Septal anterior ventricular exclusion operation (Pacopexy) for ischemic dilated cardiomyopathy: treat form not disease

Objective: Restoration of left ventricle size and shape is an effective surgical procedure in patients with dilated cardiomyopathy. This report defines early and intermediate results following the reshaping of the left ventricle from spherical to ellipsoid configuration in patients with ischemic cardiomyopathy, employing a technique for LV restoration (LVR) that uses form rather than disease as the endpoint for oblique patch placement. Methods: Between 1998 and 2004, a cohort of 83 patients with dilated ischemic cardiomyopathy underwent an operation to reshape the left ventricle. In 54 patients the Dor procedure was done, and 29 underwent the septal anterior ventricular exclusion (SAVE) procedure to emphasize the elliptical shape, whereby patch placement followed an oblique trajectory between the LV apex and septum below the aortic valve. Ventricular form, rather than the disease scar marked the suture placement site endpoint to create an ellipse. The mean age was 58 ± 27, but SAVE patients had larger end systolic volume index (135 ± 38 vs 95 ± 25*). Overall preoperative NYHA functional class III was in 69% and IV in 31 patients, but more SAVE patients were in class IV (38% vs 28%*). The procedures were elective in 72 and emergent in 11, with similar entry criteria for each procedure. Results: In combination with LVR operation, mitral surgery was performed in 49/83 and tricuspid annuloplasty in 23/83 patients, but these procedures were more common after SAVE (59% vs 44%* and 45% vs 19%*, respectively), because of larger LV volumes in SAVE patients; 2.8 ± 1.3 coronary artery bypass grafts were used. Perioperative use of IABP or LVAD was 15 and 1, respectively in 83 patients. Hospital death was in 1/11 or 9% after emergent operations and 3/72 or 4% in elective procedures, with no difference between groups. After discharge from the hospital, NYHA class improved to class I or II in 57 patients, class III/IV in 14 patients, with 10 late deaths. The 5-year survival rate after the elective operation was 80.3% in SAVE and with elective operation and 77.4% in the Dor procedure. Conclusion: The SAVE or Pacopexy technique is easy to reshape the dilated left ventricle from spherical to ellipsoid form after the LVR, and the resultant improved configuration may contribute to the overall results for patients with ischemic dilated cardiomyopathy.

Thrombotic events seem to be one of the most common and severe complications having a direct impact on the course of the disease in patients with cardiomyopathy.There were examined 94 children with dilated cardiomyopathy (DCMP) phenotype [49 children with dilated cardiomyopathy (DCMP), including 45 patients with non-compaction cardiomyopathy (NCMP) and remodeling in dilated phenotype]. Thromboses were diagnosed in 9 patients, including 7 DCMP and 2 NCMP cases. In 4 DCMP children, the thrombus was localized in the cavity of the left ventricle, one in the left atrium, the right ventricle, and the inferior vena cava. In NCMP children, intracardiac thrombus formation was not determined, one patient was diagnosed with an acute ischemic disorder of the cerebral circulation; in the second one, the thrombus was detected in the superior vena cava. Thrombosis in DCMP patients was detected against a background of a severe systolic dysfunction of the left ventricle (LVEF of below 30%), and in NCMP children with a moderate dysfunction. Also, the greatest prevalence rate of thrombotic complications was noted in Functional Class III and IV heart failure cases. At the same time, there was no established any influence of polymorphic markers G1691A of gene F5, G20210A of gene F2, C677T of MTHFR gene on the prevalence of thrombotic events. The authors believe the formation of thrombi with the severe LV dysfunction in children with cMYP should be taken into account in the determination the tactics of the treatment of such patients, as it is necessary to make a decision about administering antithrombotic therapy.

