Introduction: Bacteria play a crucial role in the pathogenesis of pulpal and periradicular pathoses. For example: Apical periodontitis is an inflammation of dental periapical tissues developed as a response to colonization of microorganisms in root canal system. The elimination of microbial species from the infected root canal system necessitates a strict aseptic condition for Instrumentation, disinfection and inter appointment medication. Enterococcus faecalis and Candida albicans had been commonly associated organisms in treatment-resistant infections. Methods: This was an observational cross-sectional study based on web-survey questionnaires developed by the authors with high confidence. The sample size was 304 estimated using the Qualtrics calculator with a confidence level of 95% and a margin of error of 5%. The questionnaire is divided into sections, regarding endodontic practice include endodontic education , years of experience , used of rubber dam or no and how to used irrigant solution whoever Sodium hypochlorite (NaOCl), Chlorhexidine, chelating agents (EDTA) or (MTAD) and reasons of every irrigant that prevents during root canal treatment. Data were collected using Google forms and prepared for analysis using Microsoft Excel. Statistical analysis was performed using the SPSS software. Results: A total of 304 participants responded, 44.1% dental interns, 39.5% General Dentist, 9.9% Endodontist, 6.3% other dental specialty. the Probable reasons that prevent them using rubber dam during root canal treatment are: 5.9% due to difficulty to apply, 9.2% due to lack of materials, 5.3% because it is a time-consuming, 1.6% said that other isolation methods are enough (partial isolation), 3.9% because of multiple reason combined together. 48.7% they always use rubber dam, so they did not choose any of the previous reasons. Conclusion: In conclusion, we identify and understand, most of the participants “always using rubber dam” during root canal treatment and the most common reasons was time consumption that prevented dentist from using rubber. In this study majority of participants preferred NaOCl 81.9% and 7.1% don’t use it.

Background:The purpose of this study was to determine incidence, survival rate, and prognostic factors as well as the frequency of Collins’ Law Violators (CLVs) in an unselected population of medulloblastoma patients. Collins’ Law dictates that ’cure’ of a child with a tumor occurs after a period that includes the child’s age at diagnosis plus 9 months.Methods:Using the Alberta Cancer Registry a population-based review identified 49 patients with medulloblastoma (19 adults, 30 children) diagnosed from 1975-96. Pathology was reviewed in all cases. All patients had surgical resection, followed by radiotherapy in 47 patients and chemotherapy in 17.Results:The overall 5-year survival was 50%. There was a trend for the extent of resection to be associated with a longer survival (Long rank test, p< 0.06) but this was not significant. Tumor recurrence occurred a median of 22.4 months (range, 6.4-192.3) after diagnosis and median survival after recurrence was 9.3 months (range, 0.4-64.9). The survival curve did not appear to plateau but was affected by tumor-related deaths in 3 (21.4%) of the 21 long-term survivors diagnosed in childhood. These three patients had recurrences a mean of 11.7 years after diagnosis and are designated as CLVs.Conclusions:The survival rate in an unselected population of patients with medulloblastoma is poor. Aggressive resection of the tumors prolongs survival. The Collins’ Law Violators were relatively common and we suggest this concept be abandoned in medulloblastoma.

ABSTRACT Introduction Rubber dam is essential for effective isolation of the root canal and operating field from salivary bacteria as well as protection of the airway. Rubber dam is easy to apply once the basic components and principles are understood. The most common reasons for not using rubber dam for a procedure were patients inconvenience and belief that it is unnecessary. The purpose of the present study was to evaluate the patients attitude to rubber dam after educating them about the use and benefits of rubber dam. Materials and methods A questionnaire containing 11 questions was circulated to 20 male and 20 female patients undergoing root canal treatment after explaining about the rubber dam. Information sought included attitude toward the current and anticipated use of rubber dam for a variety of endodontic treatments. Results A total of 24 patients out of 40 said that they had a pleasant experience and preferred its use next time. Around 30 patients felt that the use of rubber dam benefits both patient and the doctor. Conclusion There was not much of a difference in patient's attitude to rubber dam and mean application time. Those who had a good current experience preferred rubber dam use next time also.
