The use of robotics is becoming widespread in healthcare. However, little is known about how robotics can affect the relationship with patients in epidemic emergency response or how it impacts clinicians in their organization and work. As a hospital responding to the consequences of the COVID-19 pandemic “ASST dei Sette Laghi” (A7L) in Varese, Italy, had to react quickly to protect its staff from infection while coping with high budgetary pressure as prices of Personal Protection Equipment (PPE) increased rapidly. In response, it introduced six semi-autonomous robots to mediate interactions between staff and patients. Thanks to the cooperation of multiple departments, A7L implemented the solution in less than 10 weeks. It reduced risks to staff and outlay for PPE. However, the characteristics of the robots affected their perception by healthcare staff. This case study reviews critical issues faced by A7L in introducing these devices and recommendations for the path forward.

Összefoglaló. A járvánnyal szembeni védekezés során mindennapossá vált a maszkhasználat, amely jelentős kihívást jelent a hétköznapi kommunikációs helyzetekben. Az egészségügy bizonyos területein korábban is jelen volt ez a probléma, azonban a most általános érvényű, folyamatos maszkviselés és védőruha-használat szimbolikus és gyakorlati értelemben is akadályként jelenik meg az orvos és a páciens között, ami több szinten nehezíti a hatékony kapcsolatteremtést és kommunikációt. Ezért szükségessé vált olyan egységes kommunikációs protokollok kidolgozása, amelyek segítik a bizalmi kapcsolat kiépítését az egészségügyben úgy, hogy az ellátók érzelmi elérhetősége és a biztonságos fizikai távolságtartás egyszerre valósuljon meg. Tanulmányunk célja, hogy a kurrens szakirodalom áttekintése révén összefoglaljuk a maszkviselés és a védőruha-használat hatásait az orvos-beteg kommunikációra és a kapcsolatban megjelenő bizalomra és empátiás folyamatokra, valamint fel kívánjuk hívni a figyelmet az egészségügyi személyzetet érő hatásokra is. Célunk továbbá, hogy a hazai és nemzetközi tapasztalatokat figyelembe véve olyan ajánlásokat fogalmazzunk meg, melyek segítségével javítható az egészségügyi dolgozók kommunikációja, azon keresztül pedig az ellátás legfontosabb szereplőinek, a betegeknek és az ellátóknak az elégedettsége a kapcsolattal. Orv Hetil. 2021; 162(23): 897–904. Summary. In the fight against the epidemic, the use of masks has become commonplace, which is a significant challenge in everyday communication situations. This problem has been present in some areas of healthcare in the past, but today’s universal use of masks and personal protective equipment (PPE) is both a symbolic and practical barrier between doctor and patient, making it difficult to establish an effective communication and connectivity on several levels. Therefore, it has become necessary to develop unified communication protocols that help building a relationship of trust in healthcare so that emotional availability of providers and safe physical distance are achieved simultaneously. The aim of our study is to summarize the effects of mask wearing and PPE usage on the physician-patient communication and also the trust and empathy processes that emerge within the relationship through the review of the current literature. We would also like to draw attention to the effects on healthcare staff. Our goal is also to formulate recommendations, taking into account domestic and international experience, that can be used to improve the communication of healthcare professionals, and through that, the satisfaction of the most important actors in care, patients and caregivers. Healthcare disturbance is a form of workplace violence against healthcare workers perpetrated by patients, their relatives, and gangs hired by them. It is a prevalent phenomenon in China, where evidence suggests that it impacts on the job satisfaction of healthcare workers. This study aims to examine the relationship between healthcare disturbance, surface acting as a response to emotional labour, and depressive symptoms in Chinese healthcare workers. The study adopted a cross-sectional design and used an online survey methodology. Data were collected from 418 doctors and nurses from one hospital in China. The results showed that frequency of healthcare disturbance was positively related to surface acting and depressive symptoms, respectively; surface acting was also positively related to depression, while deep acting showed no effect on symptoms of depression. Furthermore, surface acting in response to emotional labour mediated the relationship between healthcare disturbance and depressive symptoms. The results highlight the importance of preventing healthcare disturbance and of training healthcare staff in strategies for managing emotional demands in reducing depressive symptoms in Chinese healthcare staff.

