3D-printed anatomic models of the knee for evaluation of patellofemoral dysplasia in comparison to standard radiographs and computed tomography

Abstract Background Material extrusion is used to 3D print anatomic models and guides. Sterilization is required if a 3D printed part touches the patient during an intervention. Vaporized Hydrogen Peroxide (VHP) is one method of sterilization. There are four factors to consider when sterilizing an anatomic model or guide: sterility, biocompatibility, mechanical properties, and geometric fidelity. This project focuses on geometric fidelity for material extrusion of one polymer acrylonitrile butadiene styrene (ABS) using VHP. Methods De-identified computed tomography (CT) image data from 16 patients was segmented using Mimics Innovation Suite (Materialise NV, Leuven, Belgium). Eight patients had maxillary and mandibular defects depicted with the anatomic models, and eight had mandibular defects for the anatomic guides. Anatomic models and guides designed from the surfaces of CT scan reconstruction and segementation were 3D printed in medical-grade acrylonitrile butadiene styrene (ABS) material extrusion. The 16 parts underwent low-temperature sterilization with VHP. The dimensional error was estimated after sterilization by comparing scanned images of the 3D printed parts. Results The average of the estimated mean differences between the printed pieces before and after sterilization were − 0,011 ± 0,252 mm (95%CI − 0,011; − 0,010) for the models and 0,003 ± 0,057 mm (95%CI 0,002; 0,003) for the guides. Regarding the dimensional error of the sterilized parts compared to the original design, the estimated mean differences were − 0,082 ± 0,626 mm (95%CI − 0,083; − 0,081) for the models and 0,126 ± 0,205 mm (95%CI 0,126, 0,127) for the guides. Conclusion This project tested and verified dimensional stability, one of the four prerequisites for introducing vaporized hydrogen peroxide into 3D printing of anatomic models and guides; the 3D printed parts maintained dimensional stability after sterilization.

AbstractComputed tomography in suspected urolithiasis provides information about the presence, location and size of stones. Particularly stone size is a key parameter in treatment decision; however, data on impact of reformatation and measurement strategies is sparse. This study aimed to investigate the influence of different image reformatations, slice thicknesses and window settings on stone size measurements. Reference stone sizes of 47 kidney stones representative for clinically encountered compositions were measured manually using a digital caliper (Man-M). Afterwards stones were placed in a 3D-printed, semi-anthropomorphic phantom, and scanned using a low dose protocol (CTDIvol 2 mGy). Images were reconstructed using hybrid-iterative and model-based iterative reconstruction algorithms (HIR, MBIR) with different slice thicknesses. Two independent readers measured largest stone diameter on axial (2 mm and 5 mm) and multiplanar reformatations (based upon 0.67 mm reconstructions) using different window settings (soft-tissue and bone). Statistics were conducted using ANOVA ± correction for multiple comparisons. Overall stone size in CT was underestimated compared to Man-M (8.8 ± 2.9 vs. 7.7 ± 2.7 mm, p < 0.05), yet closely correlated (r = 0.70). Reconstruction algorithm and slice thickness did not significantly impact measurements (p > 0.05), while image reformatations and window settings did (p < 0.05). CT measurements using multiplanar reformatation with a bone window setting showed closest agreement with Man-M (8.7 ± 3.1 vs. 8.8 ± 2.9 mm, p < 0.05, r = 0.83). Manual CT-based stone size measurements are most accurate using multiplanar image reformatation with a bone window setting, while measurements on axial planes with different slice thicknesses underestimate true stone size. Therefore, this procedure is recommended when impacting treatment decision.

