Topic The development of the Critical Care Beacon Collaborative to achieve meaningful recognition. Clinical Relevance Recognizing nurses for contributions to their work environment and care delivery is important for their professional and personal fulfillment, job satisfaction, and retention; such recognition can occur at the individual, unit, or organizational level. The American Nurses Credentialing Center’s Magnet Recognition Program acknowledges nursing excellence at the organizational level. It would, however, be difficult for an organization to achieve Magnet designation without nursing excellence at the unit level. To recognize excellence at the unit level, the American Association of Critical-Care Nurses developed the Beacon Award in 2003. Objective To describe one academic medical center’s journey toward winning Beacon Awards across 8 units within the adult critical care service. Content Covered The Critical Care Beacon Collaborative resulted in a Beacon Award for each unit and important staff outcomes. This article describes the organization, the process before the Critical Care Beacon Collaborative convened and the desired state, and the methods used to achieve our goal. It also discusses unit- and service-level stakeholder involvement. The successes, lessons learned, sustainability, and growth of the Critical Care Beacon Collaborative are shared to assist readers who aspire to pursue a Beacon Award.

Purpose – The purpose of this paper is to draw out the lessons learned from the implementation of the Individual Placement and Support (IPS) approach to supported employment in two contrasting adult mental health teams; one “standard” CMHT, and one early intervention in psychosis (EIP) team. Design/methodology/approach – These inferences are based on the evidence from a four-year study of IPS in one mental health care provider in the UK, which began by setting up a new service, and went on to run a RCT looking at the impact of psychological input as an adjunct to IPS alone. Findings – In attempting to introduce IPS to mental health teams in Nottingham the authors came across numerous barriers, including service reorganisation, funding cuts and the wider context of recession. Differences were observed between mental health teams in the willingness to embrace IPS. The authors argue that this variability is due to differences in caseload size, recovery priorities and client profiles. The authors have learnt that perseverance, strenuous efforts to engage clinical staff and the use of IPS fidelity reviews can make a positive difference to the implementation process. Practical implications – The experience suggests that setting up an IPS service is possible even in the most challenging of times, and that EIP services may be a particularly fertile ground for this approach. The authors also discuss potential barriers to implementing new services in mental health teams. Originality/value – This paper will be of value to service development and the science of implementation in mental health.

This chapter includes discussions pertaining to the role of culture in influencing electronic business diffusion in developing countries. In this chapter, the author discusses specific cultural factors and their influences on the individual components required for Internet rollout and use in developing countries. Cases from developing countries are also presented to illustrate the effects culture produces on the levels of electronic business technology usage. The discussions of the lessons learned from those cases along with the theoretical foundations presented throughout the chapter, culminate in the author’s provision of recommendations to the reader, deemed necessary for effectively increasing the diffusion of electronic business in developing countries as well as for reaping the potential benefits generated from its use.

Sex-related behavior can present challenges in the community and present serious impacts on individuals with special needs. When considering sex offender types of behavior, special considerations are made for supervision, prevention, and education for the individual and the community at large. William is an elderly gentleman who presents with inappropriate sexual behavior resulting in both child and adult victims. The following chapter discusses his history along with some unique challenges related to his sexual behavior, interventions considered as part of his treatment plan, and some lessons learned from working with William and his unique considerations.

Abstract Here we have summarized what is currently known about menstruating animal species with special emphasis on non-primate species: length of their menstrual cycle, ovulation, implantation, placentation, decidualization, and endometrial characteristics. Having an overview of all the possible animal models that can be used to study menstruation and the menstrual cycle could be useful to select the one that better matches the needs of the individual research projects. The most promising species to study menstruation seems to be the spiny mouse Acomys cahirinus. It is a rodent that could be easily held in the existing laboratory facilities for rats and mice but with the great advantage of having spontaneous menstruation and several human-like menstrual cycle characteristics. Among the species of menstruating bats, the black mastiff bat Molossus ater and wild fulvous fruit bat Rousettus leschenaultii are the ones presenting the most human-like characteristics. The elephant shrew seems to be the less suitable species among the ones analyzed. The induced mouse model of menstruation is also presented as an adaptable alternative to study menstruation.

Abstract Systematic reviews and meta-analyses in the neurosurgical literature have surged in popularity over the last decade. It is our concern that, without a renewed effort to critically interpret and appraise these studies as high or low quality, we run the risk of the quality and value of evidence-based medicine in neurosurgery being misinterpreted. Correspondingly, we have outlined 4 major domains to target in interpreting neurosurgical systematic reviews and meta-analyses based on the lessons learned by a collaboration of clinicians and academics summarized as 4 pearls. The domains of (1) heterogeneity, (2) modeling, (3) certainty, and (4) bias in neurosurgical systematic reviews and meta-analyses were identified as aspects in which the authors’ approaches have changed over time to improve robustness and transparency. Examples of how and why these pearls were adapted were provided in areas of cranial neuralgia, spine, pediatric, and neuro-oncology to demonstrate how neurosurgical readers and writers may improve their interpretation of these domains. The incorporation of these pearls into practice will empower neurosurgical academics to effectively interpret systematic reviews and meta-analyses, enhancing the quality of our evidence-based medicine literature while maintaining a critical focus on the needs of the individual patients in neurosurgery.

The Individual

The research and development of a unique combustion engine is presented. The engine converts the thrust from ramjet modules located on the rim of a disk into shaft torque, which in turn can be used for electrical power generation or mechanical drive applications. A test program was undertaken that included evaluation of the pre-prototype engine and incorporation of improvements to the thrust modules and supporting systems. Fuel mixing studies with vortex generators and bluff-body flame holders demonstrated the importance of increasing the shear-layer area and spreading angle to augment flame volume. Evaluation of flame-holding configurations (with variable fuel injection methods) concluded that the heat release zone, and therefore combustion efficiency, could be manipulated by judicious selection of bluff-body geometry, and is less influenced by fuel injection distribution. Finally, successful operation of novel fuel and cooling air delivery systems have resolved issues of gas (fuel and air) delivery to the individual rotor segments. The lessons learned from the pre-prototype engine are currently being applied to the development of a 2.8MW engine.

Objective The Spanish Network of Agencies for Assessing National Health System Technologies and Performance (RedETS) defined a patient involvement (PI) framework for health technology assessment (HTA) activities in 2016. The aim of this study is to evaluate the process and impact of those PI initiatives that were implemented in the first year following the publication of this new framework. Methods A survey was sent to those HTA researchers who implemented PI in RedETS projects. Responses were reviewed by two authors. An adapted thematic analysis was performed and the results were later discussed by all authors. Results Six responses from six agencies/units were analyzed. The objectives of PI initiatives were the following: inclusion of patient perspectives, preferences and values; elicitation of important health outcomes measures; and barriers, facilitators, or suggestions for implementation. Different methods were used for PI: surveys, focus groups, in depth interviews, and participation in an expert panel. Five main themes emerged: (i) challenges with the recruitment process, (ii) needs identified, (iii) impact of PI, (iv) lessons learned, and (v) suggestions for the future. Conclusions PI initiatives within the RedETS framework were tailored to each HTA project, its specific goals and the individual needs and resources of each HTA agency. The results also pointed out how PI has a relevant impact that has enriched RedETS products providing key information on experiences, values, and preferences of patients, contributions that benefit the HTA and the process of drawing up recommendations. The main challenges were related to recruitment processes and capacity building.