RESUMOObjetivo: identificar o conhecimento de profissionais do corpo de bombeiros militar sobre as manobras de ressuscitação cardiopulmonar. Método: estudo quantitativo, descritivo e exploratório. Foi aplicado questionário em 41 profissionais com questões referentes aos aspectos conceituais e práticos sobre a PCR de acordo com as diretrizes da American Heart Association/2015. Para a análise estatística, foram utilizados os programas Stata 12.0 e WinPepi, versão 11.43. Resultados: prevaleceu o sexo masculino, idade média de 35 anos, com formação superior e mais de cinco anos de experiência. Sobre os aspectos conceituais, apenas a definição de “Tempo de ouro” apresentou acertos inferiores a 50%. Para os aspectos práticos, as variáveis sobre a postura e profundidade das compressões torácicas ficaram abaixo de 50% de acertos. A associação da idade e a relação entre erros e acertos das questões sobre os aspectos práticos foi estatisticamente significativa. Conclusão: revelou-se a necessidade de qualificação em busca da excelência no atendimento à vítima de PCR dada a sua natureza emergencial. Aim: The relationship between force and depth during manual chest compressions depends on the patient and on the dynamics with which the rescuer applies the force. Force-depth models with many fitting parameters have been proposed making physical interpretation complicated. The aim of this work was to design a simpler force-depth model, accommodating anticipated differences in compression and recoil phases. Materials and Methods: Force and acceleration signals were extracted from out-of-hospital-cardiac arrest (OHCA) defibrillator recordings (TVF&R, OR, USA), equipped with CPR technology. Compression depth and velocity signals were computed from acceleration. We analyzed intervals of 20-s within the 1st min of chest compressions. Our model decomposes the applied force as the sum of an elastic and a damped term, considering different damping coefficients for the compression and recoil phases. Coefficient of elasticity was calculated at the instant of maximum compression depth (null velocity) and damping coefficients at the instants of maximum compression and recoil velocities. The estimated depth signal is shown in the figure. The goodness of the model was assessed through the determination coefficient R 2 . Results: We analyzed 1,074 compressions from 30 OHCA recordings. Median (IQR) compression depth was 4.6 (4.0-5.4) cm; compression rate was 107 (102–113) cpm; coefficient of elasticity was 100.67 (78.95–125.01) N/cm; compression damping coefficient was 2.57 (1.84–3.29) N/(cm/s) and recoil damping coefficient was 3.59 (2.58–4.90) N/(cm/s). Median R 2 was 0.993 (0.984–0.996). Conclusions: This model, derived using fewer parameters, could help with the interpretation of the mechanical properties of the chest during CPR. It may also be useful for the assessment of inter-patient differences with age, sex, and body constitution, as well as of the evolution of elasticity and damping of patient’s chest during the course of resuscitation.

Este estudio examinó el papel mediador del componente de uso de la emoción (UoE), de la Escala de inteligencia emocional, en la relación entre extraversión (EX) y conciencia dentro de los Cinco Grandes factores de personalidad, y las dimensiones de trabajar en exceso (WE) y trabajar compulsivamente de la Escala holandesa de adicción al trabajo. Junto con este objetivo, se recopilaron datos del personal sanitario, en su mayoría enfermeras, que trabajaba en hospitales privados que prestan servicios en Estambul. Los hallazgos indicaron que WE tuvo un efecto directo no significativo sobre EX (β = −0.358; p > .05), mientras que el efecto indirecto de WE sobre EX a través de UoE fue significativo (β = 0.147; p < .05). Este estudio revela que UoE actúa como un mediador completo en el efecto de WE sobre EX. This study examined the mediator role of Use of Emotion (UoE) component of Emotional Intelligence (EI) Scale in the relationship between Extraversion (EX) and Conscientiousness (CO) within the Big Five (BF) personality factors, and the dimensions of Working Excessively (WE) and Working Compulsively (WC) of Dutch Work Addiction Scale (DUWAS). Along with this aim, data were collected from healthcare staff, most of who were nurses, working at private hospitals serving in İstanbul. The findings indicated that Working Excessively (WE) had a non-significant direct effect on Extraversion (EX) (β = -.358; p > .05) while the indirect effect of Working Excessively (WE) on Extraversion (EX) through Use of Emotion  (UoE) was significant (β = .147; p < .05). This study revealed that Use of Emotion (UoE) acted as a full mediator in the effect of Working Excessively (WE) on Extraversion (EX).
