SamenvattingEr verschijnen steeds meer oorspronkelijke studies naar de veiligheid en effectiviteit van HIFU als salvagebehandeling van een lokaal recidief van het prostaatcarcinoom (Pca). Ook verschenen recentelijk meerdere systematische reviews (SR’s) over dit onderwerp. Deze review van SR’s geeft inzicht in de huidige stand van wetenschap en praktijk ten aanzien van salvage-HIFU van een lokaal recidiverend Pca. De meest recente en tevens grootste SR toont bij een lokaal recidief na bestraling geen verschil in overleving tussen HIFU en salvage radicale prostatectomie of salvagebestraling. De morbiditeit na salvage-HIFU was lager dan na een salvage radicale prostatectomie. In verband met de hoge morbiditeit bij een salvage radicale prostatectomie wordt deze zelden uitgevoerd. HIFU als salvagebehandeling kan daarom worden gezien als een veelbelovend alternatief. Dit geldt zeker voor patiëntengroepen bij wie leeftijd, ziektestadium en wens van de patiënt in de richting wijzen van afzien van agressievere therapieën, en bij wie het risico op bijwerkingen en vermindering van de kwaliteit van leven te hoog wordt geacht. De huidige evidence geeft voldoende aanleiding om, in afwachting van studies met hogere bewijskracht, HIFU als salvagebehandeling aan te bieden in plaats van androgeendeprivatietherapie, dan wel niet te behandelen.

Abstract The politics of land rights and low or high intensity protest in the twenty-first century has produced several land-related protesters with a variety of strategies. This study focuses on the challenges of urbanization as it affects the Kpaduma community in Abuja, Federal Capital Territory (fct), Nigeria. Kpaduma, an indigenous group, has a history of protest at various times with the government over its ancestral land. The last protest in 2016 brought massive destruction of settlements and forced displacement. Quite a number of works have been written on the land protests, with particular reference to their causes and consequences. However, the post-protests situation, particularly regarding the tense relationship and urbanization process in the locality, is yet to be sufficiently explored. This study investigates the state of relations between the government and Kpaduma as well as the urbanization processes in the country’s capital after the forced displacement of Kpaduma.

The history of chaos pertaining to acoustics is briefly reviewed from the first period-doubling experiments reported by Faraday (vibrated liquid layer) and Melde (string oscillations) to today's investigations on chaos in thermoacoustics, musical instruments, the hearing process, and ultrasonics. A closer analysis is given of the sound produced in a liquid by standing acoustic waves of high intensity.

OBJECTIVE: Patent foramen ovale is associated with paradoxical embolism (PE) and stroke. Hypercoagulable states, such as antiphospholipid syndrome (APS), can exacerbate PE by increasing clot formation. The aim of this study was to verify whether patients with APS and stroke present a right-to-left shunt (RLS) with greater frequency than patients with APS but without stroke. METHODS: Fifty-three patients with APS were tested for RLS using contrast-enhanced transcranial Doppler (cTCD): 23 patients had a history of stroke (Stroke Group) and 30 had no history of stroke (No-stroke Group). RESULTS: cTCD was positive in 15 patients (65%) from the Stroke Group and in 16 patients (53%) in the No-stroke Group (p=0.56). The proportion of patients with a small RLS (<10 high-intensity transient sign or HITS) and a large RLS (>10 HITS) was similar between the groups without significant difference. CONCLUSIONS: Our data do not support the theory that paradoxical embolism may play an important role in stroke in APS patients.

Veterans from Operation Iraqi Freedom (OIF), Operation Enduring Freedom (OEF), and the Vietnam era have sustained multiple injuries and disabilities as a result of their service, including Traumatic Brain Injury (TBI), Post-Traumatic Stress Disorder (PTSD), and depression. The use of outdoor recreation as a therapeutic modality for civilians with TBI has been found to correlate with greater self-awareness and positive affect, however, scarce literature exists regarding its use and outcomes for veterans. To explore this, a veteran from the Vietnam era reporting a history of depression who is involved with a white water kayaking group completed a series of surveys designed to measure its effect on perceived self-awareness and positive affect. The case study reported that involvement with the white water kayaking group improved his perceived self-awareness. The case study also reported the highest positive affect and lowest negative affect immediately after traveling down the river during a white water kayaking river trip. More research in this area is needed.