Aim. To demonstrate that the end volumes are indicative of the severity of the left ventricle pathological condition not only under preserved, but also under reduced ejection fraction. Material and methods. 32 patients with dilated cardiomyopathy and severe heart failure, as well as with complete monitor and computer and echocardiographic control were examined. Results. The sensitivity of the ejection fraction and end volumes to the severity of heart failure (stroke index from 15.3 to 57.2, average 29.5±1.6; ejection fraction from 16.5 to 48.0, average 27.4±127; heart rate from 52 tо 113, average 81.8±2.4) was established. It is shown that the percentage change (sensitivity) of the ejection fraction (55%) is much lower than that of the end diastolic volume index (190%) and that of the end systolic volume index (438%) to the severity of heart failure. Conclusion. Indices of end-diastolic and end-systolic volumes of critical patients with dilated cardiomyopathy are more sensitive to the severity of heart failure than the ejection fraction. It is advisable to use end volumes as indicators of heart failure. An adequate quantitative assessment of the severity of heart failure should include the heart rate (duration of the cardiac cycle T).

Abstract Introduction Various minimally invasive mitral valve (MV) repair techniques are available to treat degenerative mitral regurgitation (MR). Transapical implantation of artificial chordae on a beating heart is performed using the NeoChord DS1000 device with real-time TEE guidance. Purpose 1)To assess preoperative and the mid-term follow-up echocardiographic data in patients after MV repair using the NeoChord DS1000 device; 2)to investigate the changes of left ventricle (LV), left atrium (LA) and mitral annulus dimensions during the follow-up period; 3)to assess the difference of baseline echocardiographic parameters between successful and not-successful (severe residual MR) MV repair groups; 4)to identify the preoperative echocardiographic variables that may be associated with recurrence of MR at mid-term follow. Methods All patients after transapical MV repair with Neochord implantation in Vilnius University hospital were prospectively entered into the study. The acquired preoperative and follow-up echocardiographic datasets were analysed. According to the residual MR at follow-up, patients were stratified to 2 groups: group A – successful durable MV repair (residual MR ≤2); group B – MV repair failure (recurrence of severe MR or reintervention). Values were expressed as Mean±SD. Univariable regression analysis was used to identify anatomical predictors of residual MR. Results 53 (70.67%) patients had a residual MR ≤2 (Group A) and 22 (29.33%) residual MR≥2+ (Group B) at 26±6 months follow-up. At baseline, Group B patients had significantly larger left ventricle end diastolic diameter (LVEDD) (mean difference 5.67±1.29mm, p<0.0001) left ventricle end systolic diameter (LVESD) (mean difference 4.08±1.57mm, p=0.012), LA volume index (mean difference 21.57±5.003 p<0.0001) and higher systolic pulmonary pressure values (mean difference 10.46±3.34, p<0.003) compared with group A. Overall, a significant reduction in LA volume index (mean change 15.69±4.15ml/m2, p<0.001), LA diameter (mean change 3.15±1.24, p=0.012), LV diameter (mean change in LVEDD 4.78±0.88mm p<0.000) was observed at 24 months follow up. There was no significant changes in MV annular parameters at follow up. Left atrium volume (OR 1.018; 95% CI 1.006–1.035; p=0.009), left atrium volume index (OR 1.038; 95% CI 1.013–1.072; p=0.010), LVEDD (OR 1.201; 95% CI 1.088–1.353; p=0.0008), LVESD (OR 1.122; 95% CI 1.02–1.248); p=0.0236) and sPAP (OR 1.418; 95% CI 1.139–2.016; p=0.0014) were all significantly associated with the worse outcome (MR >2) after mini-invasive MV repair in univariable regression analysis. Conclusions Minimaly invasive MV repair with Neochord system on beating heart is effective in patients with degenerative MR. Baseline echocardiographic characteristics predictive for a worse middle term outcome are mainly related to LV and LA remodeling. Reverse remodeling of LV and LA is observed during the follow-up period with no significant changes in MV annulus. Funding Acknowledgement Type of funding source: None

Dilated cardiomyopathy (DCM) is characterized by the phenotype of a dilated left ventricle with systolic dysfunction. It is classified as hereditary when it is deemed of genetic origin; more than 50 genes are reported to be related to the condition. Symptoms include, among others, dyspnea, fatigue, arrhythmias, and syncope. Unfortunately, sudden cardiac death may be the first manifestation of the disease. Risk stratification regarding sudden death in hereditary DCM as well as preventive management poses a challenge due to the heterogeneity of the disease. The purpose of this chapter is to present the epidemiology, risk stratification, and preventive strategies of sudden cardiac death in hereditary DCM.

