Drug use is associated with lower plasma magnesium levels in geriatric outpatients; possible clinical relevance

Prepasture feeding of low quality hay versus hay and grain had no subsequent effect on rumen fluid ammonia, blood urea, or plasma calcium, glucose and lactic acid concentration in steers turned out on lush pasture. Steers previously fed hay and grain had lower plasma magnesium levels on the first day on unsupplemented pasture than those fed hay prior to pasture. Supplemental feed on pasture partly alleviated the depression in plasma magnesium on the first day on pasture. Feeding supplemental hay with pasture tended to lower rumen fluid ammonia, blood urea and plasma lactic acid compared with feeding no supplement during the initial days on pasture, indicating the desirability of hay feeding during this time. A hay and grain supplement with pasture had a similar effect to the hay supplement on rumen-fluid ammonia, and plasma lactic acid, but a lesser effect on blood urea concentration. Neither supplemental hay nor hay plus grain affected plasma calcium or glucose levels. All steers showed an increase in rumen fluid ammonia, blood urea and plasma lactic acid on the first day and a decrease in plasma calcium and magnesium on the second day on pasture. Blood urea, plasma magnesium and plasma calcium concentrations had not returned to prepasture levels by 21 days on pasture.

Abstract Background The aim of the study was to investigate the clinical relevance of IgM deposition in patients with LN in a large cohort. Results 217 patients with renal biopsy–proven lupus nephritis were enrolled. The associations between glomerular IgM deposition and clinicopathological parameters were further analyzed. IgM deposition was positively correlated with glomerular C1q and C3 deposition moderately (r = 0.436, P < 0.001; r = 0.408, P < 0.001, respectively), and inversely correlated with plasma levels of C3 and CFH mildly (r=-0.138, P = 0.043; r=-0.147, P = 0.037, respectively). By multivariate analysis, we found that glomerular IgM deposition independently contributes to glomerular C3 deposition in patients with lupus nephritis (OR = 2.002, 95% CI: 1.295–3.094, P = 0.002). In addition, we also found that patients with IgM 0+-2 + had similar plasma CFH levels, but in patients with IgM3+-4+, plasma CFH levels were significantly lower (300.4 ± 155.8µg/ml vs. 429.9 ± 187.5µg/ml, P < 0.001). Furthermore, patients with high density of glomerular IgM and low levels of CFH had heavier proteinuria, higher serum creatinine and lower plasma C3 levels (5.7 ± 3.1g/d vs. 4.7 ± 3.5g/d, P = 0.037;150.1 ± 121.0µmol/L vs. 105.6 ± 97.1µmol/L, P = 0.005; 0.3 ± 0.2µg/L vs. 0.4 ± 0.2µg/L, P = 0.04, respectively), comparing with those with low density of glomerular IgM and low levels of CFH. Conclusions Our results suggested IgM might bind to injury-associated epitopes and be involved in disease progression and provided a possible relevance of CFH and IgM in the process of alternative pathway (AP) activation.

Abstract Background Although the DSM-5 was adopted in 2013, the validity of the new substance use disorder (SUD) diagnosis and craving criterion has not been investigated systematically across substances. Methods Adults (N = 588) who engaged in binge drinking or illicit drug use and endorsed at least one DSM-5 SUD criterion were included. DSM-5 SUD criteria were assessed for alcohol, tobacco, cannabis, cocaine, heroin, and opioids. Craving was considered positive if “wanted to use so badly that could not think of anything else” (severe craving) or “felt a very strong desire or urge to use” (moderate craving) was endorsed. Baseline information on substance-related variables and psychopathology was collected, and electronic daily assessment queried substance use for the following 90 days. For each substance, logistic regression estimated the association between craving and validators, i.e. variables expected to be related to craving/SUD, and whether association with the validators differed for DSM-5 SUD diagnosed with craving as a criterion v. without. Results Across substances, craving was associated with most baseline validators (p values<0.05); neither moderate nor severe craving consistently showed greater associations. Baseline craving predicted subsequent use [odds ratios (OR): 4.2 (alcohol) – 234.3 (heroin); p's ⩽ 0.0001], with stronger associations for moderate than severe craving (p's < 0.05). Baseline DSM-5 SUD showed stronger associations with subsequent use when diagnosed with craving than without (p's < 0.05). Conclusion The DSM-5 craving criterion as operationalized in this study is valid. Including craving improves the validity of DSM-5 SUD diagnoses, and clinical relevance, since craving may cause impaired control over use and development and maintenance of SUD.

