total equivalent concentration 95th percentile, indicating that excess facility-related exposures are unlikely to require additional management to protect public health.

Abstract INTRODUCTION Perforated gastroduodenal ulcer represents a common surgical emergency and is associated with a morbimortality close to 40-50%. HP infection, tobacco and alcohol and some socioeconomic aspects represent the most commonly described risk factors. MATERIAL AND METHODS A descriptive and retrospective study was performed between 2018-2020. A total of 40 patients with a history of gastroduodenal perforation were selected. Data corresponding to: sex, age, ulcer location, treatment, tobacco and alcohol consumption, HP infection (presence or not of eradicating treatment) and other potential risk factors were collected and analysed. The aim of the study is to assess how new lifestyle changes, the evolution and improvement in the management of PH infection and the general decrease in smoking have modified the classic profile of patients with gastroduodenal perforations. RESULTS Gastroduodenal perforations predominated in the fifth/sixth decade of life (mean age 59 years). Although a predominance in men is described in the literature, in our sample no significant sex differences were found (1:1 ratio). The most frequent location was duodenal and the reference surgical treatment was primary suture + epipoplasty. Only one third of the patients had a known history of peptic ulcer. CONCLUSIONS Gastroduodenal perforations are a major problem for the on-call surgeon. It is essential to know the current risk factors and their potential relationship with morbimortality and postoperative evolution in order to guarantee the optimal management of these patients.

Regulation of occupational exposure to noise in Britain for 50 years has reduced risk. However, statistics from around the globe (and in Britain alone) suggest that the range in harm is between around zero and more than 1 in 4 workers exposed to high noise. The uncertainty in statistics and the potential high incidence and prevalence of harm justifies investigation. In Britain, we will investigate the current risk of occupational hearing loss and the effectiveness of current noise control measures. We propose to gather data during inspections of industries that are known to have high levels of workplace noise. Finding high incidence of hearing damage will indicate a failure of immediate management of risk and likely result in enforcement action. We propose to review employers' control of noise propagation in the workplace through use and maintenance of noise controls supplied with machines and supplemented with acoustic barriers and noise havens. We propose to review suppliers design and build of noise control into their products and their reported noise emissions for noisiest typical use. We are looking to benefit from the experience of our global counterparts before finalising our plans.

Abstract Concerns about infectious disease in mega-shelters following hurricanes should be a top priority. The COVID-19 pandemic demonstrated the challenge of implementing standard evacuation and sheltering procedures for hurricanes during an outbreak of a respiratory disease and the lessons learned from the 2020 hurricane season must be applied to future response efforts. In this article, we examine the current risk framing for hurricane preparedness and response utilizing Prospect Theory. We also examine how the COVID-19 pandemic has complicated this traditional framing and offer a new framework for which to provide adequate sheltering following a hurricane, while minimizing the risk of respiratory disease to those seeking shelter. We argue that such a framework is necessary to protect American citizens in future hurricane seasons.

Dynamics in the global food system, along with a cascade of technologies, drive demands for capturing information and sharing information vertically within the supply chain. Food safety, genetic engineering, and animal welfare all have contributed to the need for enhanced information flow within the supply chain. Identity preservation in grains and oilseeds is an emerging issue that may influence the structure of agriculture in the longer term. This research addresses the following questions. While demand for high-information grains appears to be growing, where and how along the supply chain is the value created and captured? Though it appears that the economy demands ever-increasing amounts of differentiation, why do opportunities for producers and life science companies to create and capture significant new sources of value remain elusive? To answer these questions needs assessments were conducted with grain procurement executives. Their responses reveal the "buyer's calculus" where buyers balance investment in specific relationship assets with the market uplift or risk mitigation return it generates. Buying from a competitively structured industry has numerous benefits. There is a "cost" or tradeoff leaving the spot market procurement model in favor of a relationship-based model; hence the calculus. The current equilibrium state reflects the current risk-adjusted value proposition suppliers deliver to end users. Though end-user benefits are on the horizon with the next generation of biotechnologies, their emergence is insufficient to guarantee farmers and life science greater returns. End users will always balance the risk mitigation and market uplift features of a supply offering with the risks of narrowing their supply base. To drive value up the chain, suppliers need to shift away from focusing solely on the products of the future and focus on the technologies, delivery systems, and organizational models that, when bundled with new products solve problems and make end users more competitive.
