Growing research suggests that high-intensity interval training (HIIT) is a time-efficient exercise strategy to improve cardiorespiratory and metabolic health. “All out” HIIT models such as Wingate-type exercise are particularly effective, but this type of training may not be safe, tolerable or practical for many individuals. Recent studies, however, have revealed the potential for other models of HIIT, which may be more feasible but are still time-efficient, to stimulate adaptations similar to more demanding low-volume HIIT models and high-volume endurance-type training. As little as 3 HIIT sessions per week, involving ≤10 min of intense exercise within a time commitment of ≤30 min per session, including warm-up, recovery between intervals and cool down, has been shown to improve aerobic capacity, skeletal muscle oxidative capacity, exercise tolerance and markers of disease risk after only a few weeks in both healthy individuals and people with cardiometabolic disorders. Additional research is warranted, as studies conducted have been relatively short-term, with a limited number of measurements performed on small groups of subjects. However, given that “lack of time” remains one of the most commonly cited barriers to regular exercise participation, low-volume HIIT is a time-efficient exercise strategy that warrants consideration by health practitioners and fitness professionals.

Introduction: Aerobic training with an acyclic and intermittent character triggers high metabolic stress, responsible for generating alterations in several blood biomarkers. Thus, investigations that clarify understanding of metabolic behavior in response to exercise seem pertinent, when considering the dynamics of prescription of physical training and recovery. Objective: Demonstrate and discuss the behavior of blood biomarkers in response to High Intensity Interval Training (HIIT) and High Intensity Functional Training (HIFT). Methods: The PubMed/MEDLINE, Scielo, Lilacs, Bireme, Google Scholar, and Scopus databases were searched from the oldest records available until January 16, 2020. The search was carried out by combining descriptors related to the terms: “HIIT”, “HIFT”, and “blood biomarkers”. To be included, studies were required to: 1) have a clinical trial design; 2) evaluate the effects of an HIIT and/or HIFT protocol; and 3) measure blood biomarkers before and after the training protocol. No restrictions were applied to the characteristics of the participants regarding health condition, age, sex, and level of training. Results: In total, seven studies were included (n=221 participants, aged between 18 and 63 years) that analyzed different population profiles such as athletes, sedentary young people, patients with breast cancer, and diabetics. The biomarkers evaluated included analysis of muscle damage (C Reactive protein and CK); oxidative stress (antioxidant capacity); kidney injury (creatinine and urea); hormones (testosterone and cortisol); cytokines (TNF-α, IL-6, IL-1β, IL-10, IL-4, and IF-γ); and hemogram. In general, the results demonstrated specific patterns for the investigated markers. Thus, there were increases in muscle damage markers, while in the inflammatory markers, there was a reduction in pro-inflammatory cytokines and an increase in anti-inflammatory cytokines. Despite the reduced values of the general blood count, markers such as neutrophils and basophils did not demonstrate statistically significant alterations. Serum testosterone levels were higher and cortisol was lower in the post-exercise period when compared to pre-exercise. Conclusion: These data are of practical relevance when demonstrating patterns of physiological responses, which also characterize knowledge and understanding essential to determine adequate periodization.

Objective: The aim of this study was to compare the effects of low-volume, high-intensity interval training (HIIT) or moderate-intensity continuous training (MICT) on body composition in adults with metabolic syndrome (MS). Methods: This is a post hoc analysis of the randomized clinical trial Intraining-MET. Sixty adults (40–60 years old) were randomized to an MICT (n = 31) or HIIT (n = 29) supervised programme 3 days/week for 12 weeks. MICT sessions were conducted for 36 min at 60% of peak oxygen consumption (VO2peak). HIIT sessions included 6 intervals at 90% VO2peak for 1 min, followed by 2 min at 50% VO2peak. Body composition was assessed with dual energy X-ray absorptiometry. Results: Body weight did not change from pre- to post-training in either MICT (78.9 ± 15.6 kg; 77.7 ± 16.5 kg, p = 0.280) or HIIT groups (76.3 ± 13.4 kg; 76.3 ± 13.7 kg, p = 0.964). Body fat percentage and fat mass (FM) decreased post-training in the MICT (−0.9%; 95% confidence interval [CI]: −0.27 to −1.47 and −0.7 kg; 95% CI: −0.12 to −1.30) and HIIT groups (−1.0%; 95% CI: −0.32 to −1.68 and −0.8 kg; 95% CI: −0.17 to −1.47). Compared to the HIIT programme, MICT significantly reduced android FM (−0.14 kg; 95% CI: −0.02 to −0.26). Lean mass (LM) increased post-training in MICT (+0.7 kg; 95% CI: 0.01–1.41) and HIIT groups (+0.9 kg; 95% CI: 0.12–1.64), but only HIIT increased the trunk LM (+0.6 kg; 95% CI: 0.06–1.20). Conclusions: Both MICT and HIIT reduced FM without changing body weight in adults with MS. MICT had additional benefits by reducing the android FM, whereas HIIT seemed to increase LM. Given the characteristics of the post hoc analysis, further research is required to confirm these results.