Muriate of potash applied at rates of 0, 1 and 2 cwt./acre to herbage which had also received similar treatments in the previous year did not influence plasma magnesium or calcium values in lactating ewes.2. 24 Cheviot ewes had slightly lower plasma calcium and magnesium levels than 24 Half-bred ewes of the same age both before and after lambing.3. 6·6 g. of magnesium oxide given as a daily drench did not increase plasma magnesium values as measured by blood samples taken 24 hr. after drenching. Values were however increased by 0·5 mg. Mg/100 ml. four hours after drenching.4. No clinical cases of hypomagnesaemic tetany occurred, even although the mean plasma magnesium level of all the ewes in the experiment was only about 1·0 mg./lOO ml.5. Plasma magnesium values of ewes before lambing were significantly (P < 0·001) correlated with post-lambing levels. Values for the ewes during 1962 were equally significantly correlated with levels for the same sheep at comparable dates in 1961.

1. An experiment is described which investigated the effects of potassium fertilizer, age of ewe, and a small magnesium supplement, on the plasma magnesium and calcium levels of ewes shortly after lambing.2. Three potassium treatments (0, 1, and 2 cwt. muriate of potash/acre) were used, each being replicated six times, which gave a total of eighteen plots. Each plot was grazed by two old and two young ewes. One ewe of each age in every plot was given supplementary magnesium in the form of two magnesium heavy pellets, which released a total of 150 mg. Mg/day.3. 22% of the ewes had plasma magnesium values below 1·0 mg. Mg/100 ml. on at least one sampling occasion during the experimental period. There was only one clinical case of tetany.4. Neither potassium fertilizer use, nor magnesium supplementation influenced plasma magnesium and calcium levels of the ewes. Old ewes had greater temporary falls in plasma magnesium values and had consistently lower plasma calcium values.5. Attention is drawn to the large individual variation between animals and the consequent need for providing adequate statistical analysis of experiments of this type.

1. A low magnesium diet is described, consisting of barley straw, sucrose and casein, which induced hypomagnesaemia and one case of clinical tetany in non-pregnant, non-lactating ewes.2. On the above diet a daily drench of 0·25 g. MgO maintained plasma magnesium levels at 1·60–1·80 mg./100 ml. which was significantly higher than the plasma magnesium level of 1·09–1·20 mg./100 ml. for the unsupplemented group.3. An experimental formulation of a magnesium heavy pellet is described and its use as a method of supplementation was investigated. On the above diet it was found that two or four heavy pellets also maintained plasma magnesium values at 1·601–80 mg./100 ml. which was significantly higher (P = 0·01) than the level of the unsupplemented group.4. Five sheep out of the forty ewes used in this experiment were found to have consistently lower plasma magnesium values than the remaining thirty-five.5. A preliminary trial of the use of heavy pellets as a magnesium supplement in non-pregnant, non-lactating ewes on winter grazing is described. The results of the trial under these conditions were encouraging.

Proton-pump inhibitors (PPIs) are commonly used after kidney transplantation and there is rarely an incentive to discontinue treatment. In the general population, PPI use has been associated with hypomagnesaemia. We aimed to investigate whether PPI use is associated with plasma magnesium, 24-h urinary magnesium excretion and hypomagnesaemia, in kidney transplant recipients (KTR). Plasma magnesium and 24-h urinary magnesium excretion were measured in 686 stable outpatient KTR with a functioning allograft for ≥1 year from the TransplantLines Food and Nutrition Biobank and Cohort-Study (NCT02811835). PPIs were used by 389 KTR (56.6%). In multivariable linear regression analyses, PPI use was associated with lower plasma magnesium (β: −0.02, P = 0.02) and lower 24-h urinary magnesium excretion (β: −0.82, P < 0.001). Moreover, PPI users had a higher risk of hypomagnesaemia (plasma magnesium <0.70 mmol/L), compared with non-users (Odds Ratio (OR): 2.12; 95% confidence interval (CI) 1.43–3.15, P < 0.001). This risk tended to be highest among KTR taking high PPI dosages (>20 mg omeprazole Eq/day) and was independent of adjustment for potential confounders (OR: 2.46; 95% CI 1.32–4.57, P < 0.005). No interaction was observed between PPI use and the use of loop diuretics, thiazide diuretics, tacrolimus, or diabetes (Pinteraction > 0.05). These results demonstrate that PPI use is independently associated with lower magnesium status and hypomagnesaemia in KTR. The concomitant decrease in urinary magnesium excretion indicates that this likely is the consequence of reduced intestinal magnesium absorption. Based on these results, it might be of benefit to monitor magnesium status periodically in KTR on chronic PPI therapy.
