The death of a child is traumatic for parents. The grief of bereaved fathers is inadequately understood since most studies on this subject have focused primarily on mothers. The goal of this phenomenological study was to understand fathers’ experiences following the death of their child. Interviews were conducted with 13 fathers whose child (aged 1–17 years) had died at least 1 and up to 6 years earlier, either from a life-limiting illness or unexpectedly in an intensive care unit in a pediatric hospital in Eastern Canada. The analysis indicates that fathers’ experience deep suffering after the death of their child and feel torn between the past and the future. Three major themes were identified: needing to push forward in order to avoid breakdown, keeping the child present in everyday life, and finding meaning in their experience of grief. Clinical implications for professionals working with this population are discussed.

19539 Background: Peripheral sensory neuropathy is a common side-effect of oxaliplatin based chemotherapy. The neuropathy is cumulative and dose-related. Symptoms include sensory ataxia and dysesthesia of the limbs, mouth, throat and larynx, and may be exacerbated by exposure to cold. Studies suggest that Glutathione (GSH) is neuroprotective against oxaliplatin-induced neuropathy. Methods: From Jan. 2004, 83 consecutive colorectal cancer patients (pts) elegibile to oxaliplatin-based regimen were treated with GSH 1500 mg/mq over a 15-minute infusion period before oxaliplatin. Treatment-related toxicity was evaluated based on National Cancer Institute (NCI) Criteria. Results: After four cycles of chemotherapy, 5 pts (6%) experienced G1 neurotoxicity. After eight cycles, 2 pts (2.4%) experienced G2 sensory neuropathy (duration < 7 days) and 8 (9.6%) pts G1. After 12 cycles, G3 sensory neuropathy was observed in 2 pts (2.4%), G2 in 8 (9.6%) and G1 in 11 pts (13%). Neither G4 sensory neuropathy was registered nor treatment interruption was required. Conclusions: These findings suggest that use of GSH may protect from oxaliplatin-induced neuropathy. In fact, in our series, only 2 (2.4%) pts experienced severe paresthesia interfering with daily activities and none hade permanent sensory loss. However, only a well designed randomised controlled study will definitely prove the protective effect of GSH on oxaliplatin induced neurotoxicity. AbstractA text written by a student in a Swedish comprehensive school, during a Biology test, is analyzed using a method based on Husserl's transcendental phenomenology. The method is presented in the article. The analysis results in an explicitation of horizons, which enables an access to the lifeworld (Lebenswelt) opened by the text. In this case, the interplay of school Biology (the school subject Biology) and "everyday life" is visible. The meaning constituted in the encounter with school Biology seems to lack natural science aspects. The visible aspects pertain to the bodily situatedness of a student in school and to the character of school knowledge. The world as constituted by school Biology seems to be a place of disembedded, general people where school is a place of non-learning.

Jumlah pasien pasca kolostomi akibat kanker kolon dan rektal di Indonesia mengalami peningkatan yang signifikan. Kolostomi menyebabkan masalah fisik, psikososial dan spiritual serta ekonomi. Tenaga kesehatan terutama perawat perlu memberikan asuhan keperawatan terhadap pasien pasca kolostomi secara menyeluruh. Penelitian kualitatif terhadap pasien kolostomi sangat diperlukan sebagai upaya untuk mengungkap secara mendalam pengalaman hidup pasien pasca kolostomi dan menemukan new insight, sehingga dapat menambah pengetahuan perawat dalam upaya meningkatkan kualitas asuhan keperawatan yang diberikan pada pasien. Penelitian ini menggunakan metode kualitatif dengan pendekatan fenomenologi. Data didapatkan dengan wawancara mendalam terhadap delapan informan yang merupakan pasien rawat jalan, terdiri dari lima laki-laki dan tiga perempuan, usia antara 30 tahun sampai dengan 73 tahun. Lamanya hidup dengan kolostomi antara empat bulan sampai dengan enam tahun. Analisis hasil wawancara menggunakan metode Colaizzi. Tema yang didapatkan dari pengalaman hidup pasien kolostomi antara lain: keterbatasan dalam melakukan aktivitas sehari-hari, perubahan psikososial informan, perubahan dalam perilaku ibadah dan distres spiritual, perubahan pada aktivitas seksual, sumber-sumber dukungan bagi informan, upaya menjalani hidup dengan kolostomi, adaptasi terhadap perubahan yang terjadi, serta penyulit dalam menjalani hidup dengan kolostomi. Individu yang hidup dengan kolostomi mengalami keterbatasan dalam pemenuhan kebutuhan dasarnya, termasuk perubahan psikososial, distres spiritual dan masalah ekonomi. Berdasarkan penelitian ini, perawat disarankan memberikan dukungan dan dan perhatian pada pasien pasca kolostomi.Kata kunci: Kanker kolorektal, kolostomi, pengalaman hidup The Life Experiences of Patients with Post ColostomyAbstractThe number of patient with colostomy that it caused by colorectal cancer has been increasing significantlyin Indonesia. Colostomy was affected to physical, psychosocial, spiritual and economic of patients. Health providers, especially nurses need to provide holistic care for post colostomy patients. The aims of this qualitative study were to describe the life experience of post colostomy patients and to explore new insight of nursing interventions. The new insight would increase nurses’ knowledge and improve the quality of nursing care. This phenomenological study was obtained data using in-depth interviews to 8 informants. The informants consist of 5 men and 3 women. The characteristic of informants included age between 30-73 years, and the length of time living with a colostomy between 4 months to 6 years. The data were analyzed using the Colaizzi method. This study found several themes such as limited daily activities, psychosocial changes; spiritual distress; changes in sexualactivities;sources of support;live with a colostomy; live adaptation; the burdens living with a colostomy. Living with a colostomy faced problems including the limitation to fulfill their needs, psychosocial changes, spiritual distress, and economic problems. Based on those problems, nurses can give support and attention for post colostomy patients.Key words: Colorectal cancer, colostomy, life experiences

Colorectal cancer is one of the most common malignancies worldwide. Oxaliplatin is the first-line chemotherapeutic agent for the treatment of advanced colorectal cancer. However, acquired resistance to oxaliplatin limits its therapeutic efficacy, and the underlying mechanism remains largely unclear. In this study, we compared the expression of a panel of microRNAs (miRNAs) between oxaliplatin-sensitive and -resistant HCT-116 colorectal cancer cells. We found that miR-454-3p was significantly up-regulated in oxaliplatin-resistant cells and was the most differently expressed miRNA. Interestingly, we observed that inhibition of miR-454-3p resensitized resistant cells to oxaliplatin and enhanced oxaliplatin-induced cellular apoptosis. Moreover, we determined that miR-454-3p promoted oxaliplatin resistance through targeting PTEN and activating the AKT signaling pathway. In vivo study revealed that overexpression of miR-454-3p decreased the sensitivity of HCT-116 xenograft tumors to oxaliplatin treatment in a mouse model. Clinically, overexpression of miR-454-3p was associated with decreased responsiveness to oxaliplatin-based chemotherapy, as well as a short progression-free survival. Taken together, our study indicated that the expression of miR-454-3p could be used to predict oxaliplatin sensitivity, and targeting miR-454-3p could overcome oxaliplatin resistance in colorectal cancer.
