Background Several studies indicate that there is a relationship between vitamin B12 levels and inflammatory status. Some studies showed a significantly correlation between vitamin B12 status and inflammation. The aim of this study is to investigate the influence of inflammatory status on the effect of different vitamin B12 supplementation regimes. Methods We selected patients with a vitamin B12 deficiency based on methylmalonic acid (MMA) levels. A moderate vitamin B12 deficiency was defined as an MMA blood level between ≥300 and 430 nmol/L. In included patients, C-reactive protein (CRP), leukocytes, serum vitamin B12, and MMA levels were measured at baseline and after 6 months of follow-up. Results A total of 63 patients were included, treated with 3, 6, or no intramuscular vitamin B12 injections. In the 6 intramuscular injections group, the presupplementation CRP levels significantly predicted the response in terms of vitamin B12 increase (P = 0.015). Also, there was a significant reduction in CRP levels (P = 0.03) after 6 injections. There was a significant correlation between presupplementation MMA and presupplementation CRP (r = 0.127, P = 0.049). Conclusion This study showed that presupplementation CRP levels significantly predicted the response on 6 intramuscular vitamin B12 injections in patients after bariatric surgery. Second, the 6 intramuscular injection regimen showed a significant reduction in CRP levels. Third, there was a significant correlation between MMA and presupplementation CRP. This might indicate that there is interplay between the vitamin B12 supplementation and inflammatory levels in patients after bariatric surgery.

Dairy cow diets are generally balanced for energy and major nutrients with B vitamins generally assumed not to be limiting, in spite of their role as coenzymes, essential to many metabolic reactions in protein, carbohydrate and lipid metabolism. Assuming adequacy of B-vitamin supply may explain some of the discrepancies between the outcomes of metabolic prediction models and measured cow performance. In lactating dairy cow, the amount of B vitamins from the diet and synthesized by the ruminal microbiota is generally sufficient to prevent deficiency symptoms and, as such, is assumed to fulfill requirements. However, reports of beneficial effects of B-vitamin supplementation on dairy cow performance suggest that B-vitamin supply is sometimes lower than its needs, as an insufficient B-vitamin supply decreases metabolic efficiency by driving a shift towards alternative metabolic pathways with greater energy cost. Using information on folates and vitamin B12 illustrated how meeting dairy cow needs for B vitamins should not be overlooked in formulation of rations for lactating dairy cattle. The present review discusses current knowledge and indicates areas presently impeded by the lack of research results, especially the limitations on the ability to estimate B vitamin need and supply.

A number of nutritional causes of peripheral neuropathy have been identified. Patients who have gastrointestinal disease or who have undergone bariatric surgery are at risk, although frequently the specific cause is not ascertained. Vitamin B12 and thiamine deficiencies are the most common causes in bariatric surgery patients. The classic clinical picture of Vitamin B12 deficiency is a myelopathy with a concomitant peripheral neuropathy. Thiamine deficiency is the classic triad of mental status changes, opthalmoplegia, and ataxia. Copper deficiency is often unrecognized. Its presentation can be similar to Vitamin B12 deficiency. It is important that bariatric surgery patients receive continual follow up of their nutritional status and adherence to vitamin supplementation. Alcohol may cause a peripheral neuropathy through nutritional deficiency or perhaps direct toxic effects. Other deficiency states are discussed.

Abstract Background Older adults are reported to have sub-optimal B vitamin status; targeted food-based solutions may help to address this. The objectives of the OptiAge food intervention study were to develop and investigate the effectiveness of a B vitamin-fortified drink in improving B vitamin biomarkers in older Irish adults with a primary outcome of change in the B vitamin biomarker status. Methods A double-blinded randomised controlled trial was performed in parallel at University College Dublin and Ulster University. Participants aged > 50 years were recruited following screening for exclusion criteria (i.e. taking medications known to interfere with B vitamin metabolism, supplements containing B vitamins, consuming > 4 portions of B vitamin-fortified foods per week or diagnosed with gastrointestinal, liver or pulmonary disease). Recruited participants meeting the inclusion criteria were randomised (by sex and study centre) to receive daily for 16 weeks either B vitamin-fortified or placebo drinks as developed by Smartfish, Norway. Each B vitamin-fortified drink (200 ml) contained 200 µg folic acid, 10 µg vitamin B12, 10 mg vitamin B6 and 5 mg riboflavin, while the placebo was an identical, isocaloric formulation without added B vitamins. Fasting blood samples were collected pre- and post-intervention which were used to measure the primary outcome of change in B vitamin biomarker levels. Results A total of 95 participants were randomised, of which 81 commenced the trial. Of these, 70 completed (37 in the active and 33 in the placebo groups). Intention to treat (ITT) analysis of the B vitamins demonstrated a significant improvement in all B vitamin biomarkers in the active compared to placebo groups: p < 0.01 for each of serum folate, serum vitamin B12 and plasma pyridoxal 5′-phosphate (vitamin B6) and the functional riboflavin biomarker, erythrocyte glutathione reductase activation coefficient (EGRac). Correspondingly, a significant lowering of serum homocysteine from 11.9 (10.3–15.1) µmol/L to 10.6 (9.4–13.0) µmol/L was observed in response to the active treatment (P < 0.001). Similar results were seen in a per-protocol analysis. Conclusions The results demonstrate that a B vitamin-fortified drink was effective in optimising B vitamin status, making this a useful intervention option to improve B vitamin status in older adults. Abstract. We aim to assess the prevalence of vitamin D deficiency (VDD) in patients scheduled for bariatric surgery (BS), and to identify factors that might be associated with VDD. We conducted a cross-sectional observational study involving all consecutive patients scheduled for BS from 2017 to 2019. The exclusion criteria were missing data for vitamin D levels, intake of vitamin D supplements in the 3 months prior to serum vitamin D determination, and renal insufficiency. A total of 206 patients (mean age and body mass index [BMI] of 34.9 ± 10.7 years, and 44.3 ± 6.99 kg/m2, respectively) met the inclusion criteria and were enrolled for data analysis. VDD (<19.9 ng/mL), severe VDD (<10 ng/mL), and vitamin D insufficiency (20–29.9 ng/mL) were present in 68.8 %, 12.5 %, and 31.2 % of patients, respectively. A significant inverse correlation was found between vitamin D levels and initial BMI, parathyroid hormone, and homeostatic model assessment of insulin resistance (r = −0.280, p < 0.05; r = −0.407, p = 0.038; r = −0.445, p = 0.005), respectively. VDD was significantly more prevalent in patients with higher BMI [−0.413 ± 0.12, CI95 % (−0.659; −0.167), p = 0.006], whereas no significant association between hypertension [−1.005 ± 1.65, CI95 % (−4.338; 2.326), p = 0.001], and diabetes type 2 (T2D) [−0.44 ± 2.20, CI95 % (−4.876; 3.986), p = 0.841] was found. We observed significant association between female sex and levels of vitamin D [6.69 ± 2.31, CI95 % (2.06; 11.33), p = 0.006]. The present study shows that in patients scheduled for BS, VDD deficiency is common and was associated with higher BMI, and female sex.

