Neoplasms continue to dominate globally as one of the major sources of human disease and death. There are multiple modifiable causes of cancer and understanding their attributable risk factors for each cancer is of importance. This chapter covers the role of cellular and molecular mechanisms as well as the experimental and epidemiological approaches as determinants of the main cancers. Even if major discoveries in the clinical management of cancer patients will be accomplished in the near future, the changes will mainly affect the affluent part of the world population. Promising approaches focused on prevention of the known causes, reducing its consequences, notably in resource-constrained settings are highlighted.

To study the importance of migration to urban area, marital status and smoking as risk factors in cancer, a cohort of 4475 Finnish men was followed up for the occurrence of cancer during the period 1964–1980. Of particular interest was the interaction of migration or marital status with smoking. For cancers at all sites, not married urbanized smoking men had the greatest risk, followed by not married native urban smokers. This pattern was mainly due to high risk of cancers of the lung and larynx among the urbanized men, with a risk peak among urbanized not married smokers. The pattern persisted even when the amount smoked was allowed for. The importance in lung cancer epidemiology of vitamin A deficiency, occupation and psychosocial stress was discussed, and some support was found for the role of psychosocial stress in both the migration and the marital status factors.

Colorectal cancer (CRC) is one of the most common types of cancer worldwide. There are many factors that predispose a patient to the disease such as age, family history, ethnicity, and lifestyle. There are different genetic factors and diseases that also increase a person’s risk for developing CRC. Studies have found associations between gut microbiome and the risk for developing versus protection against CRC. Normal gut microbiome aid in daily functions of the human body such as absorption, metabolism, detoxification, and regulation of inflammation. While some species of bacteria prevent CRC development and aid in therapeutic responses to various treatment regiments, other species seem to promote CRC pathogenesis. In this regard, many studies have been conducted to not only understand the biology behind these opposing different bacterial species; but also to determine if supplementation of these tumor opposing bacterial species as probiotics lends toward decreased risk of CRC development and improved therapeutic responses in patients with CRC. In this literature review, we aim to discuss the basics on colorectal cancer (epidemiology, risk factors, targets, treatments), discuss associations between different bacterial strains and CRC, and discuss probiotics and their roles in CRC prevention and treatment.

Judgments about causality are central to the development of interventions intended to reduce exposure to risk factors that cause cancer. Because causation is not directly observable in medicine, scientists and philosophers have had to develop sets of constructs and heuristics that define “cause” operationally. The criteria in this framework, often attributed to the British medical statistician Sir Austin Bradford Hill or to the 1964 Report of the US Surgeon General on tobacco, include consistency, strength of association, specificity, temporality, coherence/plausibility/analogy, biological gradient, and experiment. This chapter reviews these criteria in depth and considers the challenges of applying them to population research on cancer. It discusses the concepts of causation in the context of the multistage nature of cancer, the “counterfactual” notion of causation, the component cause model for understanding diseases with multiple causes, and the “weight of the evidence” approach for integrating information from multiple lines of research.