Abstract Background Obesity is associated with impaired health and lower work ability. Increased physical activity is a cornerstone in the treatment of obesity and related risk factors. Recently, high-intensity interval training (HIIT) has emerged as a popular exercise option. However, data regarding the effects on cardiometabolic health, perceived work ability and well-being in severely obese individuals are lacking. Methods Sixty-five obese individuals with sedentary occupation (48.7 ± 9.9 years, BMI: 39.6 ± 7.1 kg/m2) were randomly allocated to an extremely time-efficient HIIT (5 × 1 min at 80–95% maximal heart rate on cycle ergometers, 2×/week for 12 weeks) or an inactive control group (CON). Both groups received nutritional counseling to support weight loss. Primary outcome was maximal oxygen uptake (VO2max), secondary outcomes were cardiometabolic risk indices, body composition, work ability index (WAI), quality of life (QoL, EQ-5D-5L-questionnaire) and perceived stress (PSQ-questionnaire). Results Mean body weight reduction was 5.3 kg [95% confidence interval (95% CI) − 7.3 to − 3.3 kg] in the HIIT group (P < 0.001) and 3.7 kg (95% CI − 5.3 to − 2.1 kg) in CON (P < 0.001), respectively. Only the HIIT group showed significant (P < 0.001) changes in VO2max [+ 3.5 mL/kg/min (95% CI 2.5 to 4.6 mL/kg/min)], waist circumference [–7.5 cm (95% CI − 9.8 to − 5.1 kg)], mean arterial blood pressure [− 11 mmHg (95% CI − 14 to − 8 mmHg)], WAI [+ 3.0 points (95% CI 1.7 to 4.3 points)] and QoL [+ 10% (95% CI 5 to 16%)]. In CON, none of these parameters improved significantly. Conclusions Low-volume HIIT may induce significant improvements in cardiometabolic health, especially VO2max, WAI and well-being in obese individuals after only 12 weeks. Our results underpin the wide range of benefits on health and subjective measures through exercise that go well beyond simple weight loss through dietary restriction alone. Objective. The purpose of the study was to examine the acute effects of the timing of exercise on the glycemic control during and after exercise in T2D. Methods. This study included 26 T2D patients (14 women and 12 men) who were treated with metformin. All patients were tested on four occasions: metformin administration alone (Metf), high-intensity interval training (HIIT) performed at 30 minutes (EX30), 60 minutes (EX60), and 90 minutes (EX90) postbreakfast, respectively. Glucose, insulin, and superoxide dismutase (SOD) activity were examined. Results. Glucose decreased significantly after the exercise in EX30, EX60, and EX90. Compared with Metf, the decline in glucose immediately after the exercise was larger in EX30 (−2.58 mmol/L; 95% CI, −3.36 to −1.79 mmol/L; p<0.001), EX60 (−2.13 mmol/L; 95% CI, −2.91 to −1.34 mmol/L; p<0.001), and EX90 (−1.87 mmol/L; 95% CI, −2.65 to −1.08 mmol/L; p<0.001), respectively. Compared with Metf, the decrease in insulin was larger in EX30 and EX60 (both p<0.001). Conclusions. Timing of exercise is a factor to consider when prescribing exercise for T2D patients treated with metformin. This trial is registered with ChiCTR-IOR-16008469 on 13 May 2016.