Introduction: Cigarette smoking induces many chronic illnesses, but in developed countries it is a preventable risk factor. However, by increasing the protective protection mechanism, it may be possible to alleviate the smoke-induced damage. As smoking risk is minimised by vitamin C intake, it is recommended that smokers should take more vitamin C. This inverse correlation between both vitamin C intake and serum levels and smoking was independent of age, sex, body weight, ethnicity, and consumption of alcoholic beverages. The negative association between cigarette smoking and serum vitamin C levels continued, following further adjustment for dietary vitamin C intake. The risk of severe hypovitaminosis C, especially when not accompanied by vitamin supplementation, has been increased in smokers. These data indicate that the inverse relationship between smoking and serum vitamin C levels exists independently of dietary intake, while smoking adversely affects preferences for vitamin C rich foods. Methods: A survey method was performed, with 50 smokers (S) receiving either 500 mg of vitamin C or placebo (P) daily for 4 weeks, and 50 non-smokers receiving vitamin C without supplementation. All finished the hearing. Both groups were equal and C: 14.2 + /- 1.8 pack-years was the amount of cigarettes smoked. Concentrations of plasma vitamin C increased significantly (p < 0.005) only in the vitamin C supplement community.  Results: At SMHRC Hospital Nagpur, we examined the relationship between smoking and vitamin C status, dietary and serum vitamin C levels of 100 participants. Smokers of 20 cigarettes a day had the lowest dietary intake of vitamin C and serum levels, whereas smokers of 1-19 cigarettes a day had lower intake of vitamin C and serum levels (compared to respondents who had never smoked. This inverse correlation between vitamin C and smoking intake and serum levels was independent of age, sex, body weight, race, and consumption of alcoholic beverages. The negative association between cigarette smoking and serum vitamin C levels continued, following further adjustment for dietary vitamin C intake. In smokers, the risk of severe hypo-vitaminosis C, particularly when not accompanied by vitamin supplementation, has increased. Conclusion: These data indicate that the inverse relationship between smoking and serum vitamin C levels exists independently of dietary intake, while smoking adversely affects preferences for vitamin C rich foods. A balanced diet for smokers will obtain a hearty recommendation at this time, but guidelines should remain cautious about high-dose nutrition supplements. There are few long-term nutritional studies in subjects undergoing bariatric surgery that have assessed weight regain and nutritional deficiencies. In this study, we report data 8 years after surgery on weight loss, use of dietary supplements and deficit of micronutrients in a cohort of patients from five centres in central and northern Italy. The study group consisted of 52 subjects (age: 38.1 ± 10.6 y, 42 females): 16 patients had Roux-en-Y gastric bypass (RYGB), 25 patients had sleeve gastrectomy (SG) and 11 subjects had adjustable gastric banding (AGB). All three bariatric procedures led to sustained weight loss: the average percentage excess weight loss, defined as weight loss divided by excess weight based on ideal body weight, was 60.6% ± 32.3. Despite good adherence to prescribed supplements, 80.7% of subjects (72.7%, AGB; 76.7%, SG; 93.8 %, RYGB) reported at least one nutritional deficiency: iron (F 64.3% vs. M 30%), vitamin B12 (F 16.6% vs. M 10%), calcium (F 33.3% vs. M 0%) and vitamin D (F 38.1% vs. M 60%). Long-term nutritional deficiencies were greater than the general population among men for iron and among women for vitamin B12.