Objective. To research of possibilities of reconstruction of aorta`s ostium and ascending aorta during aortic valve replacement and simultaneous correction of mitral valve defects at patients with narrow aorta`s ostium. Materials and methods. The study group consisted of 46 patients with mitral-aortic heart diseases and combination with a narrow aortic mouth, who were operated on at the A Amosov National Institute of Cardiovascular surgery for the period from January 1, 2006 to January 1, 2020. All patients underwent reconstruction of the aortic root and ascending aorta according to the original method of posterior aortoplasty. There were 26 men (56.5%) and 20 women (43.5%). The age of patients ranged from 23 to 72 years (average - 58.4±7.3 years). 8 (17.4%) patients belonged to class III NYHA, 38 (82.6%) - to class IV. Results. Of the 46 operated patients at the hospital stage (30 days after surgery), 4 died (hospital mortality 8.7%). No fatalities were associated with surgical technique. The dynamics of echocardiographic parameters at the hospital stage was as follows: the systolic gradient on the aortic valve was before surgery 112.1 ± 15.2 mm Hg, on the aortic prosthesis at discharge - 23.2 ± 6.4 mm Hg; end-systolic index (ESI) of the left ventricle (ml/m²) - 59.1 ± 7.6 (before surgery) and 48.3 ± 5.9 (after surgery); left ventricle ejection fraction (EF) - 0.45 ± 0.04 (before surgery) and 0.53 ± 0.04 (after surgery). Conclusions. The proposed original technique of posterior aortoplasty allows to effectively expand the mouth of the aorta for further implantation of an artificial heart valve of larger diameter. The technique is quite safe. At the hospital stage there are no complications directly related to the technique of operations. At the early postoperative period, the morphometric parameters of the left ventricle (EF and ESI) improved. The technique can be successfully used for the correction of combined mitral-aortic valve defects.

The aim – to evaluate the effectiveness of glucocorticoid therapy in patients with myocarditis with reduced left ventricular ejection fraction that developed after COVID-19 infection.Materials and methods. The results of glucocorticoid therapy in 32 patients aged (35.2±2.3) years with acute myocarditis after COVID-19 infection and left ventricular ejection fraction < 40 % are presented. All patients were prescribed a 3-month course of methylprednisolone at a daily dose of 0.25 mg/kg, followed by a gradual dose reduction of 1 mg per week until complete withdrawal 6 months after the start of treatment.Results and discussion. The analysis of the results of the examinations was performed in the 1st month from the onset of myocarditis to the appointment of glucocorticoids and after 6 months of observation. Six months later, the end-diastolic volume index decreased by 18.5 %, the left ventricular ejection fraction increased by 23.8 %, and the longitudinal global systolic straine increased by 39.8 %. On cardiac MRI, the number of left ventricular segments affected by inflammatory changes decreased from 6.22±0.77 to 2.89±0.45 segments, and the number of segments with fibrotic changes did not change significantly. After 6 months of treatment, there was a significant decrease in the concentrations of proinflammatory cytokines and cardiospecific antibodies.Conclusions. The use of a 6-month course of glucocorticoid therapy in patients with myocarditis that developed after COVID-19 infection improved the contractility of the left ventricle against the background of a significant reduction in inflammatory lesions of the left ventricle and reduced concentrations of proinflammatory cytokines and cardiospecific antibodies.
