Zusammenfassung Ziel: Wir beurteilten die Ätiologie von Ulcera crurum bei Patienten mit Kollagenosen und evaluierten den Heilungsverlauf nach gezielter, kausaler Behandlung. Methoden: Retrospektive klinische Studie mit 20 konsekutiven Patienten: rheumatoide Arthritis (n = 12), progressive systemische Sklerose (n = 7), systemischer Lupus erythematodes (n = 1). Ergebnisse: Folgende relevanten Pathologien wurden vorgefunden: Makrozirkulation (insgesamt 17; arteriell 10, venös 10, kombiniert 3), kutane leukozytoklastische Vaskulitis (5), Steroid-induzierte Hautatrophie (8), arthrogenes Stauungssyndrom (9), dystrophe Kalzinose (3). Folgende Interventionen wurden durchgeführt: Arterielle Revaskularisation (9), Krossektomie und Stripping von insuffizienten Stammvenen (7), Spalthauttransplantation (17), wovon fünfmal unter Immunsuppression bei Vaskulitis. Bei Studienabschluss waren 13 abgeheilt, 4 gebessert (90% geheilt) und 3 unverändert; ferner 1 Unterschenkelamputation der Gegenseite, ein Patient verstarb. Schlussfolgerung: Ulcera crurum bei Patienten mit Kollagenosen haben meistens eine multifaktorielle Ätiologie. Die Verbesserung der arteriellen und venösen Makrozirkulation ist der wichtigste Schritt in der Therapieplanung. Selbst dann benötigen noch drei Viertel aller Patienten eine Spalthauttransplantation, um die chronische Wunde unter verbesserten Voraussetzungen zur Abheilung oder Besserung zu bringen.

The article reflects the results of a retrospective non-comparative study, the objects of which were 34 outpatient comorbid patients (15 of which are over 60 years old) with chronic wounds of the lower limbs developed against the background of varicosity, post-thrombotic disease, chronic arterial insufficiency of the lower limbs, diabetes mellitus or their combination. Their local treatment was carried out using atraumatic ointment dressing «Branolind N» containing Peruvian balsam. There were 23 women (67.6 %) and 11 men (32.4 %). In microbiological study prior to the beginning of treatment in 31 patients the growth of a pathogen of wound infection was revealed; in 19 patients — Staphylococcus aureus in monoculture and in various associations, in 6 patients — Pseudomonas aeruginosa in monoculture, in other cases — other pathogens. In 3 patients the pathogen was not detected. The average wound size was 34 cm2 . The phase of the wound process was additionally confirmed by cytological studies. After treatment the average area of the wound defect decreased by 10 cm2 and made 24 cm2 on average. Full healing of the wound defect occurred in 11 patients, the others had granulation and active marginal epithelization. There was also a decrease in bacterial semination of wounds, a change in composition of infection agents to less aggressive monoflora, and cytologically — a decrease in signs of inflammation against the background of significant activation of reparative processes.

Background: The design and characteristics of alginate biomaterial have a significant role in wound dressing and tissue regeneration. The ideal biomaterial for wound dressing must have biodegradable, biocompatible, non-inflammatory, non-toxic. Objective: Wound dressing should promote the re-epithelization and protect the wound for further infection by creating a moist environment. The physical and mechanical nature of the alginate biopolymer has the potential to influence the pathophysiological mechanisms in the chronic wound actively. The application of this biomaterial provides an ampule advantage to synthetic polymers. Method: The wound healing process is a critical step involved in different phases. The presence of cross-linkers, polymers along with alginate lead to a decrease in the mechanical property of dressing. Hence the effective material choosing along with alginate is a very critical decision. The therapeutic efficacy of the alginate dressing system (film, hydrogel, wafer, etc.) influenced by the incorporation of different materials such as bioactive agents, nanoparticles, cross-linker, nature of the excipients, etc. Results: The ion exchange occurring between dressing and exudates resulted in the formation of gel, due to the glucuronic acid presence in alginate. This gel absorbs moisture and maintains an appropriate moist environment and actively influence the pathophysiological mechanisms in the chronic wound. Conclusion: This review gives a detailed knowledge of the researchers to work with alginate and provides knowledge about the incorporation of the appropriate material and their therapeutic efficacy in wound dressing.

A prototype medical device and protocol that work well in a clinic would not necessarily work reliably in the home setting. The goal of this work is to identify factors to consider when translating a device evaluated in a clinic for use by home caregivers providing chronic wound care for persons living with dementia. One must consider the current device, the associated bundle, the protocol and how they may need to be modified. Semi-structured interviews with the research assistants who have applied a prototype device were conducted to generate a list of known issues with the current clinical protocol in terms of ultrasound application The researchers performed a set of specified tasks by following the current device protocol and explaining any difficulties with use\execution. They embellished the descriptions with details about what had happened with actual patients in the clinic. Using thematic analysis, analysts identified themes and subthemes. Issues potentially relevant to translating medical devices to the home are discussed.