BACKGROUND: An increasing number of bone graft materials are commercially available and vary in their composition, mechanism of action, costs, and indications. OBJECTIVE: A commercially available PLGA scaffold produced using 3D printing technology has been used to promote the preservation of the alveolar socket after tooth extraction. We examined its influence on bone regeneration in long bones of New Zealand White rabbits. METHODS: 5.0-mm-diameter circular defects were created on the tibia bones of eight rabbits. Two groups were studied: (1) control group, in which the bone defects were left empty; (2) scaffold group, in which the PLGA scaffolds were implanted into the bone defect. Radiography was performed every two weeks postoperatively. After sacrifice, bone specimens were isolated and examined by micro-computed tomography and histology. RESULTS: Scaffolds were not degraded by eight weeks after surgery. Micro-computed tomography and histology showed that in the region of bone defects that was occupied by scaffolds, bone regeneration was compromised and the total bone volume/total volume ratio (BV/TV) was significantly lower. CONCLUSION: The implantation of this scaffold impedes bone regeneration in a non-critical bone defect. Implantation of bone scaffolds, if unnecessary, lead to a slower rate of fracture healing.

Additive manufacturing (commonly known as 3D printing) is defined as a family of technologies that deposit and consolidate materials to create a 3D object as opposed to subtractive manufacturing methodologies. Fused deposition modeling (FDM), one of the most popular additive manufacturing techniques, has demonstrated extensive applications in various industries such as medical prosthetics, automotive, and aeronautics. As a thermal process, FDM may introduce internal voids and pores into the fabricated thermoplastics, giving rise to potential reduction on the mechanical properties. This paper aims to investigate the effects of the microscopic pores on the mechanical properties of material fabricated by the FDM process via experiments and micromechanical modeling. More specifically, the three-dimensional microscopic details of the internal pores, such as size, shape, density, and spatial location were quantitatively characterized by X-ray computed tomography (XCT) and, subsequently, experiments were conducted to characterize the mechanical properties of the material. Based on the microscopic details of the pores characterized by XCT, a micromechanical model was proposed to predict the mechanical properties of the material as a function of the porosity (ratio of total volume of the pores over total volume of the material). The prediction results of the mechanical properties were found to be in agreement with the experimental data as well as the existing works. The proposed micromechanical model allows the future designers to predict the elastic properties of the 3D printed material based on the porosity from XCT results. This provides a possibility of saving the experimental cost on destructive testing.

Purpose Micro-focus X-ray computed tomography (CT) can be used to quantitatively evaluate the packing density, pore connectivity and provide the basis for specimen derived simulations of gas permeability of sand mould. This non-destructive experiment or following simulations can be done on any section of any size sand mould just before casting to validate the required properties. This paper aims to describe the challenges of this method and use it to simulate the gas permeability of 3D printed sand moulds for a range of controlling parameters. The permeability simulations are compared against experimental results using traditional measurement techniques. It suggests that a minimum volume of only 700 × 700 × 700 µm3 is required to obtain, a reliable and most representative than the value obtained by the traditional measurement technique, the simulated permeability of a specimen. Design/methodology/approach X-ray tomography images were used to reconstruct 3D models to simulate them for gas permeability of the 3D printed sand mould specimens, and the results were compared with the experimental result of the same. Findings The influence of printing parameters, especially the re-coater speed, on the pore connectivity of the 3D printed sand mould and related permeability has been identified. Characterisation of these sand moulds using X-ray CT and its suitability, compared to the traditional means, are also studied. While density and 3PB strength are a measure of the quality of the moulds, the pore connectivity from the tomographic images precisely relates to the permeability. The main conclusions of the present study are provided below. A minimum required sample size of 700 × 700 × 700 µm3 is required to provide representative permeability results. This was obtained from sand specimens with an average sand grain size of 140 µm, using the tomographic volume images to define a 3D mesh to run permeability calculations. Z-direction permeability is always lower than that in the X-/Y-directions due to the lower values of X-(120/140 µm) and Y-(101.6 µm) resolutions of the furan droplets. The anisotropic permeability of the 3D printed sand mould is mainly due to, the only adjustable, X-directional resolution of the furan droplets; the Y-directional resolution is a fixed distance, 102.6 µm, between the printhead nozzles and the Z-directional one is usually, 280 µm, twice the size of an average sand grain.A non-destructive and most representative permeability value can be obtained, using the computer simulation, on the reconstructed 3D X-ray tomography images obtained on a specific location of a 3D printed sand mould. This saves time and effort on printing a separate specimen for the traditional test which may not be the most representative to the printed mould. Originality/value The experimental result is compared with the computer simulated results.
